Paraneoplastic neuropathies.

PURPOSE OF REVIEW: This review describes relevant advances in paraneoplastic neuropathies with emphasis on particular syndromes and the impact of new therapies. RECENT FINDINGS: Sensory neuronopathy may present with symptoms that do not raise the suspicion of a paraneoplastic origin. A recent study on sensory neuronopathies of different causes identified paraneoplastic cases in a group of older (>60 years) male patients with subacute onset early pain, and frequent involvement of the arms. Paraneoplastic sensorimotor polyneuropathies may be confused with chronic inflammatory demyelinating poly

[Neurologic paraneoplastic syndrome with anti-CV2/CRMP5 antibodies revealing a small cell lung cancer. Effectiveness of the lung cancer treatment].

INTRODUCTION: Paraneoplastic neurological syndrome associated with anti-CV2/CRMP5 antibodies are rare. Various clinical manifestations can occur, cerebellar ataxia, polyneuropathy, optic neuritis with NORB or uveitis. Small cell lung carcinoma is generally responsible. CASE REPORT: We report the case of a 64-year-old man who developed visual symptoms with papilledema, cerebellar signs, polyneuropathy confirmed with a neurophysiological studies. Anti-CV2/CRMP5 antibodies were present. A small cell lung carcinoma was responsible for this paraneoplastic syndrome revealing the cancer. The paraneo

Paraneoplastic neuropathy.

Recent progress in serological screening of paraneoplastic antibodies and in diagnostic imaging techniques to detect malignancies has enabled a broadening of the concept of paraneoplastic neurological syndromes by integrating nonclassic clinical features. The peripheral nervous system is frequently involved in patients with paraneoplastic syndrome and may be seen alone or in combination with involvement of other areas of the nervous system. Destruction of dorsal root ganglion cells due to lymphocytic infiltration, especially with CD8-positive cytotoxic T cells, has been postulated to mediate t

[Neuropathies associated with cancers and dysglobulinemias].

Peripheral neuropathies associated to cancers encompass several causes: entrapment or direct involvement of peripheral nerves, paraneoplastic and non tumoral causes, mainly infectious, metabolic or carential. Paraneoplastic neuropathy is mainly the so-called sensory ganglionopathy. A purely motor neuronopathy may be observed in Hodgkin's disease and "terminal" polyneuropathy may complicate may known cancer, in a context of severe weight loss. Polyneuropathies associated with paraproteinaemia include those associated with a defined lymphoproliferative disorder (myelomas, solitary plasmocytoma,

Paraneoplastic neuropathy: wide-ranging clinicopathological manifestations.

PURPOSE OF REVIEW: Recent progress in serological screening for paraneoplastic autoantibodies and diagnostic imaging techniques to detect malignancies has resulted in a broadening of the concept of paraneoplastic neurologic syndromes through the characterization of nonclassical clinical features. The goal of this article was to review the recent literature describing the wide-ranging clinicopathological manifestations of paraneoplastic neuropathy. RECENT FINDINGS: The classical feature of paraneoplastic neuropathy is subacute sensory neuronopathy; in addition, sensorimotor neuropathies, such

RATIONALE: Paraneoplastic syndromes occur in 10% to 20% of all malignancies, with paraneoplastic neurological syndromes in less than 1% of all malignancies. In small cell lung cancer (SCLC), paraneoplastic manifestations are more common, affecting up to 5% of cases. Common manifestations include syndrome of inappropriate antidiuretic hormone secretion, Cushing's syndrome due to adrenocorticotropic hormone secretion, paraneoplastic cerebellar degeneration, and Lambert-Eaton myasthenic syndrome. Paraneoplastic acute motor and sensory polyneuropathy is an exceptionally rare but severe complicatio

Paraneoplastic mononeuritis multiplex as a presenting feature of adenocarcinoma of the lung.

Paraneoplastic neurologic syndromes are observed in less than 0.1% of cancer patients. Neurologic syndromes in lung cancer include Lambert-Eaton myasthenic syndrome, polyneuropathy, cerebellar degeneration, and rarely mononeuritis multiplex. In this case, a patient presenting with bilateral asymmetrical sensorimotor polyneuropathy who was diagnosed with adenocarcinoma of the lung is reported.

Intravenous immunoglobulin therapy in paraneoplastic neurological syndromes.

Combining clinical and immunological information, a neurological syndrome can now be diagnosed as a "definite" or "possible" paraneoplastic syndrome according to the newly suggested diagnostic criteria of the PNS Euronetwork. Differentiated diagnosis of a paraneoplastic syndrome is essential for differential therapy in patients. According to the response to IVIG therapy, paraneoplastic disorders may be subgrouped in group A, a clinical response is the rule (prototype Lambert-Eaton myasthenic syndrome), and in group B, IVIG may be helpful in single patients and is indicated in specific clinical

Peripheral neuropathy in osteosclerotic myeloma: clinical and electrodiagnostic improvement with chemotherapy.

