An integrated simulator for surgery of the petrous bone.

This paper describes work being undertaken as part of the IERAPSI (Integrated Environment for the Rehearsal and Panning of Surgical Intervention) project. The project is focussing on surgery for the petrous bone, and brings together a consortium of European clinicians and technology providers working in this field. The paper presents the results of a comprehensive user task analysis that has been carried out in the first phase of the IERAPSI project, and details the current status of development of a pre operative planning environment and a physically-based surgical simulator.

[Anatomy of the petrous portion of the temporal bone].

The petrous portion of the temporal bone is a complex bony structure of the skull base that contains key components of the middle and inner ear as well as essential neurovascular elements. Due to its anatomical location and structural characteristics, it is frequently affected by traumatic, inflammatory, or neoplastic conditions. In addition to the clinical articles in this issue, this review provides a concise overview of the clinically relevant anatomy of the petrous portion, with an emphasis on radiological imaging.

Primary melanoma of the petrous temporal bone.

Melanoma is a malignant tumor of melanocytes that is predominantly found in the skin. In rare cases, it arises from mucosal melanocytes. We describe a case of a solitary melanoma of the petrous apex of the temporal bone in a 67-year-old woman who presented with sudden hearing loss, aural fullness, and headaches, all on the right side. Magnetic resonance imaging identified a mass located at the right petrous apex; the lesion was hyperintense on T1-weighted imaging and isointense on T2 weighting, and it enhanced brightly with gadolinium contrast. The patient underwent removal of the lesion via a

[Primary cholesteatoma of the petrous bone apex].

Primary cholesteatoma of the temporal bone is a very rare disease. Initially, the tympanic membrane is intact, and often facial palsy may be the presenting symptom. The authors present a case of primary cholesteatoma which was surgically treated using a transmastoid-translabyrinthine approach. Other surgical approaches to the petrous apex are discussed.

[Hemangioma of the petrous bone. Diagnosis and treatment].

Hemangiomas of the temporal bone are benign, vascular and are tumors. The diagnosis is in most cases made during the operation because the symptoms are non conclusive. Two cases were recently observed: one simulating a Meniere disease while the tumor was developed close to the jugular bulb and the endolymphatic sac; the second invaded the geniculate area and the petrous apex and was revealed by a progressive facial palsy.

Extensive Pneumocephalus Secondary to Petrous Bone Cholesteatoma.

Pneumocephalus refers to air inside the cranium; however, otogenic pneumocephalus is rarely reported in the literature. The neurological presentations of pneumocephalus include headache, lethargy, confusion, disorientation, and seizure. Here, we have reported a case of a 42-year-old woman with extensive pneumocephalus and cerebrospinal fluid leak secondary to petrous bone cholesteatoma. She presented to the emergency department with sudden headache and left ear discharge. Physical examination revealed watery otorrhea through a hole in the tympanic membrane. Radiological studies demonstrated ex…

Petrous bone fracture: a virtual trauma analysis.

OBJECTIVE: The temporal bone shields sensorineural, nervous, and vascular structures explaining the potential severity and complications of trauma related to road and sport accidents. So far, no clear data are available on the exact mechanisms involved for fracture processes. Modelization of structures helps to answer these concerns. Our objective was to design a finite element model of the petrous bone structure to modelize temporal bone fracture propagation in a scenario of lateral impact. MATERIALS AND METHODS: A finite element model of the petrous bone structure was designed based on comp…

Petrous Bone Cholesteatoma: Facial and Hearing Preservation.

Objectives: Petrous bone cholesteatoma (PBC) is a rare condition of the petrous portion of the temporal bone. Treatment of choice consists of radical surgical removal, paying attention to protect the facial nerve and inner ear as far as possible. The aim of the present study was to evaluate the efficacy of modified translabyrinthine techniques in preserving hearing function and the use of the adjuvant endoscopic techniques in a group of PBC patients. Methods: This study comprised 16 cases of PBCs surgically treated in our Department. Pre- and post-operative hearing status was assessed with pur…

Primary carcinoma involving the petrous temporal bone.

