[Phenotype change in hyperlipoproteinemias].

The spontaneous and the induced change of phenotype makes the correct interpretation of disturbance of fat metabolism on the base of phenotypization by Fredrickson more difficult. This is underlined by the present international practice to divide disturbances of fat metabolism for the time by increased lipids into hypercholesterinaemiae, hypertriglyceridaemiae and hypertriglyceride-hypercholesterinaemiae. The common trend, however, aims at characterization of disturbances of fat metabolism by changes of apoproteins.

The elite controller phenotype.

PURPOSE OF REVIEW: Achieving a functional cure for HIV remains a major challenge. In this context, elite controllers (ECs) - rare people living with HIV who maintain durable virological suppression without antiretroviral therapy - represent a unique and highly informative model. Recent years have seen major advances in the characterization of EC phenotypes across diverse geographic settings, as well as renewed interest in their relevance for HIV cure strategies. This review is considering emerging data refining EC heterogeneity, and the unresolved question of antiretroviral treatment indicatio

Pseudotumor Phenotype of Sarcoidosis.

Sarcoidosis is a benign multi-system granulomatous disease of unknown etiology. It can occasionally present with a pseudo-tumoral appearance, constituting a diagnostic pitfall that should be considered. We report the case of an 84-year-old patient with no significant medical history, admitted for the management of chronic dyspnea associated with chest pain evolving over the past six months, in the context of a preserved general condition. A chest CT scan revealed a pulmonary process, and the histopathological examination confirmed sarcoidosis. Through this case, we highlight the unique present

The False-negative Phenotype.

Ethical controversies may arise when genome sequencing reveals a genetic variant that is thought to be pathogenic, but the patient has no symptoms. This could be due to variable penetrance or expressivity. It could also result from a misclassification of the gene as pathogenic. In this article, I analyze 2 possibilities when such a situation occurs. The first is straightforward. We could conclude that the sequencing results should be considered a "false-positive" test result. The second is a bit more counterintuitive. In some cases, we could consider the test result to be a true-positive but i

From Genotype to Phenotype.

Genomic variants associated with inherited cardiac conditions yet detected incidentally (‘secondary findings’) are likely to arise with increasing frequency as genome sequencing transitions into clinical practice. Since genotyping has until recently been directed by clinical diagnosis, assessment and management of individuals found to harbour such a variant as a secondary finding is unclear. Here we illustrate some diagnostic and psychosocial complexities of inherited cardiac condition secondary findings, exemplified by disclosure of a pathogenic variant in KCNQ1, associated with long QT syndr

Acquired Bartter-Like Phenotype.

We present a case of middle-aged man who presented with sudden onset of weakness of both upper and lower limbs with hypotension and polyuria without any antecedent illness. Investigations showed severe hypokalemia, hypocalcemia, hyponatremia, mild hypomagnesemia, hypercalciuria, metabolic alkalosis and increased renin and aldosterone levels in the blood suggesting Bartter syndrome. Thus a diagnosis of acquired Bartter-like phenotype was made.

Childhood asthma-predictive phenotype.

Wheezing is a fairly common symptom in early childhood, but only some of these toddlers will experience continued wheezing symptoms in later childhood. The definition of the asthma-predictive phenotype is in children with frequent, recurrent wheezing in early life who have risk factors associated with the continuation of asthma symptoms in later life. Several asthma-predictive phenotypes were developed retrospectively based on large, longitudinal cohort studies; however, it can be difficult to differentiate these phenotypes clinically as the expression of symptoms, and risk factors can change

The allergic asthma phenotype.

Allergic asthma is the most common asthma phenotype. It usually is defined by the presence of sensitization to environmental allergens, although a clinical correlation between exposure and symptoms further supports the diagnosis. The average age of onset of allergic asthma is younger than that of nonallergic asthma. Although the spectrum of allergic asthma may vary from mild to severe, studies have reported that allergic versus nonallergic asthma is less severe. There is an increased prevalence of allergic rhinoconjunctivitis and atopic dermatitis in patients with allergic asthma. Total IgE le

The adult galactosemic phenotype.

