Precipitating antibodies to avian antigens play a controversial role in the diagnosis of bird breeder's disease (BBD). In order to establish the sensitivity, specificity and accuracy of precipitins in our laboratory, we conducted a prospective study including 128 sera received in 1988 for determination, by immunoelectrophoresis, of avian precipitins. Accurate information was obtained for 90 patients; definitive clinical diagnosis was given by the patient's own physician. Sera from certain patients with Hashimoto's disease or a related condition contain a non-precipitating antithyroglobulin which produces in double-diffusion tests with thyroglobulin a line of clearing in the agar. The antigen participating in the tube precipitin (TP) serologic test for coccidioidomycosis was isolated from mycelial-phase antigen (coccidioidin) by immunoaffinity and characterized by various analytical procedures. In dairy farmers exposed to the microbes present in hay, precipitating antibodies against these microbes are frequently found regardless of the state of health of the farmer. The prognostic value of these antibodies for the future health and working ability of farmers was studied in a six-year follow-up survey of 292 farmers. A metacercarial precipitin test was devised for diagnosis of human fascioliasis. The sera tested were those of cases of fascioliasis (acute and chronic), of schistosomiasis and of normal healthy individuals. Metacercariae were prepared in the laboratory from naturally infected snails. Serum dilutions of 1:5 (20%) and 1:10 (10%) were incubated with metacercariae for periods of 6, 24 and 48 hours. The recent outbreaks of human fascioliasis in Britain provided the opportunity for employing the double diffusion precipitin test in the diagnosis of this disease. In sensitivity and specificity this test appears to equal the complement-fixation test and has the advantages of simplicity and speed in its performance.
