Ethnic differences in perinatal mortality. A perinatal audit on the role of substandard care.

OBJECTIVES: The objective was to investigate the contribution of substandard care to ethnic inequalities in perinatal mortality. STUDY DESIGN: Perinatal audit in Amsterdam, the Netherlands. The study population consisted of 137 consecutive perinatal death cases (16 weeks GA-28 days after delivery). A standardized procedure to establish the cause of death and substandard care by perinatal audit was developed. The main outcome measures were perinatal mortality rates in ethnic groups, cause of death classified by extended Wigglesworth classification, presence of substandard care (unlikely to be,

Substandard factors in perinatal care in The Netherlands: a regional audit of perinatal deaths.

BACKGROUND: To determine: 1) whether substandard factors were present in cases of perinatal death, and to what extent another course of action might have resulted in a better outcome, and 2) whether there were differences in the frequency of substandard factors by level of care, particularly between midwives and gynecologists/obstetricians and between home and hospital births. METHODS: Population-based perinatal audit, with explicit evidence-based audit criteria. SETTING: The northern part of the province of South-Holland in The Netherlands. All levels of perinatal care (primary, secondary a…

Perinatal Care for Persons with Dissociative Disorders.

Caring for persons with mental health diagnoses can be daunting, especially when the conditions are rare, and there is little evidence to guide nursing practice. There is minimal information about caring for persons with dissociative disorders beyond the behavioral health literature, much less as in obstetric context. Women are more likely to experience dissociative disorders and post-traumatic stress disorders than men. Severe maternal morbidity is significantly more common in women with a history of stress and trauma-related conditions, highlighting the importance of providing guidance for c

Perinatal care improves the outcome of triplets.

OBJECTIVE: To compare pregnancy complications and neonatal outcome of 85 triplet gestations cared for during the 15 years in a single perinatal unit. METHODS: Pregnancies were divided in two groups according to the differences in the management plan and their outcomes were compared. Group I (N = 44) consisted of pregnancies cared from 1986 to 1995, using standard model of care: preventive hospitalization from the early second trimester or home bed rest with routine hospitalization after 28-32 weeks of pregnancy, routine clinical and ultrasound examinations, biophysical profile and non-stress

Perinatal care for the extremely preterm infant.

Being born preterm (prior to 37 weeks of completed gestation) is a leading cause of childhood death up to five years of age, and is responsible for the demise of around one million preterm infants each year. Rates of prematurity, which range from approximately 5 to 18% of births, are increasing in most countries. Babies born extremely preterm (less than 28 weeks' gestation) and in particular, in the periviable (200/7-256/7 weeks) period, are at the highest risk of death, or the development of long-term disabilities. The perinatal care of extremely preterm infants and their mothers raises a num…

[Perinatal care of the extremely preterm infants].

In 2018, The Danish Society of Obstetrics and Gynecology and the Danish Paediatric Society agreed on a national consensus guideline on the treatment of imminent preterm delivery prior to 25 weeks. This review summarises the recommendations: at 22 weeks transfer to a tertiary hospital, at 23 weeks tocolysis, antenatal steroid and newborn resuscitation with parental consent, if the infant is viable, and at 24 weeks caesarean section due to foetal distress. Newborn resuscitation is standard of care, unless not indicated for other reasons at 24 weeks.

Magnesium in perinatal care and infant health.

Magnesium, the second most abundant intracellular cation, is essential for life. The consequences of deficiency are severest in the smallest and youngest members of each species and may include sudden unexpected death. Magnesium deficiency, usually diagnosed by hypomagnesemia, may be congenital, as in premature infants, infants of magnesium-deficient mothers and infants with intrauterine growth retardation. It may be acquired or caused by low magnesium intake, the use of magnesium-wasting drugs, illness provoking gastrointestinal or renal losses of the mineral, or high metabolic demands impose

Outcome measures of obstetrical and perinatal care.

Outcome measures of obstetrical and perinatal care may concern the mother or the newborn and may involve mortality or morbidity. We reviewed the literature and found six outcome measures influenced by the process of care and its quality:maternal and perinatal mortalities, postpartum hemorrhage, sequelae of obstructed labor, Apgar scores and very early neonatal seizures. Birthweight and gestational age at delivery are additional variables to be taken into account as major confounding factors in inter-hospital or inter-area comparisons of mortality and morbidity. Other potentially useful outcome…

Measuring the effectiveness of perinatal medical care.

