OBJECTIVE: Attending to observe the bony healing on hard palate after palatal repair, and to discuss the factors affecting on it.
METHODS: 52 patients with repaired cleft palate over 5 years postoperatively were examined, the CT scan of head was taken. The incidence of the bone regeneration among the patients examined was calculated, the position and quality of bone tissue were measured according to the CT images. After all, analysis was applied to evaluate the factors affecting on the bone tissue formation.
RESULTS: Formation of bone bridge was found in the 37 cleft gaps out of 52 patients …

Soft tissue chondroma of palate is very rare. It has never been reported in a cleft palate patient. We report a case of 22-year-old male who came with asymptomatic swelling on the palate since birth, along with complete cleft of secondary palate. He had symptoms related to cleft palate only, i.e., nasal regurgitation and speech abnormalities. Swelling was excised and the cleft palate was repaired. Histopathological examination revealed chondroma of the palate. The patient had no recurrence after 2 years of follow-up.

BACKGROUND: The purpose of this study was to observe bony healing in the hard palate after cleft palate repair and to discuss the factors affecting it.
METHODS: Fifty-two patients with repaired cleft palate were examined at least 1 year postoperatively by means of computed tomographic scanning. The incidence of bone regeneration automatically after repair was calculated, and the region of bone formation and the quality of bone tissue were measured according to the scanned image.
RESULTS: The formation of a bone bridge was found in 37 of 52 patients (71 percent). The length of regenerated bon…

Midpalate was analyzed for the presence of nonmuscle contractile systems. The results indicate that increased amounts of actin and myosin are present in cells of regions 2 and 3. A localization of the contractile proteins in cellular projections (filopodia) and in the peripheral cytoplasm of the cell body was confirmed by indirect immunofluorescence studies, using antibodies directed against smooth muscle myosin and against skeletal muscle actin. Specificity of the immunofluorescence reactions was ascertained by immunoabsorption studies using purified myosin and actin. Electron microscopic obs…

CONTEXT: The spatial relationship of posterior palatal seal (PPS) width and vibrating lines varies among individuals. Such variability could be related to contour of the palate.
AIMS: The study was carried out to evaluate the relationship between PPS width of the patient intra-orally and cephalometric tracing of the same patient. Second part of the study was formulated to determine whether the anterior and posterior vibrating lines can be distinguished as two separate lines by different observers.
MATERIALS AND METHODS: A lateral cephalogram was made to trace the hard and soft palatal contou…

Lateral synechiae syndrome is a rare congenital malformation characterized by cleft palate and lateral intraoral adhesions from the free borders of the palate to the floor of the mouth. Synechiae can be isolated or occur more frequently in association with other congenital anomalies such as cleft lip and/or palate. These synechiae can cause functional deficits, especially in the respiratory and feeding difficulties, as well as facial abnormalities at later stages. Surgical excision of the adhesions as soon as possible is the well-adapted protocol at most centers. We report a case of a 15-day-o…

OBJECTIVE: This is a prospective study examining palatal casts from patients with unilateral cleft lip and palate (UCLP) in the first month of life, immediately before cheiloplasty, and immediately before palatoplasty. None of the patients receives presurgical orthopedics (nasoalveolar molding).
DESIGN: In this prospective study, upper arch plaster models were taken 3 times during the treatment: in the first month of life (T1), before the cheiloplasty (T2), and before the palatoplasty (T3). Anatomic landmarks were defined and linear anthropometric measurements were obtained afterward. Dimensi…

Little is known of the arterial anatomy of the palate. Injection studies and dissection of a total of 49 cadavers have shown the principal arterial supply of the soft palate to be the ascending pharyngeal artery, which anastomoses with the greater palatine artery at the junction of the hard and soft palates. The position and relations of the branches of the ascending palatine artery put it at risk during palate repair.

The palate neoplasias are rare and come from either the minor salivary glands or the stratified epithelium. The malignant tumors of the hard palate are present 3-4 times more frequently than the malpighian carcinomas which, on the contrary, are prevailing at the soft palate. The malignant tumors of the palate may be located or extended to the neighbouring anatomical structures. The application of extended surgical operations for their treatment often creates great anatomical deficiencies which need the application of a reparatory intervention for restoration. We present, in this study, our exp…

Oronasal fistulae and velopharyngeal insufficiency are common and interdependent complications after cleft palate surgery. Bone grafting can complement cleft habilitation. Early identification and intervention are vital for optimal outcomes. Collaboration with experienced healthcare professionals is crucial to develop a comprehensive treatment plan which considers speech therapy, prosthetic devices, and surgery. This article aims to review the current literature on the management of VPI and oronasal fistulae following cleft palate surgery and additionally highlight the role of alveolar bone gr…

Soft palate reconstruction is complex. It has to restore swallowing and speech replacing the defect by dynamic and sensitive tissues. The means are multiple from direct suture to free flaps. Local flaps have the advantage to bring a mucosa animated by thin muscles, like in sphincteroplasty. Free flaps allow reconstruction of large defects, but they need some local adaptation (association with a local flap, adherence, suspension with tendon…) to improve the functional result. Indications depend on defect's size and local condition especially radiotherapy history.

Submucous cleft palate (SMCP) is a subgroup of cleft palate which is often unawared by general practitioners. The diagnostic triad of (1) bifid uvula, (2) translucent zone in soft palate and (3) bony notch in the posterior edge of the hard palate can be variable in their severity among each patient. Some patients with SMCP will have speech problems with velopharyngeal insufficiency (VPI) and/or articulation errors. Patients will also have middle ear effusion as in an overt cleft palate. Surgical treatment is indicated in the presence of VPI or recurrent otitis media. There are controversies on…

An examination of 9720 Zagreb school children, 6-13 years of age, revealed submucous cleft palate (SMCP) in 5 and cleft uvula in 232. The prevalence of SMCP among Zagreb school children was 0.05% or 1:1944. Two out of 102 relatives of CP patients examined had submucous cleft palate. The findings of five SMCP in children and two in parents of CP patients, undiagnosed before the screening, confirms the difficulties of recognition that complicate the obtaining of correct data about their incidence.

Over 10,000 school children were screened for palatal abnormalities. Nine submucous clefts of the palate were detected, an incidence of 1 : 1,200. Speech defects, ear problems and hearing loss were found as complications in 55% of 50 submucous cleft palate patients studied in detail. Complications were less common (45%) if the palatal abnormality was an isolated defect. Proper management of submucous cleft palate depends upon knowledge of its incidence and natural history.
