A 42-year-old woman presented with a clinically obvious giant rhinophyma. The protocol of the unit handling the case mandated that all lesions amenable to biopsy should have a core biopsy before any definitive surgery, but the unnecessary biopsy was not representative and suggested an incorrect diagnosis of perifollicular fibroma. The lesion was excised. The sections showed dilated hair follicle pores on the skin surface, squamous lined hair follicles plugged with keratin, prominent sebaceous glands, perifollicular inflammation without granulomas, intradermal budding of hair follicle basal cel…

Rhinophyma is a chronic skin condition that predominantly affects the nose, characterized by thickening and fibrosis of the skin, leading to a bulbous, enlarged appearance. It is typically associated with rosacea, though its exact etiology remains unclear. Our department recently managed two cases of rhinophyma, which allowed us to explore the clinical presentation, diagnostic challenges, and treatment strategies. This report summarizes the key aspects of these cases and offers insights into the management of rhinophyma.

Rhinophyma is an infiltrative-productive stage of rosacea characterized by the formation of inflammatory nodes, infiltrates, and tumour-like outgrowths due to progressive hyperplasia of the connective tissue and sebaceous glands as well as persistent vasodilatation as a result of a chronic progredient disease. The authors describe a man with the severe form of rhinophyma in whom this condition was treated by the application of a cold plasma. A good cosmetic result with minimal scarring was achieved by this method.

BACKGROUND: Rhinophyma is an end stage of acne rosacea. It results in a large nose due to a proliferation of sebaceous glands and fibrous tissue. Many cases of rhinophyma have been reported in the Western world; however, in Japan, rhinophyma has been an uncommon disease.
METHODS: We present two patients associated with rhinophyma who were treated by cross-shaped full-thickness excision followed by direct closure and compare the rhinophyma in Japan with that in the West.
RESULTS: To date, only 20 rhinophyma cases including our 2 cases have been reported in Japan. Epidemiologic factors of rhin…

Treatment of rhinophyma is successful only with surgical methods. The operative techniques described in the literature are discussed critically and decortication is the method of choice. When this technique is applied, the hyperplastic nasal tissue is removed down to the basal parts of the sebaceous glands. This causes spontaneous re-epithelization of the skin surface without scarring. Methods using deeper excision and plastic surgical repair of the defect by skin grafting or pedicled flaps are cosmetically inferior to the method of decortication. Whatever the method, however, histopathologica…