A patient with a severe remote-effect polyneuropathy and other paraneoplastic features of osteosclerotic myeloma improved dramatically with melphalan and prednisone treatment. Serial electrodiagnostic studies provided an objective means of following the response to therapy and documented the improvement. We believe this represents the first reported patient with multifocal osteosclerotic myeloma and a myelomatous polyneuropathy responding to melphalan and prednisone.

Chronic Inflammatory Demyelinating Polyradiculoneuropathy as a Presentation of Plasmacytoma.

Polyneuropathy, organomegaly, endocrinopathy, M protein elevation, and skin changes (POEMS) syndrome is one of the rare paraneoplastic disorders of the multiorgan association whose initialism describes the principal clinical features as polyneuropathy, organomegaly, endocrinopathy, M-protein elevation, and skin changes caused by an underlying plasma cell disorder, one of the major diagnostic criteria for which is the presence of an osseous lesion. We present a case of POEMS syndrome with nearly all signs and symptoms on clinical and hematological examinations, which was further investigated wi

[The differential diagnostic significance of neurological syndromes in a malignant tumors].

It is reported on a 35-year-old patient with carcinoma of the stomach. The disease became manifest above all neurologically in form of a clinically and autoptically clear preterminal carcinomatous meningitis and a clinically probable paraneoplastic sensomotoric polyneuropathy as early symptoms. The case is discussed and classified on the basis of literary data. Here the differential-diagnostic significance of the paraneoplastic neuropathy as early symptom is emphasized.

Thrombosis revealing POEMS syndrome. About a case.

POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, skin changes) is a rare paraneoplastic disorder due to an underlying plasma cell dyscrasia. The diagnosis of POEMS syndrome requires a chronic polyneuropathy and a monoclonal lambda plasma cell-proliferative disorder (mandatory criteria), and various systematic symptoms such as sclerotic bone lesions, Castleman's disease, organomegaly, endocrinopathy, skin changes, papilloedema and biological abnormalities such as elevated vascular endothelial growth factor (VEGF), thrombocytosis or polycythaemia. We describe an

Paraneoplastic neuropathies.

PURPOSE OF REVIEW: This article provides an approach to the recognition and management of paraneoplastic neuropathies. RECENT FINDINGS: Paraneoplastic neuropathies may have unique phenotypic presentations, such as sensory neuronopathy, autonomic enteric neuropathy, demyelinating neuropathy, and, rarely, motor neuropathy. Paraneoplastic sensorimotor neuropathy, on the other hand, may be indistinguishable from other common types of axonal polyneuropathy. Certain patterns of neuropathies are commonly seen with different types of cancers, but this relationship is not exclusive and not all patient

Paraneoplastic neuropathies.

PURPOSE OF REVIEW: To review recent advances in paraneoplastic neuropathies with emphasis on their definition, different forms and therapeutic development. RECENT FINDINGS: A strict definition of definite paraneoplastic neuropathies is necessary to avoid confusion. With carcinoma, seronegative sensory neuronopathies and neuronopathies and anti-Hu and anti-CV2/Contactin Response Mediator Protein 5 antibodies are the most frequent. With lymphomas, most neuropathies occur with monoclonal gammopathy including AL amyloidosis, Polyneuropathy-Organomegaly-Endocrinopathy-M component-Skin changes (POE

[Paraneoplastic peripheral neuropathies].

The PNS Euronet group criteria have classified paraneoplastic peripheral neuropathies as definite or possible according as to whether the neuropathy is a classical paraneoplastic disorder, the presence of onconeural antibodies, the delay between tumor and neuropathy, and improvement of the neuropathy with tumor treatment. Denny Brown's subacute sensory neuronopathy and neuropathies with anti-Hu or anti-CV2 antibodies are definite paraneoplastic disorders. Possible paraneoplastic neuropathies are heterogeneous. This classification is useful for the diagnostic of these disorders and the selectio

Imaging Review of Paraneoplastic Neurologic Syndromes.

Paraneoplastic syndromes are systemic reactions to neoplasms mediated by immunologic or hormonal mechanisms. The most well-recognized paraneoplastic neurologic syndrome, both clinically and on imaging, is limbic encephalitis. However, numerous additional clinically described syndromes affect the brain, spinal cord, and peripheral nerves. Many of these syndromes can have imaging findings that, though less well described, are important in making the correct diagnosis. Moreover, imaging in these syndromes frequently mimics more common pathology, which can be a diagnostic challenge for radiologist

Paraneoplastic vasculitis of the peripheral nervous system.

Paraneoplastic vasculitic neuropathy (PVN) is a rare paraneoplastic syndrome characterized by non-systemic subacute vasculitic neuropathy. The two cancers most commonly associated with PVN are small cell lung cancer (SCLC) and lymphomas. Neuropathy varies from mononeurotherapy multiplex to symmetrical polyneuropathy. Two helpful laboratory abnormalities are a high erythrocyte sedimentation rate (ESR) and high cerebro spinal fluid (CSF) protein content. Axonal neuropathy is the characteristic finding in electrophysiological studies. Nerve biopsy is crucial to document microvasculitis. Recogniti
