Phenoxybenzamine in the treatment of causalgia. Report of 40 cases.

Forty consecutive cases of causalgia treated during a 7-year period are presented. The patients ranged in age between 17 and 55 years, and all patients were males who received their nerve injuries from missile or shrapnel wounds. The greater occipital nerve was involved in two cases, median nerve in 10, sciatic nerve in 12, brachial plexus in seven, cauda equina in five, and multiple nerves in four cases. Each patient was treated with phenoxybenzamine, a postsynaptic alpha 1-blocker and presynaptic alpha 2-blocking agent. The drug was given orally in gradually increasing increments until a max…

The effect of phenoxybenzamine on the gastrin response to glycine stimulation.

The nature of adrenergic mechanism involved in gastrin release in response to local stimuli and feeding is still unclear. In the present study, the effect of phenoxybenzamine, a potent alpha-blocking agent, on the gastrin release in response to glycine perfusion (pH 7.0) of an isolated canine antral pouch was examined. The experiment was repeated with truncal vagotomy performed before glycine perfusion. It was found that gastrin concentrations measured via right gastroepiploic vein were significantly increased during phenoxybenzamine infusion with and without truncal vagotomy. This finding ind…

Effect of phenoxybenzamine (dibenzyline) on sexual function in man.

Phenoxybenzamine (Dibenzyline) has been extremely effective in treating patients with detrusor dyssynergia. Its minimal side effects include: mouth dryness, nasal congestion, drowsiness and fatigue, nausea and vomiting, palpitations, ejaculatory failure, and retrograde ejaculation. Nineteen men treated with phenoxybenzamine for detrusor dyssynergia reported ejaculatory failure during treatment; normal ejaculation returned after treatment was discontinued. Postmasturbation urine and semen samples were analyzed for sperm and fructose. The results of the study suggest that ejaculatory failure was…

[Phenoxybenzamine in prostatic pathology. 300 cases (author's transl)].

The authors have used alpha-blockers in the everyday treatment of prostatic symptoms for the past four years. The results are good and would appear to be more effective than the majority of other treatments. Efficacy is remarkable in dysuria and acute urinary retention. The authors report their experience based upon 300 cases. This alpha-blocker therapy using phenoxybenzamine makes it possible to delay operation and to avoid it in certain cases. In particular, it facilitates the pre- and post-operative phases. Marketing of this drug in France should be encouraged.

Phenoxybenzamine for benign prostatic obstruction. Review of 200 cases.

Two hundred patients treated with phenoxybenzamine for benign prostatic obstruction were reviewed. In 171 of these patients the treatment was given to relieve symptoms in patients not requiring operation, or in patients in whom operation was postponed. Eighty per cent of these patients derived benefit from the treatment, most commonly for the obstructive symptoms (78.7 per cent) but almost as often for nocturia (76 per cent). The treatment was also found to be useful in aiding catheter removal after an attack of acute retention, and in preventing recurrent attacks of acute retention. Side effe…

The blood volume in pheochromocytoma patients before and during treatment with phenoxybenzamine.

Total blood volume (TBV), red cell volume (RCV) and plasma volume (PV) were determined in 15 pheochromocytoma patients (9 males and 6 females) prior to and during preoperative treatment with phenoxybenzamine. Seventeen healthy male volunteers served as controls. Untreated male patients did not differ from the controls with respect to TBV, RCV or PV. In the total group of pheochromocytoma patients, phenoxybenzamine, in a dose of 145.6 +/- 45.2 (SD) mg/day over 14.3 +/- 5.7 (SD) days, induced significant increases in TBV (+9.4%) and PV (+14.5%), whereas RCV remained unchanged. During phenoxybenz…

[Effect of phenoxybenzamine on the renal and general circulation in the hypovolemic hypotonic dog].

For the estimation of the haemodynamic and metabolic effect of 1 mg/kg phenoxybenzamine in a kidney donor with craniocerebral trauma model experiments were performed on dogs with hypovolaemic hypotension. The results show that phenoxybenzamine leads to vasodilatation in a constant aortic pressure of 80 mm Hg in the systemic circulation as well as in the renal circulation with different speed of reaction. Thus the arterial and venous O2 and CO2 partial pressure normalizes and the diuresis increases. On the other hand, the hypotension-conditioned metabolic acidosis and the symptoms of an acute r…

Enhancement of phenoxybenzamine-induced irreversible blockade by organic Ca antagonists.

Organic Ca antagonists such as D600, diltiazem and nicardipine enhanced the irreversible blockade by phenoxybenzamine (POB) of the histamine response in guinea pig taenia caecum. The isotonic contractile response of the muscle was tested after incubation with POB (5 X 10(-6) M, 10 min) with or without an organic Ca antagonist. The blockade by POB was greater in muscle treated with an organic Ca antagonist and the effect lasted for more than 7 h.

Management of refractory hypotension during adrenalectomy in a dog treated with phenoxybenzamine.

Pheochromocytoma is a functional tumor of the adrenal medullary chromaffin cells that releases excessive secreted catecholamines, often malignant and invasive of blood vessels. Preoperative treatment with an alpha-1 adrenergic receptor antagonist, such as phenoxybenzamine, has been suggested to reverse chronic vasoconstriction A 12-year-old, 8 kg, female spayed Shih Tzu dog was presented for left adrenalectomy. Clinical signs included polyuria and polydipsia, and lethargy. Abdominal ultrasound revealed chronic renal changes and an adrenal mass with phrenicoabdominal vein and vena cava invasion…

Anti-tumor activity of phenoxybenzamine and its inhibition of histone deacetylases.

The principal finding from this study was the recognition that the α-adrenergic antagonist, phenoxybenzamine, possesses histone deacetylase inhibitory activity. Phenoxybenzamine is approved by the United States Food and Drug Administration for the treatment of hypertensive crises associated with tumors of the adrenal medulla, pheochromocytomas. It has several "off label" indications relative to its capacity to relax vascular smooth muscle and smooth muscle of the urogenital tract. The drug also has a long history of apparent efficacy in ameliorating, and perhaps reversing, the severe symptoms …
