Hydrops following perilymph fistula repair.

A series of 15 ears were explored for perilymph fistulas. Fifteen fistulas were found in 13 ears. Two cases are presented which developed hydrops-like symptoms only after closure of their fistulas. Closure of the other patients' fistulas was successful in relieving vertiginous symptoms, although hearing seldom improved. There were no significant complications of surgery. Fat failed as a graft material 4 of 10 times, whereas vein and perichondrium were not associated with recurrence of symptoms. The ENG-fistula test, as described by Daspit and associates, correctly predicted the presence or abs…

Perilymph fistula: concept, diagnosis and management.

Perilymph fistula is caused by changes of cerebrospinal fluid pressure and/or middle ear pressure. For diagnosis, history taking is extremely important in regard to whether the occurrence of symptoms is related to physical exertion, such as straining, nose blowing, sneezing etc. A variety of symptoms are due to pathologic changes of the membranous labyrinth. Exploratory tympanotomy is needed to verify the occurrence of leakage. However, perilymph fistula cannot be excluded, even if leakage is not observed. Management consists of absolute rest and closure of the fistula. If dizziness or vertigo…

The Tullio phenomenon and perilymph fistula.

Establishing the diagnosis of perilymph fistula remains a dilemma. At this time, identification of an active perilymph fistula can be confirmed only by surgery. On the basis of clinical history and audiovestibular testing, 54 patients underwent middle ear exploration for possible perilymph fistula at the Colorado Ear Clinic between July 1980 and June 1986. This group represents approximately 1% of all surgical procedures performed during that period. Seven patients (12%) were found to have the Tullio phenomenon preoperatively. Six of these were found to have active, free-flowing fistulas at th…

Effect of pentobarbital anesthesia on amikacin concentrations in plasma and perilymph and evaluation of multiple sampling in perilymph of guinea pigs.

The purpose of this study was to determine whether a multiple-sampling procedure could be used in guinea pigs to study the kinetics of amikacin in perilymph. Amikacin was infused intravenously for 6 h into conscious anesthetized guinea pigs, and the concentrations of the drug in plasma and perilymph were measured. From each anesthetized guinea pig, five to six perilymph samples were collected from one ear, and one sample was collected from the other ear at 6 h. The concentrations of amikacin in perilymph were dose proportional and increased slowly during the 6-h infusion. However, after 6 h of…

Concentration of salicylate in serum and perilymph of the chinchilla.

The concentration of salicylate in serum and perilymph was measured in chinchillas after intraperitoneal administration of sodium salicylate. Serum salicylate concentration peaked 2 to 4 hours after a single injection. Clearance was approximately complete after 16 hours. The within-subject variability of serum concentration 4 hours after injection was very low. Subject weight significantly affected serum salicylate concentration, with heavier animals showing higher serum levels. The relationship of perilymph to serum salicylate concentration was approximately linear, with a high correlation be…

An unusual case of presumed perilymph fistula.

Perilymph fistulae have been described for over 30 years and yet there are no universally accepted diagnostic criteria or treatment regimes for them. This paper describes an unusual case of a presumed traumatic perilymph fistula and discusses its treatment and prognosis in relation to previous studies.

A patient-oriented approach to perilymph fistula.

The subject of perilymph fistula is controversial in part because the preoperative diagnosis of perilymph fistula is difficult. Patients suffering auditory and vestibular symptoms secondary to perilymph fistula, therefore, present a dilemma to the practicing physician. In some instances, patients with auditory and vestibular symptoms in the absence of perilymph fistula will be subjected to middle ear exploration without benefit, while in other instances patients with auditory and vestibular symptoms secondary to perilymph fistula will be denied surgical treatment. Auditory and vestibular sympt…

Organic acid transport into the cochlear perilymph.

The passage of exogenous organic and inorganic substances from blood into perilymph is likely to be controlled by the blood-perilymph barrier. This report reviews published data on the transfer of ten exogenous organic acids from the blood into the perilymph of experimental animal models. Although the range of the molecular weights of these acids is within half an order of magnitude, major differences exist in the perilymph concentration as a percentage of simultaneous serum concentration. Furthermore, these studies show that, contrary to previous suggestions, non-ototoxic compounds can achiev…

[Perilymph fistulas--symptoms and indications for tympanoscopy].

In the last two years thirteen cases of perilymph fistula were treated by tympanoscopy and covering with connective tissue. Recovery of hearing was not observed in 4 totally deaf ears, but improvement was recorded in each of 9 cases with impaired hearing. Vertigo disappeared in 10 of 11 cases without any correlation with the degree of hearing loss. We perform tympanoscopy in suspected perilymph fistula, because prognosis is uncertain, spontaneous recovery with remaining defects may induce ménièriform syndrome, and pathway of infection should be closed. The diagnosis of a perilymph fistula must…

Measurement of oxygen tension in human perilymph.

The oxygen tension of the human perilymph has been determined without complications by using a technique based upon the polarographic principle. Carbon dioxide produced the most marked increase in oxygen tension in the human perilymph. No change in the perilymphatic PO2 was observed after administration of eupaverine and low molecular weight dextran. 5% CO2-95%O2 has been found to be the gas mixture producing the largest increase in oxygen tension in the human perilymph, without side effects. In order to achieve maximal oxygenation of the perilymph, the therapeutic administration of 5% CO2-95%…

Peptides of the otosclerotic perilymph examined by analytical isotachophoresis.

Perilymphs of normal and otosclerotic origin were separated chromatographically on a Sephadex G-25 microcolumn. Peptide composition of the perilymphs was compared by capillary analytical isotachophoresis in the molecular mass range 0.3-5 kD. Otosclerotic perilymph samples contain a heterogeneous, UV-absorbing peptide subfraction which is not detected in the normal perilymph. Normal and otosclerotic perilymph, furthermore, contain four common subfractions detected in twice the normal concentration in the otosclerotic perilymph. These ITP subfractions are degraded during acid hydrolysis (6 M HCI…

Histopathological study of the cochlea with altered perilymph metabolism.

In 41 guinea pigs an experiment was conducted to determine the effect of kanamycin on ears with or without blockage of the cochlear aqueduct … han the inner hair cells. Proteinaceous precipitates were often found to be slightly greater in the perilymph of cochleae with blocked cochlear aqueducts. From these observations, it is concluded that the decrease in patency of the cochlear aqueduct will impair perilymph metabolism, causing the cochlear sensory cells at the basal turn to become more vulnera …