Between 1975 and 1985, 29 patients with the diagnosis of carcinoma of the petrous temporal bone were seen at the Princess Margaret Hospital. Twenty-seven carcinomas were graded: 13 were well-differentiated; the remaining 14 were either moderately or poorly differentiated tumors. Fifteen patients were managed with a combination of surgery and radiotherapy, 13 were treated with radiotherapy only, and one patient was treated by surgery alone. Median follow-up time was 6.1 years, and the 5-year actuarial local control and cause-specific survival rates for the entire group were 40% and 50%, respect

[Analysis of revision surgery for petrous bone cholesteatoma].

The clinical data of patients who underwent revision surgery for residual or recurrent petrous bone cholesteatoma at the Chinese PLA General Hospital between January 2009 and January 2023 were retrospectively analyzed. A total of 44 patients aged 16-66 (36±17) years were included, with 33 males and 11 females. The extent of the lesions before revision surgery was classified according to the Sanna classification. Based on preoperative imaging and intraoperative findings, the most common site of residual disease was the middle cranial fossa dura (26 cases, 59.1%). The primary reasons for residua…

The petrous temporal bone (including the cerebellopontine angle).

Lower resolution CT scanners play an important role in the evaluation of the cerebellopontine angle, but careful integration along with the older conventional examinations is often necessary. A high-resolution CT scanner may provide all the necessary information. With proper techniques, bony changes are most clearly analyzed, and small tumors of the acoustic nerve may be directly seen in the cerebellopontine angle, and even in the internal auditory canal, with only a contrast-enhanced scan. If necessary CT-air cisternography provides the most sensitive and least morbid of the definitive, invas

High-resolution computerized tomography of the petrous bone.

High-resolution computerized tomography (CT) has been used to investigate abnormalities of the ear and petrous bone. Congenital, traumatic, infective and neoplastic lesions have been evaluated in 30 patients with proven pathology. Some bone changes including abnormalities of the auditory ossicles have been better demonstrated than by conventional tomographic technigues, and high-resolution CT has some advantages in demonstrating both soft tissue abnormality and bone changes in the middle ear in primary neoplastic disease and secondary cholesteatoma. The absence of any practical means of taking…

[Malformations and abnormalities of the petrous portion of the temporal bone].

High-resolution computed tomography (HRCT) is the procedure of choice in the diagnostics of abnormalities of the middle and inner ear. It allows a detailed presentation of anatomical features and achieves the prerequisites for selection of the various therapeutic options. The highly diverse abnormalities can be described using detailed imaging analyses. Malformations with an abnormally developed modiolus are assumed to be early embryological defects, such as the classical Mondini dysplasia. The essential therapeutic option for middle ear deformities is still a cochlear implant. The domain of m

[Chronic inflammation of the petrous bone with fatal outcome].

We report the case of a 44-year-old woman, suffering from lethal cerebritis of the temporal lobe caused by an unusual chronic otitis media with granulations. A chronic inflammatory process of the left temporal bone had affected the patient over a period of 4 years. This had started with mastoiditis accompanied by a lesion of the dura mater, leading to labyrinthitis, encephalitis and cerebellar abscess. The patient's symptoms persisted, especially vertigo and uncertainty, and then deteriorated acutely. MRI showed otogenic cerebritis. Intracranial bleeding from vasculitis had worsened the patien

[Fractures of the petrous bone. An evaluation of 123 cases].

Out of 2,888 blunt head injuries that were treated at the Hermann Hospital Trauma Center of the University of Texas at Houston, only 123 temporal bone fractures were diagnosed in 110 patients. The clinical diagnosis was confirmed with high resolution computerized tomography of the temporal bones. Three main categories of fractures were identified: oblique in 74%, longitudinal in 13% and transverse in 13%. The clinical manifestations included hemotympanum in 83%, cerebrospinal fluid otorrhea in 21% and Battle's sign in 10.6%. One third of the patients had conductive hearing loss, 1/2 had mixed

[Aneurysmal cyst of the petrous bone. Apropos of a case].

Aneurysmal bone cyst (ABC) is a rare form of osseous dystrophy. A lesion of the petrous temporal bone appears to be extremely rare. It must be differentiated from a hemangioma which is a hamartoma affecting the geniculate ganglion or the internal auditory meatus. The case described involved a 23-year-old woman. The ABC presented with violent otalgia and severe endolabyrinthic-type perception deafness. It was associated with extensive bony destruction of the petrous temporal and occipital bones. Curettage only led to considerable through temporary regression of deafness and bone reconstruction