An outcome-based index of the effectiveness of perinatal care was empirically tested using 3,441,448 linked birth-death records. In order to construct the index, an "expected" perinatal mortality rate was computed for 504 California hospitals, based on each newborn's birth weight, sex, race, and plurality. The index was then defined as the ratio of the observed mortality rate to that expected. From the sixteen-fold range observed in the unadjusted rates, a two-fold variation could be attributed to differentials in the efficacy of medical care. The remainder resulted from differences in patient…

Randomised Controlled Trials for Informing Perinatal Care.

BACKGROUND: Randomised controlled trials provide the best evidence for the effects of interventions and are a key tool in the effort to improve the care and outcomes for newborn infants. METHODS: We discuss the role of randomisation for minimising selection bias in clinical trials and describe examples of seminal trials that have shaped the development of modern perinatal care. We consider the challenges inherent in designing and delivering large, simple, and pragmatic trials, and the need for the development and adoption of core outcome sets to ensure that trials provide high-quality evidenc…

Understanding Perinatal Depression Care Gaps by Examining Care Access and Barriers in Perinatal Individuals With and Without Psychiatric History.

Background: Depression affects one in seven perinatal individuals and remains underdiagnosed and undertreated. Individuals with a psychiatric history are at an even greater risk of perinatal depression, but it is unclear how their experiences with the depression care pathway may differ from individuals without a psychiatric history. Methods: We conducted a secondary analysis evaluating care access and barriers to care in perinatal individuals who screened positive for depression using the Edinburgh Postnatal Depression Scale (N = 280). Data were analyzed from the PRogram in Support of Moms (PR

'New parenting', psychotherapy, prenatal and perinatal care.

The health of future generations, both physical and psychological, depends upon good parental and early environment, free particularly from malnutrition, toxins and undue stress. Education about these negative influences is urgent, especially to encourage childbearing women in a healthy diet and lifestyle. The detrimental effects of cigarette smoking in pregnancy have been known since the 1970s, yet in Germany, for example, 60% of all children are conceived, carried, born into a household where at least one adult smokes. Even if the father desists from smoking at home, the nicotine in his body

Are some perinatal deaths in immigrant groups linked to suboptimal perinatal care services?

OBJECTIVE: To test the hypothesis that suboptimal factors in perinatal care services resulting in perinatal deaths were more common among immigrant mothers from the Horn of Africa, when compared with Swedish mothers. DESIGN: A perinatal audit, comparing cases of perinatal deaths among children of African immigrants residing in Sweden, with a stratified sample of cases among native Swedish women. POPULATION AND SETTING: Sixty-three cases of perinatal deaths among immigrant east African women delivered in Swedish hospitals in 1990-1996, and 126 cases of perinatal deaths among native Swedish wo…

Perinatal care and its association with perinatal death among women attending care in three district hospitals of western Uganda.

BACKGROUND: Provision of effective care to all women and newborns during the perinatal period is a viable strategy for achieving the Sustainable Development Goal 3 targets on reducing maternal and neonatal mortality. This study examined perinatal care (antenatal, intrapartum, postpartum) and its association with perinatal deaths at three district hospitals in Bunyoro region, Uganda. METHODS: A cross-sectional study was conducted in which a questionnaire was administered consecutively to 872 postpartum women before discharge who had attended antenatal care and given birth in the study hospital…

Quality of perinatal depression care in primary care setting in Nigeria.

BACKGROUND: Even though integrating mental health into maternal and child health (MCH) is widely accepted as a means of closing the treatment gap for maternal mental health conditions in low- and middle-income countries (LMIC), there are not many studies on the quality of the currently available mental health care for mothers in these countries. This study assessed the existing organization of service for maternal mental health, the actual care delivered for perinatal depression, as well as the quality of the care received by affected women presenting to primary care clinics in Ibadan, Nigeria

Cohort profile: The Mount Sinai Perinatal Ambulatory Care Cohort, a longitudinal electronic health record cohort of perinatal care utilization.

The Mount Sinai Perinatal Ambulatory Care Cohort was established to address limitations of existing postpartum surveillance datasets, particularly the underrepresentation of urban, minority, and publicly insured populations and the absence of systematic postpartum mental health screening data. It was designed to examine postpartum healthcare utilization, obstetric outcomes, and perinatal mental health in a large hospital-based ambulatory collaborative care practice in New York City. The cohort includes all individuals receiving prenatal and postpartum care through the Mount Sinai Hospital Obst
