Health research in Papua New Guinea.

The Papua New Guinea Institute of Medical Research (PNGIMR) is one of the most respected health research institutions in the developing world, and its studies of the local health problems of PNG have consistently had international relevance. This article examines the structural and philosophical factors that have enabled the success of the PNGIMR, and presents the PNGIMR approach to research as a potential model for other disease-endemic countries. An overview of PNGIMR research into malaria and filariasis is given with selected examples as an introduction to a Trends in Parasitology series on…

Beta thalassaemia in Papua New Guinea.

The treatment of homozygous beta thalassaemia in Papua New Guinea during the period 1980-85 is reviewed. The results showed that the average pre-transfusion haemoglobin was 4.5 g/dl. Evidence of hypersplenism, based on spleen size or annual blood requirement, was present in all patients by age 3-4 years. Mean age of death was less than 9 years. These results are compared with previously published results of therapies from countries with more intensive treatment regimes. It is concluded that supertransfusion and hypertransfusion regimes both offer significant improvements, in duration and quali…

Papua New Guinea. Yaws problems assessed.

The present status of yaws in Papua New Guinea cannot be determined because of the lack of data. Because yaws was deemed under control as a result of the nationwide total mass treatment campaign in the late 1950s and limited regional effort in the late 1970s, the health department lost interest in yaws and pursued other diseases. Individual reports from certain areas of Papua New Guinea have provided the only information on yaws since 1978. These reports concentrated on outbreaks on Karkar Island in the Madang Province and indicated the possibility of a recurrence of yaws. It was not until ear…

Alpha thalassaemia in Papua New Guinea.

Haemoglobin Bart's was detected in cord blood samples from 81% of 217 infants born in Madang on the north coast of Papua New Guinea. Analysis of the alpha globin genes of 30 infants and adults from the same region showed that all but 3 were heterozygous or homozygous for the deletion form of alpha + thalassaemia. None of 18 cord blood samples from infants born in Goroka in the Eastern Highlands Province had haemoglobin Bart's, and in each case the alpha globin genes were normal. Preliminary geographical and linguistic analyses of both groups suggest that the prevalence of alpha thalassaemia ma…

Ocular leprosy in Papua New Guinea.

The eyes may be damaged in leprosy by three pathological processes; by direct bacillary invasion, secondary to the involvement of the Vth and VIIth cranial nerves leading to corneal anaesthesia and lagophthalmos respectively, and by sensitization to Mycobacterial antigen or immune complexes during type II reactions. The prevalance of ocular leprosy varies widely throughout the world, depending on a number of factors, including the type of disease which is most prevalent, the duration of the disease, the ophthalmological facilities available, and possibly the race of the patient. Cardiovascular disease in Papua New Guinea.

The chest X-ray films of 251 patients with cardiovascular lesions, collected in the major hospitals of Papua New Guinea, were reviewed. Cor pulmonale (18%) and rheumatic heart disease (16%) were the predominant cardiac disorders, and cor pulmonale was the most common cause of cardiac failure (58%). Cardiomegaly or cardiac failure of unknown cause, possibly due to cardiomyopathy or myocarditis, made up 9% of the group and may be more important than has been thought previously. Aortic-arch calcification typical of atheroma was present in 21% of the patients, and is thus quite common, even though…

Essential tremor in Papua, New Guinea.

The clinical features of 175 cases of essential tremor are related. This disorder is prevalent among a population of the Eastern Highlands of Papua New Guinea. It affects predominantly women in middle and old age; only 27 per cent of the cases were males. The disorder is slowly progressive and significant disability appears in elderly women when the trunk muscles are involved. Epidemiological studies have shown that the presence of tremor can be correlated with linguistic distinctions between high and low prevalence populations. Although only 30 patients reported a first degree relative with t…

Melioidosis in Papua New Guinea and Oceania.

Melioidosis has only been sporadically reported throughout Melanesia and the Pacific region since the first report from Guam in 1946; theref … l reservoirs have been described from the rural district of Balimo in the Western Province of Papua New Guinea and from the Northern Province of New Caledonia. In both these locations the incidence of disease is similar to that described in tropical Australia and Burkholderia pseudomallei isolates are also phylogenetically linked to Australian isolates. Sero …

The pigbel story of Papua New Guinea.

Enteritis necroticans (EN), known as pigbel in Papua New Guinea (PNG), may be the important predisposing lesion to mid-gut volvulus, jejunal and ileal ileus and other forms of small bowel strangulation in communities where protein deprivation, poor food hygiene, epochal meat feasting and staple diets containing trypsin inhibitors co-exist. Such human habitats occur in Africa, Central and South America, western Pacific, Asian and south-east Asian cultures. Isolated outbreaks of necrotizing enteritis have been reported from Uganda, Malaysia and Indonesia but as yet no systematic epidemiological …

The monitoring of foodhandlers in Papua New Guinea.

In early 1987 guidelines were re-enforced for pre-employment medicals and 6-monthly health checks on foodhandlers in the Ok Tedi mining project in Papua New Guinea. The health monitoring program was stepped up as a result of two typhoid cases imported from the highlands and catering subcontractors failing to comply with the necessary pre-placement medicals for foodhandlers. Highlanders made up 28% (49/174) of the Ok Tedi catering department's workforce in 1987. The initial screen of 155 foodhandlers and 85 non-foodhandlers in February 1987 showed 6-7% of each group asymptomatically harbouring …

[New Guinea and Australia: their common pathology].

New Guinea and Australia have been joined together during several hundred millions of years. Inhabited by men 40,000 years ago, these populations (papous and aborigenes) have been separated of the others during several millenaries. The first invaders brought with them plasmodiums, Necator americanus, Wuchereria bancrofti and Dengue virus. Brugia malayi, because lack of primates, took non root in New Guinea; the virus of Murray Valley encephalitis is autochthonous; the high frequency of Burkitt lymphoma in New Guinea is, perhaps, a racial feature.
