Defibrillators: Selecting the Right Device for the Right Patient.

Advances in the field of defibrillation have brought to practice different types of devices that include the transvenous implantable cardioverter-defibrillator (ICD) with or without cardiac resynchronization therapy, the subcutaneous ICD (S-ICD), and the wearable cardioverter-defibrillator. To ensure optimal use of these devices and to achieve best patient outcomes, clinicians need to understand how these devices work, learn the characteristics of patients who qualify them for one type of device versus another, and recognize the remaining gaps in knowledge surrounding these devices. The transv

How to Treat Distal Radius Fractures: Right Patient, Right Care, Right Time, and Right Cost.

Operative intervention for distal radius fractures is typically reserved for patients with displaced fractures that may result in bothersome compromises in function, although patient-specific factors (age, activity level, and preference) are considered. Operative intervention is associated with earlier improvement in function but exposes the patient to the risk of anesthesia and surgery. Although surgery is associated with an initial increase in cost of care, the benefits of earlier return to activity may offset these increases. Efforts to contain cost through implant selection, use of ambulat

Airway Clearance Techniques: The Right Choice for the Right Patient.

The management of bronchial secretions is one of the main problems encountered in a wide spectrum of medical conditions ranging from respiratory disorders, neuromuscular disorders and patients undergoing either thoracic or abdominal surgery. The purpose of this review is illustrate to the reader the different ACTs currently available and the related evidence present in literature. Alongside methods with a strong background behind as postural drainage, manual techniques or PEP systems, the current orientation is increasingly aimed at devices that can mobilize and / or remove secretions. Choosing the Right Biologic for the Right Patient With Severe Asthma.

In this installment of the How I Do It series on severe asthma, we tackle the clinical conundrum of choosing the right biologic for the right patient with severe asthma. With six biologics now approved for use in this area comprising four different targeting strategies (anti-Ig E: omalizumab; anti-IL-5 and anti-IL-5-receptor: mepolizumab, reslizumab, and benralizumab; anti-IL-4-receptor: dupilumab; anti-thymic stromal lymphopoietin: tezepelumab), this question is increasingly complex. Recognizing that no head-to-head trial has compared biologics, we based our review on the expected effects of

Selection criteria for antifungals: the right patients and the right reasons.

The increasing number of new antifungal agents and their range of licensed indications mean that clinicians now have several pharmacological options for combating fungal infections. This paper briefly reviews the current available agents and seeks to introduce evidence that will help ensure optimal treatment for each type of fungal infection.

Treating hypertension. Are the right drugs given to the right patients?

OBJECTIVE: To evaluate whether physicians are prescribing antihypertensive drugs appropriately and according to the recommendations of the Canadian Hypertension Society. DESIGN: Retrospective cohort study. SETTING: Family medicine teaching clinic in Montreal. PARTICIPANTS: A cohort of 183 patients followed between 1993 and 1995. Of 350 patients registered at the clinic, 167 were excluded because diagnosis of hypertension was not supported by chart review, their charts contained insufficient information, they were pregnant or younger than 18 years, or they had secondary hypertension and comp

The right patient, the right moment, the right therapy: reimagining precision oncology from the bedside.

Precision oncology has delivered an unprecedented expansion of molecularly targeted therapies, yet patient selection remains the defining unmet need of the field. Too many patients receive treatments that will not benefit them, while those most likely to respond go unidentified. Drawing on two decades of translational work in treatment-refractory triple-negative breast cancer (TNBC) and inflammatory breast cancer (IBC), this Perspective argues that the central question of oncology's next decade is not what new drug to develop, but which patient should receive which therapy, at which moment, an

Toward Personalized NSAID Therapy in Osteoarthritis: The Right Patient, the Right Treatment, at the Right Time.

BACKGROUND: Non-steroidal anti-inflammatory drugs (NSAIDs) remain the cornerstone of pharmacological treatment for osteoarthritis (OA) and are recommended by international guidelines for the management of symptomatic pain. Despite their well-established efficacy, substantial interindividual variability exists in treatment response, with some patients experiencing meaningful pain relief while others derive little clinical benefit despite appropriate drug selection and dosing. This variability reflects, at least in part, the biological heterogeneity of OA pain. OBJECTIVE: To review the mechanis

Navigating the Journey in Psoriatic Arthritis: Matching the Right Patient, the Right Drug, and the Right Time.

Psoriatic arthritis (PsA) is a heterogeneous, immune-mediated disease that significantly impacts quality of life, functional capacity, and healthcare systems. Over the past two decades, treatment options have expanded from conventional therapies to biologic and targeted synthetic DMARDs, enabling more effective disease control. However, many patients still fail to achieve remission or low disease activity (LDA), reflecting challenges in selecting the right treatment at the right time for the right patient. This perspective introduces a conceptual framework for PsA management using the metaphor

Current methods of circumcision: choosing the right technique for the right patient.

PURPOSE OF REVIEW: Circumcision remains one of the most commonly performed urological procedures worldwide. This narrative review discusses contemporary methods of circumcision, with emphasis on patient selection, conventional surgery, device-assisted circumcision, ShangRing, stapler devices, laser and energy-assisted techniques, wound closure, and special situations such as severe pathological phimosis and lichen sclerosus. RECENT FINDINGS: Recent literature has focused on adult phimosis management, modified conventional techniques, disposable circumcision devices, stapler devices, ShangRing

Successful Right Hemihepatectomy for a Patient with Right-Sided Round Ligament.

Right-sided round ligament (RSRL) is a rare congenital anomaly that is strongly associated with a number of intrahepatic vascular anomalies. Here, we report a 77-year-old male case of hepatolithiasis associated with RSRL for which resection was performed using right hemi-hepatectomy (RHH). Intraoperative ultrasonography revealed that the portal branch of segment 4 ramified from the right anterior portal branch, and the patient was diagnosed as having RSRL. To decide the resection line, the portal branch of segment 4 was punctured and stained with indigo carmine under ultrasonographic guidance,

A patient with headache, right upper extremity and right hemithorax hypothermalgesia.

We present a case of partial Wallenberg syndrome also called partial lateral medullary syndrome, a hemorrhagic or ischemic stroke of the area fed by the posterior inferior cerebellar artery and the clinical manifestation depends on the extension of the lesion: dorsal-ventral, medial-lateral and rostrocaudal. Five types have been described. Our patient had headache, hoarseness, right upper extremity, right hemithorax and right upper gluteal hypothermalgesia implicating the involvement of the cervical, the thoracic and part of lumbar fibers of the left lateral spinothalamic tract and the ambiguo
