Computed tomography is currently used as an adjunct to oral contrast studies in the diagnosis and management of esophageal and posterior pharyngeal perforation. We present the CT findings of a surgically proven iatrogenic pyriform sinus perforation. Although extravasation of contrast material was depicted by pharyngography, the site of perforation could not be demonstrated. Knowledge of the pattern of spread of pharyngeal neoplasms was essential for correct interpretation of CT. This is a review of 418 patients with cancer of the pyriform sinus who had complete treatment at University of Texas M. D. Anderson Hospital, treated over a 28-year time span. Treatment modalities used included radiation therapy, surgery, and planned combined treatment, primarily in the form of surgery and postoperative radiation therapy. Radiation therapy is sometimes prescribed as an alternative to partial or total laryngopharyngectomy in the initial management of carcinoma of the pyriform sinus. Forty-two patients were irradiated for cure and have a follow-up period of 2-14 years. Local control by irradiation alone was 11 of 14 for T1, 6 of 10 for T2, 3 of 6 for T3, and 1 of 7 T4. Three T2 failures were salvaged by laryngopharyngectomy for an ultimate control of 9 of 10 for T2. At the Roswell Park Memorial Institute, 111 patients were treated for squamous cell carcinoma of the pyriform sinus over a 15-year period. Treatment modalities included surgery, radiation therapy, or a combination of both. A retrospective study was carried out to determine how these treatments, used singly or in combination, affected the cure rate. TNM clinical staging and recurrence patterns were studied, and a high tendency for distant metastases was documented. The five-year survival figure in this study was 29% (28/97).

In early primary lesions, many different approaches have been tried for larynx preservation. Management of third and fourth branchial cleft anomalies are similar. These anomalies should be suspected in a child with recurrent low-anterior neck abscess. Investigations in the form of cross-sectional studies and examination of the pharynx under anaesthesia will facilitate diagnosis and resolution of abscess. Spontaneous closure of the pyriform sinus can occur following conservative management with antibiotic treatment and abscess drainage. This emphasise the role of second-look prior to implementing endoscopic cauterisation or surgery.
