Pylorus-preserving pancreaticoduodenectomy.

In patients with resectable adenocarcinoma of the head of the pancreas, pancreaticoduodenectomy serves as the cancer-directed operation of choice. The pylorus-preserving modification of pancreaticoduodenectomy is commonly performed in this setting. Reconstruction of the gastrointestinal tract following pylorus-preserving pancreaticoduodenectomy can be accomplished using various techniques. The operative mortality rate in experienced hands is generally less than 3%, with a postoperative complication rate in the range of 35% to 40%. Patient survival following resection is largely determined by t…

Pylorus preserving pancreaticoduodenectomy.

We have performed 9 pylorus preserving pancreaticoduodenectomies (PPPD) for periampullary malignancies. In these 9 patients, six had been diagnosed as having carcinoma of the ampulla of Vater, two pancreatic carcinoma and one duodenal carcinoma. We have no operative mortality and a 44 per cent rate of morbidity. Complications include wound infections (3), pleural effusion (1), intraabdominal abscess (1), delayed gastric emptying (1). There was neither biliary nor pancreatic leakage in these patients. None of the patients had gastrointestinal haemorrhage and marginal ulceration. We have complet…

Acquired double pylorus.

We have reported two cases of double pylorus or duplicated pyloric channel as an unusual complication of peptic ulcer disease. It is usually noted as an incidental finding on x-ray films or at endoscopy. Double pylorus seems to be an acquired lesion arising from gastric ulceration and does not necessarily require surgical correction.

Double channel pylorus.

Radiographic and clinical features in five patients with the double channel pylorus deformity are reviewed and compared with previously reported cases. The double channel pylorus is a form of gastroduodenal fistula with a characteristic radiographic appearance. An accessory channel connects the lesser curvature of the prepyloric antrum with the duodenal bulb. A peptic ulcer lies in or immediately adjacent to the accessory channel in almost all cases. The double channel pylorus appears to be an acquired form of peptic deformity in the great majority of cases and is probably more common than the…

Congenital double pylorus.

The double pylorus is an uncommon finding and maybe congenital due to gastrointestinal duplication abnormality or more commonly secondary to peptic ulcer disease. The case we present is an elderly patient with mild dyspeptic symptoms who had an upper endoscopy as part of her investigative workup. The congenital double pylorus, being asymptomatic, may often go undetected. It is sometimes found incidentally on upper endoscopy, but needs no directed therapy. It is not associated with any specific complication.

Is there comparable morbidity in pylorus-preserving and pylorus-resecting pancreaticoduodenectomy? A meta-analysis.

Pancreaticoduodenectomy (PD) is the most effective treatment for patients with pancreatic head or periampullary lesions. Two major strategies exist: pylorus-preserving pancreaticoduodenectomy (PPPD) and pylorus-resecting pancreaticoduodenectomy (PRPD). However, it is yet unclear regarding the morbidity after PPPD and PRPD. This study analyzed the morbidity after PPPD and PRPD to determine the optimal surgical treatment of masses in the pancreatic head or periampullary region. A systematic search of databases identifying randomized controlled trials (RCTs) from the Cochrane Library, PubMed, EMB…

The Case of the "Double Pylorus".

A 68-year-old female with a history of small cell carcinoma of the lung and peptic ulcer disease presented for evaluation of dysphagia after undergoing radiation and chemotherapy. Esophagogastroduodenoscopy (EGD) revealed a radiation-induced esophageal stricture and incidentally a "double pylorus (DP)" with an adjacent similar opening cephalad to the pylorus. There was no evidence of perforation or traumatic injury leading to this finding and upon intubation, both openings led to the duodenal bulb that appeared to be normal. No endoscopic and surgical intervention was offered as patient was as…

Double pylorus in cirrhosis.

Double pylorus is an uncommon clinical condition that can be acquired or congenital. Most acquired cases arise as a complication of peptic ulcer disease and less commonly from other conditions such as gastric malignancy. We present a case of double pylorus in a cirrhotic patient diagnosed during surveillance endoscopy for esophageal varices.
