[Accommodation and presbyopia : part 1: physiology of accommodation and development of presbyopia].

Accommodation is a dynamic change in the dioptric power of the eye. According to the widely accepted and experimentally confirmed theory of Helmholtz, it is achieved by release of zonular tension with contraction of the ciliary muscle and consecutive modelling of the shape of the crystalline lens by the elastic lens capsule. The ability to accommodate is gradually lost with age (presbyopia). Because of difficulties in examining the accommodative apparatus in vivo, many theories, in part contradictory, about the mechanism of accommodation and the origin of presbyopia have been developed. In rec…

Correction of presbyopia.

Presbyopia, a progressive visual difficulty caused by weakened physiological regulation, is one of the main causes of visual impairment in people over 40 years old. Currently, the main methods of correction of presbyopia include optical correction, surgical correction, and drug treatment, which can improve the visual nearness disorder to some extent. Optical correction is the most common way with advantages of safety, which can adjust the lens parameters at any time, while cause kinds of inconvenience in life by wearing and taking off glasses frequently. Surgical intervention, including cornea…

Accommodation and presbyopia.

The mechanism of accommodation has been studied for at least four hundred years. The most interesting aspect of accommodation is that its time course is well in advance of other physiological functions--it begins to decline by adolescence and is lost about two-thirds of the way through the normal life span. The state of presbyopia is reached when accommodation has declined sufficiently to interfere with close tasks requiring acute vision. Presbyopia is generally considered to originate with the 'plant' of the accommodative system, either within the lens and its capsule or within their support …

Impact of Refractive Status on Presbyopia Progression among Patients with Presbyopia.

PURPOSE: To explore the impact of refractive status on presbyopia progression among patients with presbyopia. METHODS: This retrospective observational study included patients with presbyopia who visited the Seventh Affiliated Hospital of Sun Yat-sen University and Shenzhen Polytechnic Medical College between May 2018 and August 2022. The amplitude of accommodation (AMP) and near addition power (ADD) at 6 months and 1 year were collected. RESULTS: A total of 103 patients with presbyopia were included in this study: 42 patients with myopia, 23 patients with emmetropia, and 38 patients with hy…

[Accommodation and presbyopia : part 2: surgical procedures for the correction of presbyopia].

There are two basic approaches for the surgical correction of presbyopia: increasing depth of focus (e.g. by means of multifocal laser abrasion of the cornea), Or restoring accommodation in the sense of a dynamic change in ocular refraction (accommodative intraocular lenses, scleral expansion). Pseudoaccommodative procedures are able to achieve satisfactory near vision, albeit at the price of lower performance in the intermediate range and decreased image quality. The restoration of accommodation remains problematic, partly because the mechanism of accommodation and the development of presbyop…

Refraction during incipient presbyopia: The Aston Longitudinal Assessment of Presbyopia (ALAP) study.

PURPOSE: To investigate non-cycloplegic changes in refractive error prior to the onset of presbyopia. METHODS: The Aston Longitudinal Assessment of Presbyopia (ALAP) study is a prospective 2.5 year longitudinal study, measuring objective refractive error using a binocular open-field WAM-5500 autorefractor at 6-month intervals in participants aged between 33 and 45 years. RESULTS: From the 58 participants recruited, 51 participants (88%) completed the final visit. At baseline, 21 participants were myopic (MSE -3.25±2.28 DS; baseline age 38.6±3.1 years) and 30 were emmetropic (MSE -0.17±0.32 D…

Pharmacological treatment of presbyopia.

Presbyopia is a very common condition, which can easily be treated with reading glasses. Although a number of contact lenses and surgical solutions have been introduced, also a number of pharmacological treatments have been proposed in recent years for treating presbyopia. As presbyopia is a multifactorial condition, a number of pharmacological approaches have been explored so far. Most eye drops contain miotic/parasympathomimetic agents (e.g. pilocarpine, carbachol, aceclidine), which create a pinhole effect. Pilocarpine and aceclidine already have been approved. Brimochol (carbachol + brimon…

Pharmacological Treatments for Presbyopia.

Accommodation is the change in dioptric power of the eye. It is a dynamic process that allows focusing on an object at all distances. In order to focus sharply, three physiological responses, known as the triad of accommodation, are produced by a change in pupil size, a change in shape and position of the lens, and ocular convergence. This is modulated by the autonomic nervous system, mainly the parasympathetic nervous system. Presbyopia is a refractive condition that occurs with aging, usually manifesting around 40-50 years of age, and is a result of the loss of accommodation in the eye, caus…
