Image-guided stereotactic radiotherapy for patients with vestibular schwannoma. A clinical study.

INTRODUCTION: Local tumor control and functional outcome after linac-based stereotactic radiosurgery (SRS) and fractionated stereotactic radiotherapy (FSRT) for vestibular schwannoma (VS) were assessed. METHODS: In all, 250 patients with VS were treated: 190 patients with tumors < 2 cm diameter underwent SRS and 60 patients with tumors >2 to 3.5 cm underwent FSRT. Dose prescription for all cases with SRS (n = 190, 76 %) was 13.5 Gy. For FSRT, mainly two hypofractionated schedules (n = 60, 24 %) with either 7 fractions of 5 Gy (total dose: 35 Gy; n = 35) or 11 fractions of 3.8 Gy (total dose:

Image-guided radiotherapy platform using single nodule conditional lung cancer mouse models.

Close resemblance of murine and human trials is essential to achieve the best predictive value of animal-based translational cancer research. Kras-driven genetically engineered mouse models of non-small-cell lung cancer faithfully predict the response of human lung cancers to systemic chemotherapy. Owing to development of multifocal disease, however, these models have not been usable in studies of outcomes following focal radiotherapy (RT). We report the development of a preclinical platform to deliver state-of-the-art image-guided RT in these models. Presence of a single tumour as usually dia

Daily ultrasound-based image-guided targeting for radiotherapy of upper abdominal malignancies.

PURPOSE: Development and implementation of a strategy to use a stereotactic ultrasound (US)-based image-guided targeting device (BAT) to align intensity-modulated radiotherapy (IMRT) target volumes accurately in the upper abdomen. Because the outlines of such targets may be poorly visualized by US, we present a method that uses adjacent vascular guidance structures as surrogates for the target position. We assessed the potential for improvement of daily repositioning and the feasibility of daily application. METHODS AND MATERIALS: A total of 62 patients were treated by sequential tomotherapeu

Dosimetric evaluation of image-guided adaptive radiotherapy for locally advanced cervical cancer.

This study evaluates the dosimetric benefits of off-line adaptive radiotherapy (ART) planning during radiotherapy for locally advanced cervical cancer. Forty-four patients in our hospital were included. The patients were monitored by cone-beam CT (CBCT), and the secondary CT scanning was performed timely. The ART2 planning was performed based on tumor regression and compared with the initial radiotherapy planning (ART1). The mean time of the secondary CT scanning was the thirteen fractions, and the mean gross tumor volume (GTV) decreased by 23.3%. The ART2 compared with the ART1 planning, sign

Antibody-Targeted Bismuth Gadolinium Nanoconjugate for Image-Guided Radiotherapy of Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC), one of the most lethal cancers of the liver, has limited treatment options at advanced stages. Here, bismuth gadolinium (BiGd) nanoparticles (NPs) conjugated with anti-vascular endothelial growth factor antibody (aVEGF) are designed and tested for targeted image-guided radiation therapy against HCC. The BiGd NPs are synthesized using the sol-gel technique, functionalized with silica NPs, and labeled with fluorescent protamine-rhodamine B. For tumor targeting, the NPs are conjugated with aVEGF, and an in vitro study confirms the binding of the aVEGF-BiGd nanoconj

Investigation of pelvic floor influence on prostate displacement in image-guided radiotherapy.

PURPOSE: The uncertainty of target location during prostate cancer radiotherapy plays an important role in accurate dose delivery and radiation toxicity in adjacent organs. This study analyzed displacement correlations between the prostate and pelvic floor. METHODS AND MATERIALS: We retrospectively analyzed registration results from 467 daily cone-beam computed tomography (CT) in 12 patients with prostate cancer who received radiation therapy. We analyzed prostate displacement and the pelvic floor relative to the pelvic bone's anatomy in the translational and rotational directions and identif

Image-Guided Radiotherapy for Gynecologic Malignancies: What the Radiologist Needs to Know.

Pelvic imaging is integral to contemporary radiotherapy (RT) management of gynecologic malignancies. For cervical, endometrial, vulvar, and vaginal cancers, three-dimensional imaging modalities aid in tumor staging and RT candidate selection and inform treatment strategy, including RT planning, execution, and posttherapy surveillance. State-of-the-art care routinely incorporates magnetic resonance (MR) imaging, 18F-fluorodeoxyglucose-PET/computed tomography (CT), and CT to guide external beam RT and brachytherapy, allowing the customization of RT plans to maximize patient outcomes and reduce t

Effect of rectal enema on intrafraction prostate movement during image-guided radiotherapy.

