Non-traumatic cerebrospinal fluid rhinorrhea is uncommon. Review of the literature revealed that 45% of cases are high pressure leaks and 55% of cases are normal pressure leaks. Meningoencephaloceles are very rare occurrences in the latter category and we present the third reported case occurring in the frontal sinus. Radiographically this lesion appeared as a mucocele because of its expansile nature, however non-traumatic mucoceles have not been associated with cerebrospinal fluid rhinorrhea. The only non-osseous expansile lesion of the paranasal sinuses found to be associated with this compl…

Cerebrospinal fluid rhinorrhea is an uncommone occurrence, especially when a history of previous trauma is absent. This report describes a case of cerebrospinal fluid rhinorrhea occurring in a pregnant patient during the pushing portion of her Lamaze exercises, and discusses the management of labor and delivery.

BACKGROUND: Clear anterior rhinorrhea is commonly observed in older adults and is widely known as "senile rhinorrhea." Although appreciated in the literature, no studies support that clear rhinorrhea increases with age. Our goal was to determine the prevalence of clear rhinorrhea in adults and the variation between young and old. Additionally, we sought to determine if clear rhinorrhea was bothersome enough for patients to seek treatment.
METHODS: A questionnaire was randomly distributed to 158 adults, 76 young adults on the University of Pittsburgh Undergraduate campus, and 82 older adults a…

A woman with multiple illnesses including allergic rhinitis presented for a follow-up visit at our clinic with constant rhinorrhea for 2 weeks despite regular use of nasal corticosteroids. Two weeks earlier, after alcohol drinking and doubling some of her medications for missed doses, she fell on her face. The Emergency Department records documented headache, bradycardia, hypotension, dehydration, and right infraorbital swelling. She was admitted for hydration and observation, and was discharged after two days without radiologic evaluation of the head. At our clinic, physical examination revea…

Cerebrospinal fluid (CSF) rhinorrhea occurs due to communication between the intracranial subarachnoid space and the sinonasal mucosa. It could be due to trauma, raised intracranial pressure (ICP), tumors, erosive diseases, and congenital skull defects. Some leaks could be spontaneous without any specific etiology. The potential leak sites include the cribriform plate, ethmoid, sphenoid, and frontal sinus. Glucose estimation, although non-specific, is the most popular and readily available method of diagnosis. Glucose concentration of > 30 mg/dl without any blood contamination strongly suggest…

The skull base is an intricate interface between the cranium and face allowing the passageway of vital structures. Anatomic conceptualization forms the platform for surgical approaches and solutions to skull base pathology. Understanding embryogenesis provides further depth into the pathophysiology of congenital defects. This chapter aims to highlight skull base anatomy and embryology along with cerebrospinal fluid physiology and diagnostic evaluation of cerebrospinal fluid leaks.

OBJECTIVE: To investigate the best treatment of cerebrospinal fluid rhinorrhea.
METHOD: A retrospective study of 45 consecutive patients who presented with CSF leak, was conducted. Forty-two patients were treated with transnasal endoscopic approach, three with double approach transnasal and extranasal. The fascia lata, the myoplasm, the free middle turbinate graft and the free inferior turbinate graft were used for fistula repairing. Four repair techniques as onlay technique, inlay technique, underlay technique and obliteration technique were used.
RESULT: Postoperative follow-up lasted from …

Cerebrospinal fluid rhinorrhea and meningoencephaloceles extending into the nasal cavity from the anterior, middle, and posterior cranial fossae often are managed by otolaryngologists. A thorough understanding of the underlying pathophysiology, management principles, and treatment options is essential to achieve optimal outcomes. Surgical and medical management is highly individualized and depends on many factors, including etiology, anatomic site, and underlying intracranial pressure. This article highlights the history, physiology, pathophysiology, diagnosis, surgical techniques, and postope…

PURPOSE: Most anterior skull base defects causing cerebrospinal fluid (CSF) rhinorrhea can be readily approached using endoscopic techniques when surgical repair is necessary. We present our data from endoscopic repair of CSF rhinorrhea with long-term follow-up.
METHODS: Retrospective data analysis of patients that were diagnosed with anterior skull base CSF rhinorrhea and underwent endoscopic repair at a tertiary institution. Data were analyzed to determine the etiology and location of CSF leaks. Diagnostic techniques, surgical techniques, and surgical outcomes were reviewed.
RESULTS: Ninet…

The diagnosis of cerebrospinal fluid rhinorrhea is usually straightforward. It need not be. The location of the leak is usually clear. It need not be. With modern rigid endoscopes, the repair is usually technically straightforward. It need not be. When a cerebrospinal fluid leak is suggested and the fluid is collectable, testing for beta 2 transferrin will usually settle the issue. The site of the leak is usually obvious. If it is not, but the leak is active, an isoview computed tomography scan will reveal it. Once the leak is established and located, the experienced endoscopic sinus surgeon c…

INTRODUCTION: Nasal liquorrhea indicates a cerebrospinal fluid fistula, an open communication between the intracranial cerebrospinal fluid and the nasal cavity. It can be traumatic and spontaneous.
OBJECTIVE: The aim of this study was to assess the outcome of endoscopic repair of cerebrospinal fluid fistula using fluorescein.
METHODS: This retrospective study included 30 patients of both sexes, with a mean age of 48.7 years, treated in the period from 2007 to 2015. All patients underwent lumbar administration of 5% sodium fluorescein solution preoperatively. Fistula was closed using three-la

The evolutionary concepts of pinpointing and treating cerebrospinal fluid (CSF) fistulas over the past few decades are reviewed taking into consideration the dynamic character of CSF circulation. The localization of the site of leakage is greatly facilitated by the judicious use of a variety of radioisotope and radiological techniques. The most recent are Metrizamide CT Cisternography and Magnetic Resonance Imaging. It is asserted that still the most challenging cases are those with intermittent liquorrheas, and those patients in which the site of the fistula is not easily defined. The authors

A spinal fluid leak from the intracranial space to the nasal respiratory tract is potentially very serious because of the risk of an ascending infection which could produce fulminant meningitis. Cerebrospinal rhinorrhea typically stems from a complication of head trauma, and fortunately these leaks tend to heal spontaneously. In a smaller percentage of cases, intracranial lesions or congenital osteomeningeal defects are harbored, allowing spinal fluid to enter the nasal cavity, and patients with this condition rarely heal without operative intervention. Succesful repair of a dural defect manda…

CONCLUSIONS: The overall risk of meningitis in patients with persistent CSF rhinorrhea was 19%. The annual incidence of meningitis was 0.3 episodes per year, with most episodes occurring within the first year following the onset of the leak.
