UVEAL MELANOMA EXTENSION TO THE OPTIC CHIASM.

PURPOSE: Case report describing a patient who developed intracranial extension of a uveal melanoma through the optic nerve. METHODS: We reviewed the patient's medical history and images. A 41-year-old woman who was blind in one eye had a uveal melanoma that extended through the optic nerve into the optic chiasm and involved the hypophysis. The patient then developed metastasis. RESULTS: The patient developed uveal melanoma extension into the optic chiasm through the optic nerve resulting in a visual field defect in the fellow eye. CONCLUSION: Uveal melanoma extension through the optic nerve…

[Expansive processes in the optic chiasm region].

81 patients with expansive lesion in the optic chiasm region were reviewed. Pituitary adenomas were diagnosed in 37 patients: 5 with acromegaly, 2 with prolactinoma and the remaining 30 with hormone inactive adenomas. Meningiomas were diagnosed in 39, craniopharyngiomas in 4 patients and chordoma in one patient. The most common symptom was visual disturbance in 86%, but headache, hormonal disturbance occurred as well. All patients with adenomas were endocrinologically evaluated after detailed ophthalmologic and neurologic examinations. 28 patients were transcranially operated, 5 transsfenoidal…

Optic nerve and chiasm gliomas in children.

In a 30 years period, 42 children were admitted for optic nerve and chiasm gliomas; 20 patients aged less than 5 years and 22 between 6 and 16 years. Duration of symptoms ranged from 13.15 months in children aged less than 5 years, to 10.18 months in patients aged more than 6. Onset of symptoms were characterized by visual impairment and/or proptosis in 73.8%, diencephalic disorders in 16.6% and intracranial hypertension in 9.5%. All patients underwent skull X-rays; 31 had a PEG taken and 12 a CT scan. Surgery was prompted in 27 patients, 2 of which died in postoperative course. Radiotherapy w

Isaac Newton's description of the optic chiasm.

Sir Isaac Newton is widely regarded as one of the greatest scientific minds in history, with seminal contributions across mathematics, physics-particularly optics-and the formulation of the law of universal gravitation. Less well known, however, is his extraordinary and rarely recognized contribution to the field of neurology: the early conceptualization of fiber decussation within the optic chiasm. Through his studies on light and vision, Newton proposed that optic nerve fibers cross at the chiasm-a hypothesis that would later be anatomically confirmed and provided the basis for understanding…

Ultrasound Assessment of the Fetal Optic Chiasm.

This article reviews the literature on different methods of prenatal ultrasound visualization of the optic chiasm (OC) and its applications. Prenatal imaging of the OC is feasible from 19 to 37 weeks of gestation. Evaluation of the OC has been shown crucial in differentiating isolated agenesis of the septum pellucidum from septo-optic dysplasia. Multiple methods can be applied for imaging of the OC, including three-dimensional and two-dimensional ultrasounds in different views, as well as color Doppler. According to the literature, both transabdominal and transvaginal routes produce equally ac…

Langerhans cell histiocytosis of the optic chiasm.

Langerhans cell histiocytosis (LCH) is a rare disease with clonal proliferation of dendritic histiocytes forming a pseudotumoral growth. LCH occurs most frequently in infancy or early childhood. It can present either as a localized self-limiting form or as a diffuse form with multisystem involvement. Herein an unusual case of LCH involving the optic chiasm is presented. To the authors' knowledge, this is the first reported case of LCH presenting with visual disturbance. The clinical, radiological and histopathological findings are discussed.

Magnetic resonance imaging of the optic chiasm.

Magnetic resonance imaging (MRI) has become an important diagnostic modality for the evaluation of patients with chiasmal or perichiasmal disease. In this review, normal chiasmal anatomy and various pathologic conditions will be illustrated to demonstrate the advantages of MRI over computed tomography (CT). MRI is the neuroimaging method of choice in chiasmal evaluation, but sometimes it may be complemented by CT scanning.

Guidance of retinal fibers in the optic chiasm.

At the optic chiasm of vertebrates, there occurs a sharp divergence in the destination of retinal ganglion cell axons as they are on their way to more central targets. Recent studies in the mouse indicate that the growth cones of each set of fibers diverge close to the midline of the optic chiasm, at the borders of a palisade of immature radial glia. Static and dynamic views of retinal growth cones in this decision region reveal that extensive exploratory behavior and selective retraction of parts of the growing tips of uncrossed fibers, in response to cellular cues at the midline, is a major …

A rare choristoma of the optic nerve and chiasm.

Two patients with a heretofore undescribed unilateral choristomatous malformation of the intracranial optic nerve and chiasm underwent craniotomy for partial resection of the optic nerve in the belief that the tumor was either a glioma or a meningioma. Visual acuity in the affected eye was reduced to 20/200 in one patient and to finger counting in the other. Chiasmal involvement in the first case was associated with a superotemporal field defect in the opposite eye. Ophthalmoscopy disclosed atrophic nerve heads in both affected eyes. Heteroplastic masses of smooth muscle and adipose tissue rep

Glioblastoma in the optic chiasm: A case report.

Malignant optic gliomas are an uncommon pathology, with around 67 cases reported worldwide in the literature. We present the case of a 77-year-old-male with a two-month history of progressive vision loss, ultimately leading to bilateral blindness. The initial clinical suspicion was a non-inflammatory ischemic optic neuropathy. Stereotactic biopsy was performed on the optic chiasm, and the histopathological diagnosis was confirmed as Glioblastoma.

Developmental abnormalities of the optic nerve and chiasm.

Developmental anomalies of the optic nerve are an important and growing cause of lifelong visual handicap and they are often associated with systemic abnormalities. This review focuses on the ocular and systemic aspects of developmental anomalies arising from defects of fetal fissure closure and retinal ganglion cell development, and covers some other optic-disc anomalies that have systemic significance.

Reactive lymphohistiocytosis with recurrence in the optic chiasm.

Histiocytic infiltration of the optic chiasm is rare. We report a patient with a seizure disorder on anticonvulsant therapy, who developed systemic reactive histiocytosis. Treatment with splenectomy, corticosteroids, and anticonvulsant medication change resulted in clinical remission. The patient later developed recurrent lymphohistiocytosis restricted to the optic chiasm. Extensive re-evaluation yielded no evidence for infectious or malignant etiology. Radiation therapy and withdrawal of anticonvulsant therapy resulted in clinical remission. We conclude that both phenytoin and phenobarbital m…

Gliomas of the optic nerve and chiasm in children.

This paper reviews the clinical features, management, and prognosis of patients with gliomas of the optic nerves and chiasm who have received treatment at the Royal Children's Hospital, Melbourne, between 1954 and 1977. The relationship of this tumour to other neoplasms in the region is discussed, and emphasis is placed on the natural history of the disease. The controversy concerning the exact nature of this lesion is summarized, and an attempt is made, in the light of this evidence and from the reviewed series, to formulate an appropriate overall plan of management of these patients.
