This is the first report of a case of aesthetic calf augmentation with microvascular transferred autologous material. The technique regarding to calf augmentation using silicon prostheses is described and the advantages and disadvantages of this method are discussed. This should incite discussion on the use of free-flap technique in aesthetic operations.

Eighty patients receiving rectus abdominis free-tissue transfers are discussed. The operations were performed between July of 1983 and December of 1986. Specifically, the patients were followed in the clinic to determine the extent and degree of donor-site morbidity. Sixty-two flaps were transferred to the lower extremities, 15 to the head and neck, 2 to the arm, and 1 was placed intrathoracically. The authors present their experience with a previously described but infrequently used variation of the rectus abdominis myocutaneous flap. Skin paddles angled obliquely from the line of the rectus abdominis and toward the rib cage were successfully carried on periumbilical perforators from the inferior epigastric system. Extraskeletal osteosarcoma is a rare malignant tumor with an osteoid or bone matrix that develops in soft tissues that are not in contact with bone or periosteum. A 47-year-old woman presented with a subcutaneous mass in her right abdomen. A needle biopsy revealed a giant cell tumor of the soft tissue. Denosumab was administered, but the tumor enlarged. Wide tumor resection was performed. Histopathological analysis confirmed the diagnosis of extraskeletal osteosarcoma. The patient remains alive and disease-free one year after the surgery. We examined four patients with a demand pacemaker who exhibited transient symptoms of vertigo while trying to rise from the supine position. In two of these there was an epicardial pacing system with the pacemaker in the abdominal wall superficial to the rectus abdominis muscle; in the other two cases the pacing system was transvenous with the pacemaker superficial to the right pectoralis major muscle. A case of rectus sheath haematoma and essential thrombocytosis during anticoagulation treatment is described. The difficulties encountered in reaching a clinical, differential diagnosis between rectus sheath haematoma and other abdominal acute conditions are stressed.
