Essential genetics for the obstetrician/gynecologist.

The science of genetics has become increasingly important in the practice of medicine. This article reviews the practical, clinical aspects of genetics relevant to obstetrics and gynecology. The basic fundamentals of molecular biology techniques currently used in DNA diagnostic tests are discussed.

Listeriosis revisited: the role of the obstetrician.

The clinical course and outcome is described of three pregnancies complicated by listeriosis encountered during a 6-month period. All three ended in preterm labour characterized by the passage of meconium by the fetus. Two babies died in the early neonatal period and the third survived with neurological handicap. Although the diagnosis of listeriosis was made only after delivery, all the mothers had a pyrexial illness before delivery. A high index of suspicion is needed for the early detection and prompt treatment of this life-threatening perinatal infection. Effective advice concerning preven

The obstetrician's responsibility in infant mortality.

Although infant mortality has been remarkably reduced, stillbirth and neonatal death rates have been improved very little. Efforts at lowering the fetal death rate must be directed to those conditions affecting the fetus during labor or immediately afterward. Prevention of premature labor and better care of the premature infant during labor and the neonatal period offer hope of a greater salvage of premature infants. Proper environment and trained personnel are necessary. Spontaneous delivery is safest for the infant. Difficult operative procedures are associated with a high incidence of birth

Retinopathy of prematurity: A refresher for obstetricians.

Advances in neonatal care improved the survival of many preterm infants, but also increased the incidence of retinopathy of prematurity (ROP … managing children with ROP. A portion of these guidelines is particularly applicable to practicing obstetricians who, along with the pediatricians, are often the primary physicians for the affected families. The aim of this paper is to present a comprehensive overview of the epidemiology, etiology, manifestations, prevention, and management of ROP. In the last …

Anal incontinence and the obstetrician-gynecologist.

OBJECTIVE: To gather, synthesize, and present useful scientific information concerning the anal continence mechanism that will aid obstetrician-gynecologists in managing vaginal birth and evaluating women with anal incontinence not caused by disruption of the external anal sphincter. DATA SOURCES: Sources included a Medline search and reference lists of relevant articles and standard textbooks. METHODS OF STUDY SELECTION: Articles were identified that contained scientific data on the pathophysiology of anal incontinence, the influence of vaginal delivery on the continence mechanism, and ther

Perinatal infections: the obstetrician's viewpoint.

The obstetrician's role is to recognize and treat maternal systemic infections which may spread transplacentally to the fetus, and to ensure that the lower genital tract is kept free from pathogenic organisms, particularly during the last trimester of pregnancy. In some cases the obstetrician can reduce the likelihood of premature rupture of the membranes by inserting a cervical suture. Where this has not been possible conservative management should be used to keep the fetus in utero before the 34th week and more active after this time. In trapartum infections can be reduced to a minimum by ke

Why blame the obstetrician? A review.

Because of superficial thinking obstetricians have been blamed unjustifiably for causing brain damage, cerebral palsy, mental subnormality, congenital torticollis, and facial palsy. It is essential to look behind obvious difficulties in labour, such as abnormal presentation or anoxia, to the underlying causes, which are often genetic or social, or concern other prenatal factors.

Obstetricians' attitudes towards 'isolated' oligohydramnios at term.

Oligohydramnios at term in the absence of maternal and fetal complications poses a dilemma in management. It is one of the major indications for antenatal surveillance and induction of labour. There is a growing body of evidence that oligohydramnios in the absence of maternal or fetal risk factors ('isolated' oligohydramnios) is not associated with an adverse perinatal outcome. In this study, we surveyed the management of isolated oligohydramnios at term in a tertiary referral maternity unit. Out of 92 women who were scanned at term, 22 (24%) women had isolated oligohydramnios. A greater tende

Hantavirus and Pregnancy: What Obstetricians Need to Know.

A recent cluster of cases of severe respiratory illness due to Andes hantavirus (ANDV) infection highlights the need for obstetricians to understand hantavirus infection, including its implications during pregnancy. We have summarized English-language reports indexed in PubMed that describe hantavirus infection in pregnancy. Although we did not identify any published cases of ANDV infection in pregnancy, reports of ANDV infection among nonpregnant individuals as well as other types of hantavirus infection in pregnancy can help inform diagnosis, management, and prevention in pregnancy. Patients…

Analysis of obstetricians' decision making on CTG recordings.

Interpretation of cardiotocogram (CTG) is a difficult task since its evaluation is complicated by a great inter- and intra-individual variability. Previous studies have predominantly analyzed clinicians' agreement on CTG evaluation based on quantitative measures (e.g. kappa coefficient) that do not offer any insight into clinical decision making. In this paper we aim to examine the agreement on evaluation in detail and provide data-driven analysis of clinical evaluation. For this study, nine obstetricians provided clinical evaluation of 634 CTG recordings (each ca. 60min long). We studied the …

Obstetricians' attitudes and beliefs regarding umbilical cord clamping.

OBJECTIVE: Although delayed umbilical cord clamping has been demonstrated to reduce the incidence of intraventricular hemorrhage and neonatal sepsis, and decrease the need for neonatal transfusions (without affecting cord pH, Apgar scores or the need for phototherapy), the extent to which this practice is being employed is unknown. We conducted a survey of US obstetricians to assess their attitudes and beliefs about cord clamping. STUDY DESIGN: Questionnaires were randomly mailed to members of the American College of Obstetricians and Gynecologists (ACOG), and the Collaborative Ambulatory Res…
