The clinical profile of optic neuritis. Experience of the Optic Neuritis Treatment Trial. Optic Neuritis Study Group.

The baseline characteristics of 448 eligible patients entered into the Optic Neuritis Treatment Trial are described in an effort to summarize the clinical profile of acute optic neuritis. A total of 77.2% of the patients were women. Mean age was 31.8 years. Pain accompanied the visual loss in 92.2% of cases. The optic disc appeared swollen in 35.3% of the patients and normal in 64.7%. A wide variety of visual field defects were present. Abnormalities in asymptomatic fellow eyes were noted, particularly on perimetry. Magnetic resonance imaging showed changes consistent with demyelination of the…

Pediatric Optic Neuritis.

Optic neuritis is rare in children in comparison to adults, but accounts for approximately 25% of pediatric acute demyelinating syndromes. Features of pediatric optic neuritis that differ from adults include a higher rate of bilaterality, poor visual acuity on presentation, and papillitis. Diagnostic work-up includes brain magnetic resonance imaging, lumbar puncture, and blood tests to exclude infectious and inflammatory disorders. Pediatric optic neuritis may occur following infection or vaccination, or in association with a systemic demyelinating process such as acute disseminated encephalom…

Masquerading optic neuritis.

A 54-year-old woman presented to the ophthalmology emergency department with a 10-day history of blurred vision. The best-corrected visual acuities and Ishihara colour vision were bilaterally reduced with a left relative afferent pupillary defect. Slit-lamp examination was otherwise normal. Retrobulbar optic neuritis (ON) was presumed as she had suffered with this previously and was known to have multiple sclerosis (MS). She was recalled the following week for visual field (VF) testing, which was not available at the time of presentation. VFs demonstrated an incongruous left homonymous hemiano…

[Optic nerve neuritis].

Optic nerve neuritis is one of the most important differential diagnoses of visual loss in young and middle aged adults. The prognosis in terms of functional outcome is generally good. The diagnosis of optic neuritis is clinical. Steroids can reduce the recovery time but do not affect the long-term functional outcome. Magnetic resonance imaging (MRI) is the most important investigation for assessing an associated risk of multiple sclerosis. Optical coherence tomography (OCT) contributes additional details on the course and functional outcome of optic neuritis. In the future OCT may additionall…

Lyme optic neuritis.

Lyme optic neuritis (ON) is a rare disease and only a few cases have been reported. We describe two cases of isolated Lyme ON, one with recurrence 9 months after the appearance of initial symptoms. Diagnosis criteria for multiple sclerosis and neuromyelitis optica were not met. The etiological diagnosis was based on European case definition criteria for neuroborreliosis. Both patients had positive serum and cerebrospinal fluid serology, a positive intrathecal anti-Borrelia antibody index, and a good outcome on ceftriaxone. Specific diagnosis of Lyme ON is important since improvement of visual …

Ehrlichiosis optic neuritis.

PURPOSE: To describe a case of ehrlichiosis optic neuritis. DESIGN: Single observational case report. METHODS: A 41-year-old woman with symptoms and clinical and imaging signs consistent with optic neuritis presented to a tertiary care academic center for comprehensive neuro-ophthalmic evaluation. Main outcome measures included preoptic and postoptic neuritis polyvalent ehrlichiosis titers and magnetic resonance imaging (MRI) of orbits with gadolinium. RESULTS: Ehrlichiosis titers drawn 11 days before onset of eye symptoms were negative. Titers drawn 7 days after symptoms began were positiv…

Cytomegalovirus optic neuritis.

Cytomegalovirus (CMV) optic neuritis is a rapidly blinding disease. Untreated eyes have a final visual acuity of no light perception. Treated eyes maintain visual acuity of approximately 20/100. Relapse occurs often in subjects maintained on single-dose ganciclovir and is controlled by double-dose ganciclovir. Central visual field testing is indicated to detect indolent cases of optic neuritis, because relapse can be silent with stable visual acuity and fundus appearance. Survival of patients with CMV optic neuritis is comparable to that of a matched group having CMV retinitis alone. Early rec…

Optic neuritis: guidelines.

A major breakthrough in the therapy of demyelinating disease has recently been disclosed by the Optic Neuritis Treatment Trial. Over the past 6 years the trial has accumulated evidence that a high dose of corticosteroids taken to treat patients with an isolated acute attack of optic neuritis reduces the rate at which multiple sclerosis develops over a 2-year period of follow-up. The findings of the trial have had an immediate impact on the neuro-ophthalmic and neurologic community in the USA; therefore, I have focused my review on this important topic.

Chiasmal optic neuritis.

We describe two cases of chiasmal optic neuritis. The presenting symptoms were reduced visual acuity and bitemporal visual field defects. The enlargement of the optic chiasm in the acute stage and normalization after recovery is demonstrated by magnetic resonance imaging (MRI).

Varicella optic neuritis.

A 14-year-old girl developed severe bilateral optic neuritis 1 week after the onset of chickenpox. Though previous case reports state that complete recovery is the rule, this child had a persistent decrease in visual acuity in the right eye and loss of color vision and defects in visual fields in both eyes.

Chiasmal optic neuritis.

Two patients exhibiting bitemporal hemianopia due to chiasmal optic neuritis with pathological confirmation of noncompressive lesions are presented; the first due to a chronic, idiopathic demyelinating process, and the second a result of ethchlorvynol (Placidyl) toxicity. The literature regarding chiasmal optic neuritis is reviewed, and a discussion of other noncompressive causes of bitemporal hemianopia is presented.
