Laying the foundation for Real-world evidence studies: a case study from Newfoundland and Labrador.

The Janssen and Newfoundland and Labrador Health Innovation Partnership (JANL-HIP) was established to carry out Real-World Evidence (RWE) projects to generate evidence about disease pathways, healthcare delivery, the effects of clinical interventions. Doing so will support and influence clinical decision-making in Newfoundland and Labrador (NL). This case study describes the foundational elements necessary for a real-world evidence generation project in NL and may provide learning for the effective execution of real-world studies in other jurisdictions. It uses an ongoing project in psoriatic …

Characteristics of Interval Colorectal Cancer: A Canadian Retrospective Population-Level Analysis from Newfoundland and Labrador.

Interval colorectal cancers (I-CRCs) arise during the interval time period between scheduled colonoscopies. Predicting which patients are at risk of I-CRCs remains an elusive undertaking, but evidence would suggest that most I-CRCs arise from lesions missed on index endoscopy. The procedural factors that lead to missed lesions are numerous and lack consensus in the literature. In Canada, the province of Newfoundland and Labrador has the highest incidence of CRCs. In this study our aim was to examine I-CRCs (3-60 months after last colonoscopy) in NL through a population-level analysis covering …

Prenatal Education for Pregnant Parents on Opioid Agonist Therapy in Newfoundland and Labrador: A Qualitative Study.

Women on opioid agonist therapy often enter labour with limited information about neonatal withdrawal, newborn care, and hospital routines. Stigma and fear of child protection involvement can further limit communication. This study explored parents' information needs regarding opioid agonist therapy and newborn care. Seven virtual in-depth interviews were conducted between December 2020 and April 2021. Participants described inconsistent prenatal education on neonatal opioid withdrawal syndrome, hospitalization, and newborn care. Concerns about NICU admission, child protection involvement, dos

Establishing a regional drug profile in Newfoundland and Labrador, Canada, using data from acute drug deaths.

INTRODUCTION: Drug deaths are rising in Canada and are driven by polydrug toxicity (that is toxicity resulting from multiple drugs belonging to different drug classes). Addressing polydrug toxicity requires the establishment of regional drug profiles so that appropriate harm reduction policies can be implemented. METHODS: Using toxicology data from individuals who died from acute drug toxicity, we established regional drug profiles for the Canadian province of Newfoundland and Labrador. Medical examiners determined which drugs contributed to each acute death. Counts were described to establis…

Peromyscus maniculatus, a possible reservoir host of Borrelia garinii from the Gannet Islands off Newfoundland and Labrador.

Thirty-five deer mice, Peromyscus maniculatus, were trapped on Gannet Cluster 2 (GC-2), one of a group of islands numbered by convention in the Gannet Island Archipelago, and examined for ectoparasites. One species each of Acari (Ixodes uriae) and Siphonaptera (Orchopeas leucopus) were recovered. Samples of mice favored males to females (3.4∶1). Twenty-nine percent (10) of the mice were free of ectoparasites. Males were more heavily parasitized than females when both parasites were considered. No ticks were recovered from the female mice, while the males that were parasitized carried adult Ixo

Knowledge of and adherence to radiographic guidelines for low back pain: a survey of chiropractors in Newfoundland and Labrador, Canada.

BackgroundLow back pain (LBP) rarely requires routine imaging of the lumbar spine in the primary care setting, as serious spinal pathology is rare. Despite evidence-based clinical practice guidelines recommending delaying imaging in the absence of red flags, chiropractors commonly order imaging outside of these guidelines. The purpose of this study was to survey chiropractors to determine the level of knowledge, adherence to, and beliefs about, clinical practice guidelines related to the use of lumbar radiography for LBP in Newfoundland and Labrador (NL), Canada.MethodsA cross-sectional survey…

Quantifying SARS-CoV-2 Omicron variant spread and the impact of non-pharmaceutical interventions in Newfoundland and Labrador, Canada.

The highly transmissible Omicron variant of SARS-CoV-2 caused many infections in Newfoundland and Labrador, Canada, and the fraction of infections that were unreported varied as PCR testing capacity was exceeded and eligibility rules changed. Due to these inconsistencies in the testing rate, we developed a mechanistic model that was calibrated to serological data (Dec 2021-May 2022) to estimate underreporting and understand the impact of non-pharmaceutical interventions on transmission. Our model considers the epidemiology of SARS-CoV-2 spread, natural and vaccine-derived immunity, and the boo…

Prevalence of dental caries and fluorosis in seven- to 12-year-old children in northern Newfoundland and Forteau, Labrador.

In 1993, a cross-sectional oral health survey of 739 randomly selected seven- to 12-year-old Canadian school children was conducted in Forteau (Labrador), Roddickton (northern Newfoundland), and Flower's Cove (northern Newfoundland) to determine the prevalence of dental caries and fluorosis. Children in grades two through six (n = 838) were selected to participate in the survey, which achieved a response rate of 88.2 per cent. Caries prevalence was estimated using modified World Health Organization (WHO) criteria (non-cavitated carious pits and fissures were coded separately from cavitated car

Changing contact patterns in Newfoundland and Labrador, Canada in response to public health measures during the COVID-19 pandemic.

The provincial government of Newfoundland and Labrador, Canada implemented a contact tracing program as part of a containment strategy during the COVID-19 pandemic. A high proportion of cases were detected and contact traced, and our analysis provides insights into secondary case distributions and contact patterns in Newfoundland and Labrador. We used a heuristic approximation of secondary cases to account for ambiguities in who infected whom. These approximate values provide an empirical distribution of secondary cases. These distributions are compared against the stringency of public health …

Importation models for travel-related SARS-CoV-2 cases reported in Newfoundland and Labrador during the COVID-19 pandemic.

During the COVID-19 pandemic, the World Health Organization updated guidelines for travel measure implementation to recommend consideration of a region's specific epidemiological, health system and socioeconomic context. As such, travel measure implementation decisions require region-specific data, analysis and models to support risk assessment frameworks. From May 2020 to May 2021, the Canadian province of Newfoundland and Labrador (NL) implemented travel measures that required self-isolation and testing of individuals returning from out-of-province travel. We found that during the pandemic t…
