Resuscitation thoracotomy is a rarely performed procedure whose use, in France, remains marginal. It has five specific goals that correspond point-by-point to the causes of traumatic cardiac arrest: decompression of pericardial tamponade, control of cardiac hemorrhage, performance of internal cardiac massage, cross-clamping of the descending thoracic aorta, and control of lung injuries and other intra-thoracic hemorrhage. This approach is part of an overall Damage Control strategy, with a targeted operating time of less than 60minutes. It is indicated for patients with cardiac arrest after pen…

The geriatric population makes up a large portion of the emergency patient population. Geriatric patients have less reserve and more comorbid diseases. They are frequently on multiple medications and are more likely to require aggressive treatment during acute illness. Although it may not be obvious, it is important to recognize the signs of shock as early as possible. Special care and monitoring should be used when resuscitating the elderly. The use of bedside ultrasound and monitoring for coagulopathies are discussed. Clinicians should be constantly vigilant and reassess throughout diagnosis

PURPOSE OF REVIEW: The new International Liaison Committee on Resuscitation documents, published in 2005, include important changes in pediatric resuscitation. Some issues that were left pending have since been supplemented with new studies. Their impact will be discussed here.
RECENT FINDINGS: Studies on oxygen use for neonatal resuscitation have consistently found room air to be superior to 100% oxygen. Prospective studies indicate that intubation by first responders in preclinical resuscitation of children is dangerous and should probably be avoided. New studies point to a better neurologi…

This investigation is based on 140 autopsy protocols of unsuccessful resuscitation procedures (Resuscitation Center of the Army Hospital, Hamburg); injections and closed-chest cardiac massage had been performed in every case. - Most of the patients had collapsed because of cardiac shock; patients with thoracic or abdominal injury were excluded from this study. In individual cases only, the following severe complications originated from the resuscitation procedures: fracture of a chest vertebra, serial fractures of ribs resulting in an unstable thorax, bilateral haemothorax, tension pneumothora…

These European Resuscitation Council (ERC) Guidelines 2025 on Special Circumstances in Resuscitation are based on the 2025 Consensus on Science with Treatment Recommendations (CoSTR) of the International Liaison Committee on Resuscitation (ILCOR), reviews by the expert writing group and relevant peer-reviewed literature. The guideline chapter provides guidance for laypeople and healthcare professionals on the modifications required to basic and advanced life support in adults for the prevention and treatment of cardiac arrest for in-hospital and out-of-hospital cardiac arrest. The ERC Guidelin…

BACKGROUND: After hemorrhage control, fluid resuscitation is the most important intervention for hemorrhage. Even skilled providers can find resuscitation challenging to manage, especially when multiple patients require care. In the future, attention-demanding medical tasks like fluid resuscitation for hemorrhage patients may be reassigned to autonomous medical systems when availability of skilled human providers is limited, such as in austere military settings and mass casualty incidents. Central to this endeavor is the development and optimization of control architectures for physiological c…

OBJECTIVE: This study aimed to investigate the effect of resuscitation guideline terminology on pediatric cardiopulmonary resuscitation (CPR) performance.
METHODS: This was a prospective randomised crossover simulation trial. A total of 32 medical doctors conducted 2-min single-rescuer CPR using the one-handed chest compression technique (OHCC) or two-handed chest compression technique (THCC) on a pediatric manikin. The participants conducted chest compressions according to the chest compression depth (CCD) target of '5 cm (Test 1)' or 'at least one third of the anterior-posterior dimension o…

PURPOSE: The dose and timing of early fluid resuscitation in sepsis remains a debated topic. The objective of this study is to evaluate the effect of fluid timing in early sepsis management on mortality and other clinical outcomes.
METHODS: Single-center, retrospective cohort study of emergency-department-treated adults (>18 years, n = 1032) presenting with severe sepsis or septic shock. Logistic regression evaluating the impact of 30 mL/kg crystalloids timing and mortality-versus-time plot controlling for mortality in emergency department sepsis score, lactate, antibiotic timing, obesity, se…

The 2025 Korean Guidelines for Cardiopulmonary Resuscitation provide updated, evidence-based recommendations to optimize the transition from intrauterine to extrauterine life. While approximately 85% of term newborns initiate spontaneous breathing independently, timely intervention is critical for those requiring assistance. Key updates in the 2025 guidelines emphasize umbilical cord management, recommending deferred cord clamping for at least 60 seconds in vigorous term and preterm infants (<37 weeks). When deferred cord clamping cannot be performed, intact umbilical cord milking may be reaso…

In children, severe emergencies and cardiorespiratory arrests in particular are relatively rare but time-critical events. As compared to adults, hypoxic arrests caused by respiratory disorders that may subsequently result in pulseless electrical activity or asystole are more prevalent. The current Paediatric Life Support (PLS) Guidelines 2015 of the European Resuscitation Council (ERC) acknowledge both limited scientific evidence and aspects of practicability. They also take into account the rather limitedpaediatric routine that most providers have as well as national and local infrastructural…

The use of 100% oxygen for delivery room resuscitation is currently the recommended standard of the American Academy of Pediatrics and the Neonatal Resuscitation Program. However, there is mounting evidence from animal and human studies suggesting that resuscitation with room air (RA, 21% oxygen), including positive pressure ventilation with bag and face mask, may be as effective as that with 100% oxygen, and that the use of 100% oxygen may pose a risk of adverse physiologic sequelae. Resuscitation with RA has been demonstrated to result in faster recovery and improved neonatal mortality in co…

BACKGROUND: More than 640,000 combined in-hospital and out-of-hospital cardiac arrests occur annually in the United States. However, surviva … imultaneous external defibrillation, stellate ganglion blockade, and extracorporeal cardiopulmonary resuscitation. We discuss the underlying physiology of each therapy, review relevant literature, describe when these approaches should be considered, and provide evidence-based recommendations for these techniques.

George Washington Crile was a successful surgeon who lived at the end of the 19th century. He was born on 11 November 1864 on a farm near Chili, Ohio. He became interested in the study of shock after a close friend died from hemorrhage. Crile dedicated his research years to the study of shock, cardiac arrest, and the use of adrenaline. His research on shock and cardiac arrest led to treatment guidelines that are still used today. He also participated in the Spanish-American War and in World War I as a Navy Surgeon and saved the lives of many soldiers with his principles of blood transfusion an

A study was undertaken to determine the pattern of arterial blood gas (ABG) concentration in the canine model undergoing prolonged cardiopulmonary resuscitation (CPR) from fibrillatory arrest, and to determine the importance of acid base abnormalities in predicting resuscitation. Ventricular fibrillation was induced electrically in 12 dogs. CPR was begun at 3 minutes and continued for 27 minutes, at which time the dogs were defibrillated. ABG samples were taken at 0, 8, 18, and 28 minutes of ventricular fibrillation. Seven of the 12 dogs were resuscitated successfully. There was no difference …

BACKGROUND: Resuscitation of paediatric cardiac and respiratory arrest is a high-stakes and low frequency event in the paediatric emergency department. Resuscitation team performance assessment tools have been developed and validated for use in the simulation environment, but no tool currently exists to evaluate clinical performance in non-simulated, live paediatric resuscitations.
METHODS: This is a validation study assessing inter-rater reliability of a novel assessment tool of clinical performance of non-simulated resuscitations, the Team Resuscitation for Paediatrics tool. Videos of medic…
