Magnetic resonance (MR) is well suited to imaging the pericardium. High resolution images synchronized with the cardiac cycle can be obtained in any plane. Congenital foramen of the left parietal pericardium is uncommon. The condition has the potential to cause angina pectoris, myocardial infarction, or even death. Forty-three confirmed cases have been retrieved from the English language literature, and a case report of "life-threatening" herniation of the heart is here added. The diagnosis, made at a mean age of 20 years (range 2 to 48), was five times more common in men. In 5 fatal cases, the heart had become incarcerated. This is the report of a 46-year-old patient with the preoperative diagnosis of an atrial septal defect (ASD) of the ostium secudum type. After sternectomy, partial agenesis of the left pericardium was diagnosed. It is our opinion that, if the radiographic picture is suggestive of this entity, a clinical search for cardiopulmonary anomalies should be performed, because the majority of these associated anomalies can and should be surgically corrected.

The importance of recognizing congenitally absent left pericardium in the setting of acquired cardiovascular disease was recently appreciated during a case of coronary artery bypass grafting. We describe the use of the Starfish 2 heart positioning device as an aid to pericardium reconstruction after en bloc resection of mediastinal tumors of the left pericardium by use of median sternotomy with anterolateral thoracotomy. The Starfish device, which is a tool for off-pump coronary artery procedures, allows excellent cardiac positioning and hemodynamic stability during pericardium reconstruction through a median sternotomy with anterolateral thoracotomy.

A 52-year-old male presented with dyspnoea on exertion. He was found to have a clinically normal cardiac status and a mass lesion in the anterior mediastinum, probably arising from the pericardium near the right atrium, as shown by both echocardiography and a computerized tomographic scan of the chest. He was successfully operated. The histopathology of the mass revealed it to be a hemangioma of the pericardium. This is one of the rare tumors of the pericardium and only a few cases have been reported in the literature.

Congenital defects of the pericardium are very rare and are often not recognized before surgery. We report a case study of a 16-year-old girl with a four-year history of dyspnoea on exertion, palpitations, left sided thoracic pain and near-syncope. Explorative thoracotomy revealed total absence of the left sided pericardium. This allows the heart to become displaced, thus giving the patient her uncharacteristic symptoms. The large defect was left open, to avoid compromising the venous circulation.