INTRODUCTION: Rectal volume and movement are major factors that influence prostate location. The aim of this study was to assess the effect of a rectal enema on intrafraction prostate motion. METHODS: The data from 12 patients with localised prostate cancer were analysed. Each patient underwent image-guided radiotherapy (RT), receiving a total dose of 70 Gy in 28 fractions. Rectal enemas were administered to all of the patients before each RT fraction. The location of the prostate was determined by implanting three fiducial markers under the guidance of transrectal ultrasound. Each patient un

The use of fiducial markers in image-guided radiotherapy for gastric cancer.

The use of fiducial markers (FM) in image-guided radiotherapy (IGRT) to increase treatment precision is emerging for upper gastrointestinal malignancies. To our knowledge there is no data beyond technical reports for the use of FMs in IGRT for gastric cancers in the current literature. We report a case of an 89-year old gentleman with localised gastric cancer who was deemed unfit for surgery and chemotherapy. He had FMs inserted endoscopically around the tumour via ultrasound guidance and received radiotherapy with a high-dose palliative intent via a two-phase technique to 54 Gy in 30 fraction

Phase II trial of concurrent sunitinib and image-guided radiotherapy for oligometastases.

BACKGROUND: Preclinical data suggest that sunitinib enhances the efficacy of radiotherapy. We tested the combination of sunitinib and hypofractionated image-guided radiotherapy (IGRT) in a cohort of patients with historically incurable distant metastases. METHODS: Twenty five patients with oligometastases, defined as 1-5 sites of active disease on whole body imaging, were enrolled in a phase II trial from 2/08 to 9/10. The most common tumor types treated were head and neck, liver, lung, kidney and prostate cancers. Patients were treated with the recommended phase II dose of 37.5 mg daily suni

Imaging dose in image-guided radiotherapy for localized prostate intensity-modulated radiotherapy: a nationwide survey in Japan.

This study aimed to establish the diagnostic reference levels (DRLs) of imaging doses for image-guided radiotherapy (IGRT) used in intensity-modulated radiotherapy for prostate cancer in Japan. A nationwide survey was conducted to gather data on image acquisition conditions, parameters, and frequencies across 193 radiation therapy institutions using intensity-modulated radiotherapy. IGRT modalities, such as kilovoltage and megavoltage cone-beam computed tomography (CBCT), two-dimensional imaging, and in-room computed tomography (CT), were targeted. Data analysis focused on image acquisition pa

Prior information-based subtraction radiography for enhanced tumor visibility in image-guided radiotherapy.

PURPOSE: Two-dimensional (2D) X-ray imaging is routinely used in radiotherapy for patient alignment but offers limited tumor visibility due to anatomical overlap. We propose a novel technique, prior information-based subtraction (PIBS) radiography, to enhance tumor visualization on 2D X-ray and facilitate more accurate patient alignment. MATERIALS AND METHODS: PIBS X-ray improves tumor visualization on 2D X-ray by subtracting signal contributions from non-essential (NE) tissues, which can be obtained from the planning CT (prior information). To demonstrate the feasibility, we acquired four CT

A viscous fluid model for large-scale motion estimation in image-guided radiotherapy.

BACKGROUND: Image-guided radiotherapy (IGRT) is often hampered by geometric inaccuracies introduced by anatomical and physiological motion. Although a wide array of deformable image registration (DIR) solutions have been proposed toward motion estimation and compensation during IGRT, their accuracy and precision generally deteriorates within areas showcasing particularly large displacements such as the thorax, abdomen and pelvis. PURPOSE: In this work, we propose a physics-derived DIR algorithm for motion estimation during IGRT, designed to be specifically suitable for highly deforming anatom

Evaluation and mitigation of deformable image registration uncertainties for MRI-guided adaptive radiotherapy.

PURPOSE: We evaluate the performance of a deformable image registration (DIR) software package in registering abdominal magnetic resonance images (MRIs) and then develop a mechanical modeling method to mitigate detected DIR uncertainties. MATERIALS AND METHODS: Three evaluation metrics, namely mean displacement to agreement (MDA), DICE similarity coefficient (DSC), and standard deviation of Jacobian determinants (STD-JD), are used to assess the multi-modality (MM), contour-consistency (CC), and image-intensity (II)-based DIR algorithms in the MIM software package, as well as an in-house devel
