Risk adjustment for evaluating the outcome of urological operative procedures.

PURPOSE: Considerable public and media attention has been directed in recent years toward comparing performance at individual hospitals. So-called death league tables have been published in the media, ranking hospitals according to crude mortality rates. Crude rates of mortality and morbidity are clearly misleading. Therefore, scoring systems comparing treatment outcomes among physicians or hospitals on an objective basis are urgently required. MATERIALS AND METHODS: During a 12-month period we prospectively evaluated 651 patients at 2 urological units using a simple and well validated surgic

[Adjustment disorders with developmental risk in children after inpatient treatment].

The relatively broad spectrum of symptoms in child psychiatry (disturbances of behaviour and of anxiety and others) was examined in order to find smaller diagnostic groups. Two selected groups of probands of the Department of Child and Adolescent Psychiatry at the University Hospital for Children in Jena were analysed. This analysis was made by recording journals of treatment and a questionnaire. Both groups differed significantly in 33 out of 128 characteristics. The following factor analysis resulted in 10 anxiety- and non-anxiety-syndromes leading back to basic disturbances of affect, psych

Is adoption a risk factor for the development of adjustment problems?

The extent to which being adopted increases a child's risk for the development of adjustment problems has been debated for decades. Results from studies examining prevalence of adopted children and adolescents in outpatient and inpatient mental health treatment suggest that the risk associated with adoption is modest or nonexistent. This body of research is reviewed and critiqued. Two possible explanations for the apparent disparate findings of the clinically based and nonclinically based studies are explored: biases in referral for mental health treatment and the influence of the shape of the

Risk factors for suicidal behavior in patients hospitalized for adjustment disorders.

INTRODUCTION: Adjustment disorder (AD) is a common psychiatric diagnosis, but it is often considered less severe than other diagnoses. However, it is strongly associated with suicidal behavior. AIM: To identify the factors linked to suicidal behavior in patients hospitalized for AD. METHODS: This is a retrospective, descriptive, and analytical study conducted in the psychiatric department of Mahdia's hospital over a period of nine years. The study included patients who were hospitalized for the first time due to adjustment disorder, according to DSM-5 diagnostic criteria. RESULTS: The study

Social Risk Adjustment and Bonus Eligibility in Medicare Advantage Star Ratings.

IMPORTANCE: The Medicare Advantage (MA) star ratings program determines eligibility for quality bonus payments that directly affect plan revenue, benefit generosity, and market competitiveness. To improve fairness, the Centers for Medicare & Medicaid Services (CMS) introduced the Categorical Adjustment Index (CAI) in 2017, which adjusts ratings based on enrollee social risk factors. How these adjustments have affected bonus eligibility for MA contracts over time remains unclear. OBJECTIVE: To assess the association between CAI adjustments and bonus eligibility in the MA star ratings program a

Competing risk adjustment reduces overestimation of opportunistic infection rates in AIDS.

To illustrate the importance of adjusting the estimates of cumulative incidence of acquired immunodeficiency syndrome (AIDS) related illnesses for competing risk of other causes of death, we compared unadjusted and adjusted (for competing events) incidence estimates for four AIDS illnesses: pneumocystis cavinii pneumonia (PCP), mycobacterium avium complex (MAC), cytomegalovirus (CMV), and esophageal candidiases. The study population was patients followed by the Johns Hopkins Hospital AIDS Service between 1989 to 1995. Ratios of 4 year unadjusted incidence estimates to 4 year adjusted incidence

Advancing Surgical Quality: Performance Measures and Risk Adjustment in Modern Thoracic Surgery.

Thoracic surgery has transitioned from simple mortality tracking to more complex, risk-adjusted performance metrics. This article reviews this evolution, applying a structure-process-outcome framework to detail current performance indicators in the field. It then discusses how large registries and advanced statistical methods facilitate fair benchmarking and point-of-care decision support, while also addressing limitations, including incomplete data and the absence of social risk considerations. It concludes by exploring future directions, including frailty adjustment, equity-focused modeling,

Confounder adjustment in observational studies investigating multiple risk factors: a methodological study.

BACKGROUND: Confounder adjustment is critical for accurate causal inference in observational studies. However, the appropriateness of methods for confounder adjustment in studies investigating multiple risk factors, where the factors are not simply mutually confounded, is often overlooked. This study aims to summarise the methods for confounder adjustment and the related issues in studies investigating multiple risk factors. METHODS: A methodological study was performed. We searched PubMed from January 2018 to March 2023 to identify cohort and case-control studies investigating multiple risk