BACKGROUND: Classic galactosemia is an autosomal recessive disorder due to galactose-1-phosphate uridyltransferase (GALT) deficiency. Newborn screening and early treatment do not completely prevent tremor, speech deficits, and diminished IQ in both sexes and premature ovarian insufficiency (POI) in women. Data on how individuals with galactosemia fare as adults will improve our ability to predict disease progression. METHODS: Thirty-three adults (mean age = 32.6 ± 11.7 years; range = 18-59) with classic galactosemia, confirmed by genotype and undetectable GALT enzyme activity, were evaluated.

DNA reviews: predicting phenotype.

The prediction of an individuals physical appearance from small biological samples, such as those collected from crime scenes may still sound like science fiction, but how close are we to achieving this goal? This review provides a brief introduction to the areas under investigation for direct and indirect phenotypic inference from DNA alone and suggests some sources of further reading for those interested in gaining a more in-depth knowledge of this complex subject.

Phenotype of triploid embryos.

The phenotypes of triploid fetuses and placentae are now well established and known to correlate with parental origin of the extra haploid set of chromosomes. In fetuses, it is not clear whether there is a direct parent of origin effect on the fetus itself or if the phenotypes are the result of growth differences influenced by abnormalities in growth and function of the placenta. Examining the phenotype of triploid embryos at an earlier stage in gestation, when the placenta effects may be less pronounced, could help clarify this question. A phenotype characteristic of triploidy in the embryoni

The carpenter syndrome phenotype.

Carpenter syndrome (Acrocephalopolysyndactyly type II), first described in 1901, consists of acrocephaly, syndactyly, polydactyly, congenital heart disease, mental retardation, hypogenitalism, cryptorchidism, obesity, umbilical hernia and bony abnormalities. We report a 6 years old boy presenting as a union of these malformations and also having bilateral sensorineural hearing loss. Auditory disturbances are not common among Carpenter syndrome patients. According to our knowledge, this is the first Carpenter syndrome case whose hearing loss is demonstrated by auditory brainstem response (ABR)

Albinism: phenotype or genotype?

As part of a combined ophthalmological, genetic, clinical, biochemical, ultrastructural and electro-physiological study of albinism we have examined over one hundred albinos, together with their heterozygote family members. Given this substantial number, we have developed a diagnostic protocol to facilitate albino classification and heterozygote detection. The major difficulty in the detection and differential diagnosis of albinism is that for a given albino, not all symptoms, either ophthalmological or cutaneous, may be manifest, whereas several may be in a non-albino. To compensate for the w

Prognostic value of the clinical phenotype in patients with cardiac sarcoidosis: SARCO phenotype.

BACKGROUND: Sarcoidosis, a multisystem granulomatous disease, can affect any organ and has various clinical presentations. Patients with sarcoidosis with cardiac involvement have a poorer prognosis than those without cardiac involvement. The prognostic value of the clinical manifestations at the time of cardiac sarcoidosis (CS) diagnosis remains unknown. METHODS: This secondary analysis of the ILLUMINATE-CS study evaluated the clinical characteristics and prognoses of patients with CS. The primary endpoint was a composite of all-cause death, hospitalization for heart failure (HF), and documen

Dynamic phenotype monitoring to prevent genotype-phenotype discrepancies in pharmacogenetic-guided drug therapy.

BACKGROUND: One of the limiting factors for the clinical application of pharmacogenomics (PGx) is phenoconversion, i.e., the dynamic discrepancy between genotype-predicted and actual drug-metabolizing phenotypes. Systemic inflammation, polypharmacy, transporter dysfunction, redox and mitochondrial stress, and epigenetic modification can rapidly alter cytochrome P450 (CYP) enzyme and transporter activity. This suppression can lead to unexpected poor metabolizing phenotypes, increased drug accumulation, and even an increased risk of drug-induced liver injury. Therefore, static PGx genotyping sho

Comprehensive study of thiopurine methyltransferase genotype, phenotype, and genotype-phenotype discrepancies in Sweden.

Thiopurines are widely used in the treatment of leukemia and inflammatory bowel diseases. Thiopurine metabolism varies among individuals because of differences in the polymorphic enzyme thiopurine methyltransferase (TPMT, EC 2.1.1.67), and to avoid severe adverse reactions caused by incorrect dosing it is recommended that the patient's TPMT status be determined before the start of thiopurine treatment. This study describes the concordance between genotyping for common TPMT alleles and phenotyping in a Swedish cohort of 12,663 patients sampled before or during thiopurine treatment. The concorda
