In a child with a hypothalamic glioma, elevated intracranial pressure and retrograde optic tract atrophy produced distinctive patterns of papilledema and disc pallor in each eye. On the side of the tract lesion, the disc was swollen nasally and was pale temporally. In the contralateral fundus, the swollen optic disc was bisected horizontally by a pale atrophic band. These funduscopic signs exemplify a rare instance in neuro-ophthalmologic diagnosis wherein specific lateralizing and localizing information can be deduced solely from the appearance of papilledema.

PURPOSE: To report a patient who was initially examined with optic tract syndrome caused by a focal metastasis to the lateral geniculate body and optic tract.
METHOD: Case report.
RESULTS: A 44-year-old woman with metastatic breast cancer was initially examined with normal central visual acuity, afferent pupillary defect, homonymous hemianopia, and optic atrophy.
CONCLUSION: This case demonstrates that an isolated optic tract syndrome may occur as a result of metastatic lesions to the lateral geniculate body and optic tract.

Optic neuritis occurred in three of our patients receiving treatment with alpha interferon-2b (Intron-A; 3MU thrice weekly) for chronic hepatitis. The complication appeared within, 1, 9 1/2 and 10 months of treatment, respectively. In all cases, blurred vision was the initial complaint and subsequent electrophysiologic investigation confirmed the presence of optic tract neuropathy. The patients had no other neurologic signs. Two cases are reported of optic tract lesions with complete homonymous hemianopia, characteristic optic atrophy and retinal nerve fiber bundle defect, and relative afferent pupillary defect (RAPD). RAPD was 0.6 log units in both cases. Infrared pupillography revealed pupillary light response of the ipsilateral side to be 1.2 or 1.3 times higher than that of the contralateral side. We confirmed that the detection of RAPD is helpful in differentiating optic tract lesions from suprageniculate lesions in cases of complete homonymous hemianopia.

Optic tract oscillatory responses directly recorded during posteroventral pallidotomy were investigated to reveal their features with respect to extracranially recorded visual oscillations and to clarify their contributions to scalp-recorded or far-field visual evoked potentials. Three patients with optic tract lesions, incomplete homonymous hemianopias, and good vision in both eyes are presented. All three had a Marcus Gunn pupil in the eye with temporal field loss (eye contralateral to the tract lesion). This pupillary sign may serve to differentiate acute optic tract from acute geniculostriate lesions when the characteristic infrageniculate optic atrophy has not had time to develop.

Intracranial sarcoidosis was diagnosed in a 23-year-old man when he developed a markedly incongruous right homonymous hemianopic visual field defect. The diagnosis was substantiated by radiologic evidence of bilateral hilar and paratracheal adenopathy, typical of pulmonary sarcoidosis. Subsequently, computed axial tomography indicated a large intracranial lesion in the suprasellar cistern with extension into the left temporal lobe along the optic tract.
