Acute Intermittent Porphyria: A Diagnostic Conundrum.

This case report presents the case of a 20-year-old female patient who sought emergency medical attention for severe abdominal pain, nausea and vomiting, tachycardia, hypertension, and discolored urine. Initial diagnostic evaluations yielded no significant abnormalities; however, subsequent analysis revealed elevated urinary porphobilinogen, corroborating a diagnosis of acute intermittent porphyria (AIP). The patient's medical history included recurrent urinary tract infections and a prior episode of syndrome of inappropriate antidiuretic hormone secretion (SIADH), in conjunction with psychiat

[Acute intermittent porphyria: a diagnostic dilemma].

Acute intermittent porphyria (AIP) is a rare condition characterized by abdominal pain and a wide range of nonspecific symptoms. We report the case of a 24-year-old woman with syndrome of inappropriate secretion of antidiuretic hormone (SIADH) and motor and sensory neurological deficits due to an attack of acute porphyria. The patient presented to the emergency department with abdominal pain. The results of physical examination and laboratory investigations were normal. Two days after admission, the patient developed seizures as a result of hyponatremia due to SIADH, which, together with the o

Respiratory failure in acute intermittent porphyria.

We report two patients of acute intermittent porphyria (AIP) who presented with acute respiratory failure. Only one such previous report could be found. Occasionally, neuropathy may be the presenting feature in AIP which may progress to respiratory embarrassment. The cause of this neuropathy has been hypothesized to be direct neurotoxicity of delta-ALA by interaction with GABA receptor, altered tryptophan metabolism and may be heme depletion in nerve cells.

Acute intermittent porphyria and hormone disbalance.

A study of acute intermittent porphyria (AIP) in three female patients indicates the necessity of stressing that in cases of clinical and laboratory exacerbation of the disease in patients of reproductive age the possibility of the existence of pathological changes in the ovaries (polycystic ovary, anovulatory cycle) should be taken into consideration. By examination of patients' endocrinological status and on the basis of the experience of clinicians it can be concluded that in such cases of exacerbation of the disease, apart from conventional therapy with glucose, Panhematin, electrolyte cor

[False surgical acute abdomen with intermittent acute porphyria].

This article presents a case of acute intermittent porphyria admitted to the Surgery Department of C.F. Craiova Hospital between 18.08.2003-26.08.2003 then transferred to the Colentina Hospital in Bucharest for diagnosis confirmation and adequate treatment. The purpose of this paper is to bring attention on a rare metabolic inherited disease that, due to its non-specific and often noisy symptoms and limited possibilities of biochemical, enzymatic and genetic diagnosis, could generate potential serious confusions. The presentes case illustrates the fact that sometimes the acute attack may be mi

New form of dual porphyria: coexistent acute intermittent porphyria and porphyria cutanea tarda.

A previously unrecognized form of dual porphyria has been identified in four patients. One male and one female with acute symptoms were diagnosed as having acute intermittent porphyria (AIP), and two males with cutaneous and acute symptoms were diagnosed as having porphyria cutanea tarda (PCT). Biochemically, the excretion of haem precursors showed a complex constellation, with signs characteristic of both AIP and PCT. In one male, a clinical course with both overt PCT and acute manifestations of AIP was observed. Enzyme studies of haem biosynthesis in erythrocytes revealed a dual deficiency,

Rhabdomyolysis in a patient with acute intermittent porphyria.

There are several conditions associated with rhabdomyolysis, such as direct muscle injury, viral infections, metabolic disorders and toxic effects from ingestion of drugs such as alcohol, opiates, cocaine or heroin. We report on a patient who was admitted to the accident and emergency unit with a clinical presentation of rhabdomyolysis and acute intermittent porphyria.

[Cimetidine in treatment of acute intermittent porphyria].

Two female patients with repeated exacerbations of acute intermittent porphyria (20 exacerbations in 44 and 5 in 34 months, respectively) were treated with cimetidine (2 x 400 mg/d) over 4 months. Therapeutical aim of the medication was to prevent further porphyria manifestations. In contrast to previous casuistic reports and experimental studies in laboratory animals cimetidine did not stabilize porphyrin biosynthesis. One patient had an acute attack during the period of treatment, while the other had no further exacerbations. However, cimetidine did not worsen the clinical course of our pati

Caring for patients with acute intermittent porphyria.

The porphyrias are a group of metabolic disorders of heme biosynthesis genetically determined defects. Acute intermittent porphyria is the most common form of porphyria found in the United States. It is caused by a genetic defect in chromosome 11, where one of two genes for porphobilinogen deaminase is defective. Acute intermittent porphyria is characterized by intermittent, acute, occasionally fatal attacks of abdominal, neurologic, psychiatric, and renal symptoms. Attacks are often confused with acute abdomen or bowel obstruction. A variety of drug, hormonal, nutritional, and infectious fact

Acute intermittent porphyria: analgesia can be dangerous.

Acute intermittent porphyria (AIP) is a rare condition, a metabolic disorder of the haem biosynthesis. An acute crisis of AIP can present as a combination of symptoms, such as abdominal pain, autonomic dysfunction, hyponatremia, muscle weakness and neurological symptoms in the absence of others obvious causes. We report the case of a 53-year-old woman, who was previously diagnosed with AIP 5 weeks after therapeutic suspension has developed an acute disease exacerbation. During hospitalisation, further exacerbation has occurred after analgesia with metamizole. Glucose and hemin infusions result

Tubulointerstitial nephritis associated with acute intermittent porphyria.

Progressive renal impairment associated with acute intermittent porphyria is not well recognized and the mechanism of renal damage remains unclear. We report a case of a 51-year-old female with acute intermittent porphyria and long-term follow-up who developed proteinuria and renal insufficiency. Her biopsy showed marked tubulointerstitial damage with mitochondrial abnormalities. Urinary excretion of lipid peroxidation was increased compared to healthy controls. The porphyrin precursors may increase lipid peroxidation products and damage mitochondria leading to tubulointerstitial nephritis.

Infectious mononucleosis presenting as acute intermittent porphyria.

We have described a patient with infectious mononucleosis, confirmed by serologic studies, who had an initial episode of acute intermittent porphyria during the course of the infection. Although infections have been implicated in precipitating AIP, infectious mononucleosis has rarely, if ever, been described. A similar constellation of symptoms has been described for both infectious mononucleosis and AIP. Although infectious mononucleosis is a common infection, with 90% to 95% of the United States population showing seropositivity by adulthood, neurologic complications are uncommon. Acute inte

Takotsubo Cardiomyopathy Triggered by Acute Intermittent Porphyria.

Background Takotsubo cardiomyopathy (TC) is a reversible condition characterized by myocardial akinesis due to catecholamine-mediated myocardial stunning. Acute intermittent porphyria (AIP) is associated with a rise in catecholamine, which could trigger TC. This study aims to evaluate patients with porphyria-triggered TC. Methods Data from the National Inpatient Sample (NIS) was used to study the prevalence rate and clinical outcome of porphyria-triggered TC among patients with TC. Results Overall, 32,500 cases were admitted between 2012 and 2016 with TC. The rates of smoking, hypertension, hy
