Modified Postauricular Incision for Preservation of the Lesser Occipital Nerve and the Great Auricular Nerve in Ear Surgery.

OBJECTIVE: To describe the feasibility of preserving the lesser occipital nerve (LON) and the great auricular nerve (GAN) in postauricular incision in ear surgery. METHODS: The distribution of the LON and the GAN was first identified in human cadavers. Then a clinical study was performed in 34 patients who underwent middle ear surgery between September 2016 and January 2017. Patients were divided into the conventional incision group and the modified incision group, according to incision types, and underwent sensory testing and subjective evaluation of auricular numbness after surgery at diffe

Drill-induced noise level during cochleostomy.

CONCLUSIONS: Noise induced by drilling during cochleostomy is of high level and can cause acoustic trauma to the inner ear, particularly after the membranous labyrinth has been exposed. Our method utilizes non-invasive noise measurement and the equipment that is connected to the operative area can be sterilized, thus it can be applied to intraoperative recordings. OBJECTIVES: To investigate the noise level generated by otological electrical drills during the cochleostomy procedure and to further explore the feasibility of noise monitoring in vivo. METHODS: Acoustic measurements during drilli

Iatrogenic cholesteatoma: a tertiary referral center experience.

PURPOSE: To report and compare different etiopathogenic mechanisms for iatrogenic cholesteatoma formation together with their respective symptomatic onset, diagnostic latency, anatomical extension and surgical management. METHODS: A retrospective single-center cohort observational study was conducted on patients who presented history of previous otologic surgery and subsequent diagnosis of iatrogenic cholesteatoma and underwent revision otologic surgery between January 2014 and December 2023. RESULTS: Seventeen cases of iatrogenic cholesteatoma were investigated. Previous otologic procedures

Ear deformities, otoplasty, and ear reconstruction.

LEARNING OBJECTIVES: After reviewing this article, the participant should be able to: 1. Evaluate patient's ears for needed adjustments to size, shape, prominence, and symmetry. 2. Identify common ear deformities and describe methods to repair them. 3. Avoid or manage common complications associated with otoplasty and ear reconstruction. SUMMARY: The essentials of otoplasty will be described/illustrated for the following conditions: Prominent ears, underdeveloped helical rims (shell ear), macrotia, Stahl's ear, constricted ear, cryptotia, and question mark ear.

Preservation of function in vestibular schwannoma surgery.

The management and surgical technique for microsurgical tumor removal of vestibular schwannomas (acoustic neuroma) with the suboccipital retrosigmoid approach and semi-sitting patient positioning is described. An emphasis is placed on the preservation of auditory and facial nerve function with a stepwise description of the technical and operative nuances, including presurgical evaluation, positioning, anesthesiological and neurophysiological aspects, approach, microsurgical techniques, and postsurgical care.

An otoneurosurgical approach to non-pulsatile and pulsatile tinnitus.

OBJECTIVE: Most treatments proposed for tinnitus are non-surgical, to such an extent that it is sometimes forgotten that a certain number of patients with tinnitus may benefit from a surgical solution. The aim of this paper is to review the possible otoneurosurgical approaches in tinnitus treatment, treating the tinnitus causally or symptomatically. METHODS: A Pubmed search on the words "surgery", "tinnitus" and "pulsatile" was performed and compared to the authors' personal experience with surgical approaches for alleviating tinnitus. The most relevant different pathologies presenting as pul

Key Interventions Related to Middle Ear Cholesteatoma: A Systematic Umbrella Review.

OBJECTIVE: This systematic umbrella review aimed to identify current evidence for the interventions to prevent, diagnose, and/or manage cholesteatoma. Results from this review will inform the development of the World Health Organization (WHO) Package of Ear and Hearing Care Interventions (PEHCI). The PECHI will describe priority interventions that can be integrated into health systems to reduce the global burden of otologic conditions. DATABASES REVIEWED: We searched Ovid MEDLINE, Scopus, and PubMed in January 2025 to identify systematic reviews describing interventions for cholesteatoma. ME

Beyond Tympanomastoidectomy: A Review of Less Common Postoperative Temporal Bone CT Findings.

Postoperative temporal bone imaging after surgical procedures such as ossiculoplasty, tympanomastoidectomy, cochlear implantation, and vestibular schwannoma resection is often encountered in clinical neuroradiology practice. Less common otologic procedures can present diagnostic dilemmas, particularly if access to prior operative reports is not possible. Lack of familiarity with the less common surgical procedures and expected postoperative changes may render radiologic interpretation challenging. This review illustrates key imaging findings after surgery for Ménière disease, superior semicirc

Otologic manifestations of Langerhans' cell histiocytosis.

Eighteen of 62 children diagnosed with Langerhans' cell histiocytosis at the Children's Hospital of Pittsburgh (Pa) between 1970 and 1986 demonstrated ear and temporal bone involvement. In six children, such otologic disease was their sole presenting manifestation. Common signs and symptoms included aural discharge, postauricular swelling, and conductive hearing loss. The otologic findings in these children, if not investigated properly, could easily be attributed to acute or chronic infectious ear disease. Computed tomography with contrast enhancement proved to be particularly valuable as a d

Educational Benefit of the Three-Dimensional Exoscope Versus Operating Microscope in Otologic Surgery.

OBJECTIVE: To compare observation of otologic surgery using a traditional operating microscope (OM) and a three-dimensional exoscope. STUDY DESIGN: Prospective, nonrandomized, noncontrolled study. SETTING: Tertiary care center. METHODS: Senior medical students and otolaryngology trainees observing otologic surgery performed with OM and exoscope were asked to complete a questionnaire comparing the two experiences. The key variables were image clarity, depth perception, observer's level of motion sickness during the surgery, the participant's understanding of middle ear anatomy, and their ove
