We have measured plasma volume expansion (Evans blue and hematocrit changes) and hemodynamic responses in conscious hemorrhaged and normovolemic splenectomized sheep after a 30-min infusion of either 20 ml/kg of diaspirin cross-linked hemoglobin (DCLHb), 20 ml/kg of human albumin (Alb), or 60 ml/kg of a solution of Ringer lactate (RL). All regimens expanded blood volume and increased blood pressure and cardiac output after hemorrhage. OBJECTIVE: Whether intraoperative fluid infusion should contain glucose during pediatric cardiac surgery remains controversial. This study was performed to compare the effects of glucose and glucose-free solutions on blood glucose and blood insulin levels during total repair of congenital heart diseases.
DESIGN: Prospective randomized and blinded study.
SETTING: Cardiovascular university center.
PARTICIPANTS: Forty nondiabetic children, weight ranging from 4 to 10 kg, scheduled for cardiac surgical procedures requiring cardiopulmonary bypass (CPB) without total circulatory arrest.

Two regimens of fluid and electrolyte therapy were studied in children with severe cholera and noncholera diarrhoea. BACKGROUND: The Surviving Sepsis Campaign suggested preferential resuscitation with balanced crystalloids, such as Lactated Ringer's (LR), although the level of recommendation was weak, and the quality of evidence was low. OBJECTIVE: The effectiveness of the intraosseous (IO) route for fluid resuscitation remains uncertain. This study compares IO infusion rates and estimated volume expansion using clinically relevant infusion pressures with lactated Ringer's (LR) and hetastarch (HES).
METHODS: IO needles were placed in the sternum or tibia for infusion of HES and LR in 8 Yorkshire pigs. Pressures were recorded at variable flow rates and linear regression used to identify flow rates at 100 and 400 mm Hg. Volume expansion was calculated for a 10-minute infusion.

The effects of dextran 1.8% in Ringer's lactate solution were compared with infusion of Ringer's lactate alone in two groups of 12 patients after minor surgical interventions. Plasma volume increased significantly only in the dextran treatment patients. We observed a marked hemodilution with both infusions, but all changes had disappeared after 24 hours. No adverse effects on the coagulation process, hepatic and renal function were noted. These results suggest that dextran 1.8% in Ringer's lactate is an effective and safe solution in plasma volume expansion for this type of patient.
