BACKGROUND: The critical time from onset of complete occlusion of the central retinal artery (CRA) to functionally significant inner retinal infarction represents a window of opportunity for treatment and also has medical-legal implications, particularly when central retinal artery occlusion (CRAO) complicates therapeutic interventions. Transient central retinal artery occlusion (CRAO) was produced in 63 eyes of rhesus monkeys by lateral orbitotomy and temporary clamping of the central retinal artery (CRA) for between 15 and 270 minutes. An unusual case of central retinal artery occlusion (CRAO) occurring in the setting of a chronic retinal detachment was reported. A 47-year old woman with a history of slowly declining peripheral vision in her left eye presented with acute painless hand motion vision. She had an extensive chronic retinal detachment in the left eye with multiple demarcation lines and marked whitening of the fovea. Her right eye was normal. A CRAO was confirmed with fluorescein angiography and optical coherence tomography testing. An exhaustive work-up was negative and there was no history of antecedent trauma.

Pre-retinal arterial loops are usually incidental findings during examinations in healthy individuals. They are rarely associated with arterial occlusion, with only nine previously reported cases in the literature. The authors report a case of a pre-retinal arterial loop associated with a branch retinal artery occlusion in a myopic young man. They also report the first documented optical coherence tomography scan through the loop causing the branch retinal artery occlusion.

Retinal artery and vein occlusions can both lead to significant vision loss. Vein occlusions are much more common, while visual acuity is usually more severely affected in artery occlusions. A healthy 6-year-old boy with a clinical picture of central retinal artery occlusion (CRAO) of his left eye is presented here. The underlying cause was a trauma to the boy's left cheek some 6 months earlier. Possible pathophysiological mechanisms are discussed. To the best of our knowledge, the late onset of traumatic CRAO has not yet been reported in the literature.
