Despite recent advance in diagnosis and surgical techniques of acoustic neuroma, ideal criteria for hearing preservation attempt remain unsatisfactory. This study reports sonomotor vestibular reflex recorded pre and postoperatively in 20 patients with acoustic neuroma. Hearing preservation was attempted according to tumor size and preoperative pure tone audiometry results. The studied population was divided in 2 groups according to hearing preservation was attempted (group 1 n = 13, retrosigmoid approach) or not (group 2 n = 7, translabyrinthin approach). In accordance with postsurgical auditi

A commercially available impedancemeter was modified to permit measurement of hearing thresholds. Both the ipsilateral and the contralateral acoustic reflex thresholds were determined in dB SL in a group of 20 young adults with normal hearing. No significant differences in reflex thresholds were obtained.

Acoustic reflex impedance measurements are made on forty-seven normal and two otosclerotic ears using fixed probe frequencies from 220 to 2000 Hz. In general, the reflex increases the middle ear impedance up to 700 Hz probe frequency and decreases it thereafter. It is postulated that this characteristic is related to removal of the cochlear fluid resistance due to stapes decoupling. At one probe frequency in the range 500--900 Hz, the reflex of 50% of the normal ears has either a diphasic pattern or shows no change in the impedance vector length. The accompanying phase change is always monopha

Acoustic reflex characteristics were studied in six persons with spastic dysphonia. Two methods of analysis were used: (1) a double-blind qualitative judgment of acoustic reflex recordings, and (2) a quantitative analysis of specific temporal characteristics of the acoustic reflexes. Judgments of reflex recordings did not reliably differentiate spastic dysphonia subjects from controls. An analysis of five latency measures revealed a difference between groups only in one measure, reflex contraction time.

OBJECTIVE: To determine average acoustic reflex thresholds in geriatric groups by assessing ipsilateral and contralateral acoustic reflex and reflex decay tests.
METHODS: A total of 25 elders between ages 65-84 years (74.3±5.4) and 25 individuals between ages 18-45 years (30.4±4.2) were recruited for the study. After ear, nose, and throat examination, ipsilateral and contralateral acoustic reflex thresholds at 500, 1000, 2000, and 4000 Hertz (Hz) were determined and a reflex decay test at contralateral 500 Hz was conducted. Ipsilateral acoustic reflex thresholds were obtained with high-freque

OBJECTIVE: The objective of this study was to examine the role of the acoustic stapedius reflex in the protection of speech recognition from the upward spread of masking arising from low-frequency background noise.
DESIGN: Speech recognition scores were measured for nine control participants (19-34 years) and six patients with transected stapedius tendons poststapedotomy (39-57 years) as a function of the amplitude of a low-frequency masker, presented at nominal signal to noise ratios of +5 dB, -5 dB, and -15 dB. All participants had pure-tone hearing thresholds in the normal range. Continuou

Contralateral and ipsilateral acoustic reflex adaptation (decay) were examined using 500-, 1000-, and 2000-Hz tonal activators for stimulation periods of 30 seconds in 30 normal subjects. For these normal subjects, ipsilateral adaptation always exceeded contralateral adaptation, although differences between the two conditions were not significant. In addition, contralateral and ipsilateral acoustic reflex adaptation of four subjects with varying degrees of sensorineural hearing loss were compared to the normative data. Results for these impaired ears indicated that a longer period of activatio

High-frequency sensitization of the acoustic reflex was studied in 10 normal-hearing subjects. The procedure involved pairing a subreflex 6000 Hz sensitizing tone with activating frequencies of 500, 1000, and 2000 Hz. Both threshold and growth of amplitude of the acoustic reflex were examined. Acoustic reflex threshold was reduced by an average of 10 to 12 dB in the presence of a 6000 Hz sensitizing tone, an amount significantly less than previously reported in similar investigations. Variations in the method used to adjust the presentation level of the high-frequency sensitizing tone probably

The anatomic, neurologic, and physiologic characteristics of the middle ear structures have created confusion concerning the nature of the intraaural muscle reflex. After reviewing the relevant literature, we correlated this information with our studies of human temporal bones and computer analyses of the acoustic reflex responses of individuals with normal hearing. Our hypothesis is that although the stapedius is the initiator of the reflex and the primary contributor to ossicular chain fixation, the tensor tympani is responsible for the major observed response. The negative deflection presen

Acoustic reflex thresholds elicited using clinical acoustic impedance techniques were obtained on normal infants between one month and one year of age. Reflex activating signals of 500, 1000, 2000 Hz, low-pass, high-pass, and white noise were used. Less intensity was required to elicit the acoustic reflex to the noise than to the pure tones, with the 500-Hz tone requiring the greatest intensity. Inference of auditory thresholds from the acoustic reflex indicated the presence of normal hearing in the infant group.

The acoustic stapedial reflex, which normally occurs at a hearing level of about 80 dB, can be detected at lower levels by means of preactivation. Two techniques are presented, which allow the detection of reflex activity at intensities from 30 to 50 dB HL. It was the authors' intention to find a way to determine the hearing threshold from the stapedius reflex threshold even in patients, in whom the hearing threshold was not horizontal.

This study examined stapedial reflex recordings of pulsed pure tones (500, 1,000, 2,000, and 4,000 Hz) in 26 subjects with normal hearing and 18 with sensorineural impairment. The stapedial reflex tracings clearly differentiated the two groups of subjects. Like many of our special audiometric procedures, this test was least effective in those with mild hearing loss. The test also appeared to be useful in identifying high-frequency hearing losses. Although this reflex measurement appears to have potential as a clinical tool, additional field study appears necessary.

The probe tone of an impedance bridge has a direct effect on the results of tympanometry. The frequency of the probe tone should be close to the resonant frequency of the middle ear. Of three electroacoustic bridges using three different probe tones 800, 625 and 220 Hz, the one using 800 Hz provided the most identifiable tympanographic pattern in cases with ossicular discontinuity. Five different types of tympanograms could be identified when an 800 Hz tone is used. Important clinical information can be obtained by combining the type of tympanogram with the results of stapedius reflex measurem

The presence of inverted acoustic reflex (IAR) was observed in patients with surgically-confirmed otosclerosis. The characteristics of intensity, stimuli, and thresholds were analyzed. IAR was found in 90% of the patients stimulated on the same side at 110 dB HL with 500, 1000, and 2000-Hz frequencies. The intensity of IAR was higher at 1000-Hz and in patients with mixed hearing loss. We conclude that the detection of IAR in patients with conductive hearing loss and normal tympanic membrane allows otosclerosis to be differentiated from other pathologies that produce ossicular chain fixation. Conduction of a broad-band signal from a supra-aural earphone to an acoustic-immittance instrument coupled to the contralateral ear canal was studied using three commercial immittance instruments. Results indicated that portions of the broad-band signal reached the contralateral ear canal at presentation levels of 100-110 dB SPL. At levels as low as 100 dB SPL, the signal was detected by at least one of the acoustic-immittance instruments. At higher sound pressure levels the transmitted signal caused deflections of the acoustic-immittance meter similar to those associated with an acoustic-refl
