Open reduction and screw fixation of mallet fractures.

Twelve patients with mallet fractures treated by open reduction and internal fixation with small screws were reviewed at an average of 31 months after surgery. The indication for surgery was a fracture involving more than one-third of the distal phalanx articular surface or with subluxation of the distal interphalangeal joint. Loss of reduction occurred in one patient and in another one screw loosened slightly without loss of reduction. There were no nail deformities, infections, or secondary procedures. The mean range of motion was from 6 degrees (range, 0-30 degrees ) (extensor lag) to 70 de

Delayed open reduction and fixation of ankle fractures.

Because of clinical circumstances in the period from 1970-1980, 26 consecutive ankle fractures were treated by delayed (14-31 days after injury) open reduction and internal fixation. Follow-up examination (two to 12 years) was obtained in 25 patients. Based on clinical performance index, outcome was judged satisfactory in 17 patients (68%). Seventeen (68%) had an anatomic reduction, and 15 (88%) of these 17 had a satisfactory result. Compared to a group of 26 similar patients treated within 24 hours after injury, the major effect of delay was an adverse influence on the probability of achievin

Open v closed reduction of the fractured nose.

A stepwise study of nasal fractures includes an initial investigation that was carried out to discover why so many patients were awaiting rhinoplasty, which led us to find a 30% to 40% failure rate in nasal manipulation operations. We then studied the mechanics of nasal fracture by striking fresh cadaver noses. We found that if the nasal bones were deviated by more than half the bridge width, there was a concomitant C-shape fracture of the bony and cartilaginous septum. Two trials were performed, and patients with nasal deviation after trauma were allocated to either manipulation or manipulati

Open Reduction and Pinning of Lateral Condyle Fractures.

Lateral condyle fractures are the second most common elbow fracture in pediatric patients. Unlike supracondylar humerus fractures, these injuries involve the articular surface and present unique challenges. These injuries are graded based on the degree of displacement and this dictates your treatment. Both closed and open techniques are appropriate depending on the degree of displacement. Our preferred technique for open reduction and internal fixation is described and illustrated in the accompanying video. Key Concepts•Lateral condyle fractures are common elbow injuries in the pediatric popul

Open Intrameniscal Examination and Reduction of Depressed Tibial Plateau Fracture.

In the Schatzker classification, approximately 60.5% of tibial fractures can potentially involve depression fractures of the plateau, on average in types II, III, and IV, and are particularly prevalent among the elderly population. A significant challenge in managing these fractures is achieving optimal visualization of the depressed fracture segments during surgical intervention. Traditional management techniques include intraoperative fluoroscopy, submeniscal arthrotomy, and arthroscopic-guided reduction. However, each of these methods presents specific drawbacks: exposure to radiation, the

Malocclusion after open reduction of midfacial fracture: a case report.

Malocclusion is a serious complication of open reduction surgery for facial fractures. It is often caused by the lack of adequate consideration for the occlusal relationship before the trauma and intermaxillary fixation during the operation. This is a case report of postoperative malocclusion that occurred in a patient with a midfacial complex fracture.

[Effectiveness of open reduction and internal fixation for Bosworth fracture].

OBJECTIVE: To explore the operative methods and effectiveness of open reduction and internal fixation for Bosworth fracture. METHODS: Between January 2005 and January 2012, 6 patients with Bosworth fractures caused by sprain were treated. There were 4 males and 2 females with an average age of 45.8 years (range, 24-73 years). The time from injury to operation was 1-5 days (mean, 1.8 days). They were all closed fractures. According to Lauge-Hansen classification, 6 cases were classified as supination-external rotation type. The surgical treatments included open reduction and internal fixation

Capitellum Fracture Open Reduction and Internal Fixation With Headless Screws.

Partial articular fractures involving the capitellum represent an injury to the lateral column of the distal humerus. Capitellum open reduction and internal fixation restores this portion of the distal humerus articular surface and biomechanical integrity to the lateral column. This video demonstrates open reduction and internal fixation of a type I capitellar fracture with headless compression screws through an extensile lateral approach.

Open reduction of acetabular fracture in pregnancy. A case report.

A 24-year-old woman in her 20th week of pregnancy sustained a severely displaced acetabular fracture. Skeletal traction failed to achieve reduction. Open reduction and internal fixation were performed through an extended iliofemoral approach without affecting the pregnancy. At full term, she had a normal delivery. Four years after the injury, the patient had a pain-free hip with full range of motion and a healthy child. The potential benefit or danger of surgery to the patient and her pregnancy was thoroughly considered.

Open ankle fractures. The indications for immediate open reduction and internal fixation.

Twenty-two patients with open ankle fractures or fracture-dislocations were treated with irrigation and debridement, reduction, and immediate stable internal fixation at an average of six hours from initial evaluation. There were 13 women (59%) and nine men (41%), having an average follow-up period of 32 months (range, five to 111 months). There were six Grade I (27%), 15 Grade II (68%), and one Grade III (5%) injuries. Fractures also were classified according to the Danis-Weber scheme (Type A [three cases]; Type B [eight]; Type C [11]). Excellent results were achieved in 14 patients (64%); go

Open reduction and internal fixation of fractures of the acetabulum. Results in 163 fractures.

Management of displaced fractures of the acetabulum represents one of the greatest challenges in fracture surgery. This study reports intermediate term results for a single surgeon of 163 fractures in 161 patients treated by open reduction and internal fixation. The mean length of followup was 3.7 years. The most common fracture patterns treated were both column (67), transverse/posterior wall (32), posterior wall (14), and T shaped (12). The surgical approaches chosen were ilioinguinal (86), Kocher-Langenbeck (58), and extended iliofemoral (26) with 7 patients having sequential or simultaneou

Open reduction and internal fixation for intraarticular fracture of metacarpal head.

Intraarticular fracture of the metacarpophalangeal (MP) joint presents complex problems related to the sophisticated functional aspects of the hand. Injury to the metacarpal head may have a severe effect on hand function but few studies have investigated the management of this condition. In this study, we applied open reduction and internal fixation for the displaced fracture of the metacarpal head and report the clinical and radiographic outcomes of our experience. Thirteen patients (12 men, 1 woman; mean age 21 years) were included in this study, and medical records and radiographs were revi

Open Versus Arthroscopic Reduction for Tibial Eminence Fracture Fixation in Children.

BACKGROUND: Stiffness is a common complication following surgically treated tibial eminence fractures. Fractures can be addressed with either open reduction and internal fixation (ORIF) or arthroscopic reduction and internal fixation (ARIF). We sought to evaluate the effects of surgical approach and other modifiable perioperative factors on postoperative arthrofibrosis. We hypothesized that ARIF would result in a lower risk of arthrofibrosis. METHODS: We retrospectively reviewed the records of all patients aged 18 years and below treated for tibial eminence fractures at our institution from 1
