Among consumers, families, therapists, physicians, and other rehabilitation professionals, there has been an increasing interest in shoulder pain associated with spinal cord injury. These disorders primarily affect the soft tissues, including the tendons (eg, rotator cuff tendonitis and bicipital tendinitis), muscles (eg, myalgias and myofascial pain syndromes), and bursae. Disorders of bone and joints also have been of interest (eg, osteoporosis and osteoarthritis of the acromioclavicular and glenohumeral joints). This article addresses the historical context, epidemiology, pathophysiology,di

A case is reported of spinal cord compression resulting from extramedullary haemopoiesis in a patient with thalassaemia. A 28-year-old woman with beta thalassaemia intermedia presented with a two week history of paraparesis with bladder and bowel incontinence. Recovery followed laminectomy and the removal of haemopoietic tissue in the spinal epidural space. This is a rare complication and only eight cases have been reported in the literature.

In the period 1965-80, 125 women were treated because of paralysis or paresis of the lower extremities resulting from spinal trauma. In this report the causes of trauma, as well as the clinical and functional treatment of this group are discussed. The most frequent causes of trauma were falls from heights, from a tree, a ladder, a horse-cart as well as suicidal jumps and road accidents. It was ascertained that the results of treatment of spinal cord injuries in the upper thoracic segment are worse than in its lower part. These results do not differ from men with similar degrees and neurologica

Hereditary spastic paraplegias (HSPs) are a group of neurodegenerative disorders which involve the corticospinal tracts and present with distinct spasticity and weakness of the lower extremities. The estimated prevalence of HSP is around 1.8/100,000 cases for both autosomal dominant and autosomal recessive types. Classification of HSP is based on inheritance pattern, clinical phenotype, and molecular pathophysiological mechanisms. The most common neuropathological sign is the axonal degeneration involving the lateral corticospinal tracts in both the cervical and thoracic spinal cord. The targe…

The hereditary spastic paraplegias (HSPs) are a heterogeneous group of neurologic disorders with the common feature of prominent lower-extremity spasticity, resulting from a length-dependent axonopathy of corticospinal upper motor neurons. The HSPs exist not only in "pure" forms but also in "complex" forms that are associated with additional neurologic and extraneurologic features. The HSPs are among the most genetically diverse neurologic disorders, with well over 70 distinct genetic loci, for which about 60 mutated genes have already been identified. Numerous studies elucidating the molecula…

We report a rare case of postoperative paraplegia in a patient with carcinoma of the oesophagus following oesophagectomy. Neurological deficit was characterised by loss of sensation from the spinal level T2 down to T6, together with flaccid paraparesis of both lower extremities. This was initially thought to be secondary to an epidural haematoma and a magnetic resonance scan was arranged. This suggested changes consistent with spinal cord infarction from D2 to D6 with no haematoma or abscess. This severe complication was most probably caused by embolisation from an atherosclerotic plaque of th…

Of 330 Black patients admitted to hospital with paraplegia, the causes in 33 remained unexplained after investigation. These patients had features of predominant corticospinal tract degeneration with lesser degrees of sensory loss. This series of patients is similar to some reported previously from South Africa and from other tropical countries. Among several possible causes the most likely is a toxic dietary factor.

A 45 years old married lady was admitted with spastic paraplegia of insidious onset without any sensory deficit. Serological studies of blood and CSF for syphilis were positive and cerebrospinal fluid showed increased cells and protein content. She responded well to parenteral penicillin therapy, showed gradual subjective improvement and appreciable decrease in cellular and protein content of the CSF. Her husband also was found to have latent syphilitic infection. Review of relevant literature showed no report of such paraplegia following syphilis in recent times. The case is being reported wi…

A 20-year-old man suffered head, chest, and abdominal trauma in an auto accident resulting in a traumatic dissecting aneurysm of the thoracic aorta. Hypotension developed. The aneurysm was resected and replaced with a prosthetic graft. Postoperatively, the patient was found to be paraplegic below T-9, areflexic and anesthetic to pain and temperature, with preservation of vibration and position senses. In the ensuing nine months, the patient regained considerable sensory function in his lower extremities and had severe constant hyperhydrosis below the T-9 dermatome. The exaggerated sweating was
