We present an unusual case of a ruptured descending thoracic aortic aneurysm into the right pleural cavity of a patient with pectus carinatum. The presence of pectus carinatum played an important role in the development of the aneurysm at the atypical site and the rupture into the right pleural cavity. A small amount of right pleural bleeding on admission can increase and develop to massive hemothorax until emergency operation. Massive bleeding in the right pleural cavity where the dependent lung is located causes atelectasis and increased shunt fraction under one lung ventilation. Therefore, …

A 61-year-old man visited a community hospital because of hemosputum. A solitary nodule in the left lower lung field was pointed out on a chest roentgenogram. The patient was treated with antibiotics, but the solitary nodule increased in size. He was referred to our hospital because of high fever and observations of cavity formation and pleural effusion on a chest roentgenogram. The pleural effusion showed no cytologic evidence of malignancy, and cultures were also negative for bacteria. An increased percentage of lymphocytes was detected in the pleural effusion, but slight eosinophilia was fo

A very rare case is being described; common pleural cavity was accidentally diagnosed in a 3-year-old boy operated for funnel chest (pectus excavatum). During 36 years 516 patients were operated in our department and we often notice pectus excavatum associated with other types of congenital pathology but only one had the common pleural space. In normal human beings pleural space is divided into left and right chambers separated by the mediastinum with no communication in between. In some mammals such as pigs, cows etc. a congenital communication is found between the pleural cavities, but this …

INTRODUCTION: Recent studies have proposed primary empyema and demonstrated a correlation between it and the microbial composition of the or … ecific for the 16S rRNA gene.
RESULTS AND DISCUSSION: Compared with the primary empyema group, the pleural cavity microbial diversity of pneumonia complicated with empyema was significantly decreased (p < 0.05). At the phylum level, the relative abundance of Proteobacteria was significantly higher in the primary empyema group than pneumonia with empyema (p < 0. …

A 29-year-old female was admitted to our hospital complaining of severe dry cough and low-grade fever. A chest roentgenogram and a chest computed tomographic scan revealed a huge tumor and massive pleural effusion in the left pleural cavity. Pathological examination revealed spindle shaped malignant cells. SYT-SSX fusion gene transcripts were detected, so we diagnosed the tumor as a synovial sarcoma in the pleural cavity. After neoadjuvant chemotherapy we resected the tumor completely. A synovial sarcoma in the pleural cavity is extremely rare. To the best of our knowledge, this is the thirtee…

An extremely rare case of supernumerary spleen contained in the left pleural cavity is reported. Only one other case of this type is known. The anomaly, which was discovered by accident, may be explained by the probable delayed fusion of the mesenchymal tissue islands which form the spleen bud at the embryonic stage. One of these islands was imprisoned in the chest by the diaphragm, which was also in the final stages of development, thus creating the supernumerary spleen.

Description Strongyloides stercoralis is a soil-transmitted helminth that causes strongyloidiasis, a chronic parasitic infection in humans. S. stercoralis is one of several worm species that cause soil-transmitted helminthiasis, a neglected tropical disease. Herein, we discuss a 78-year-old female residing in a nursing home presenting with abdominal pain and shortness of breath. During a thoracentesis, physicians found multiple rhabditoid larvae consistent with S. stercoralis. However, before the procedure, a serial assessment of stool sample was performed and failed to demonstrate a parasitic

The role that pleural mesothelial cells play in leucocyte transmigration into the pleural cavity was investigated in lipopolysaccharide-stimulated mice. Changes in mesothelial cell morphology and changes in expression of adhesion molecules on mesothelial cells and leucocytes were analysed by light microscopy, immunohistochemistry, transmission electron microscopy (TEM) and immuno-scanning electron microscopy (immuno-SEM). After stimulation, the mesothelial cells separated completely from one another before leucocyte penetration across the mesothelial layer occurred. These changes occurred prim…

The authors describe 19 cases of rarely encountered complications of hydatid cyst of the lung: rupture into the pleural cavity in 13 patients and simultaneous rupture into the bronchus and pleural cavity in another 6 patients, which accounted for 2.4% of the total number of patients (840). These complications are serious and their diagnosis is made with certain difficulties. Complex roentgeno-bronchological and laboratory methods of examination must be used. Patients with the above-mentioned complications must be subjected to strictly individual surgical treatment in specialized clinics in the…

Central venous catheter (CVC) insertion is a routine procedure in the management of critically ill patients. We report a clinical case of inadvertent placement of an internal jugular vein CVC into the right pleural cavity, despite employing clinical and imaging-based techniques to ensure proper catheter positioning. Infusion of fluids and vasopressors through this misplaced catheter led to hypertensive pleural effusion and subsequent cardiorespiratory arrest. Return of spontaneous circulation was achieved after two cycles of cardiopulmonary resuscitation. While multiple imaging modalities are …

Congenital pulmonary arteriovenous fistula is frequently associated with hereditary haemorrhagic telangiectasia (Rendu-Osler-Weber disease). With the increased blood flow in pregnancy such fistulas enlarge, occasionally giving rise to haemothorax, which generally has a poor prognosis. A familial case is presented in which massive haemothorax required emergency thoracotomy in the 27th week of pregnancy.

Intrathoracic injections of bradykinin (1-100 micrograms/cavity) induced a dose-dependent increase in the number of eosinophils recovered from the rat pleural cavity 24 h later. Eosinophilia by bradykinin was preceded by a marked pleural neutrophil influx within 6 h and was absent only 72 h following stimulation. Bradykinin (10(-9)-10(-5) M) failed to induce in vitro eosinophil chemotaxis, indicating that its in vivo effect must be mediated by an intermediate messenger. BW 755C (25 mg/kg) and the more selective lipoxygenase inhibitor BW A4C (20 micrograms/cavity) suppressed the pleural eosinop…

Introduction The aim of the study was to evaluate the results of surgery to remove huge mediastinal masses and their pathology. Surgical resection was chosen for accurate diagnosis and treatment of the huge mediastinal masses extending into the pleural cavity. Methods Records were reviewed for eight patients who had the diagnosis of huge benign mediastinal masses and who underwent operation; details of the patients and operations were recorded. Results Mean age was 34.5 (range 22 to 44) years, and male-to-female ratio was 2:6. Computed tomography and magnetic resonance imaging (MRI) were us…

A 37-year-old man with a huge pleural cyst, presented with symptoms of right heart compression. The mass on the right side of the chest seemed initially to be in connection with the mediastinum. Computed tomography failed to define its relationship with the pericardium, and echocardiography excluded any involvement of the mediastinal structures. The final diagnosis was a congenital thymic cyst exclusively located in the pleural cavity.

Changes in eosinophil counts after intrathoracic (it) injection of endotoxin (LPS) were investigated in Wistar rats (150-180 g). Increasing doses of endotoxin (62.5-500 ng/cavity) induced a dose-dependent increase in the number of eosinophils recovered from the rat pleural cavity 24 h later. The eosinophilia was apparent within 24 h, peaked within 48 h (from 0.76 +/- 0.12 to 3.68 +/- 0.51 eosinophils x 10(6)/cavity, P less than 0.001) and returned to basal levels 120 h after the it injection of endotoxin (250 ng/cavity). Endotoxin (3 ng-4 micrograms/ml) failed to attract eosinophils in vitro u…
