CONTEXT: Despite the strong interest in health care quality, little is known about the quality of preventive care among women in rural primary care settings.
PURPOSE: We sought to assess the quality of screening practices in Rural Health Clinics (RHCs) as measured by the rates at which female patients received screening within national guidelines.
METHODS: A cross-sectional, retrospective chart review of 480 charts of female patients in 12 randomly selected RHCs was conducted. Data were collected on screening activities documented in >3,800 patient visits. Chart data was extracted by trained…

Rural hospitals face the issue of survival on a daily basis. Because of their small economic base and more complex service area, rural hospitals will require external assistance. The impact of government regulations on the formation of rural hospital consortia is scrutinized, particularly the incentives arising through Medicaid, certificate of need, and physician licensure requirements that spur hospitals to join consortia. These organizations of individual hospitals appear to be a feasible option for ensuring rural hospital survival.

BACKGROUND: We describe reproductive health issues among pregnant women in a rural area of Kenya with a high coverage of insecticide treated nets (ITNs) and high prevalence of HIV (15%).
METHODS: We conducted a community-based cross-sectional survey among rural pregnant women in western Kenya. A medical, obstetric and reproductive history was obtained. Blood was obtained for a malaria smear and haemoglobin level, and stool was examined for geohelminths. Height and weight were measured.
RESULTS: Of 673 participants, 87% were multigravidae and 50% were in their third trimester; 41% had started

China's rural health care system has undergone major changes since the early 1980s, when the country began privatising rural health services. Following fiscal devolution, the rural primary health service was transformed into a fee-for-service system, dependent on the availability of local resources. This article reports some of the results of a study undertaken in 1994-96 to examine the impact of privatization on financing, provision and use of reproductive health services by women in two rural counties in Yunnan Province, China. The most common self-reported symptoms of reproductive morbidity…

At a rural community health center, 183 adult patients were invited to participate in an unspecified study. In a closed-door session, interested invitees were told the study required taking the Test of Functional Health Literacy in Adults (TOFHLA). After the purpose of the study was privately disclosed, 70 patients agreed to enroll (38.25 percent acceptance rate). Approximately 15 percent demonstrated literacy/numeracy deficits, scoring in the Inadequate and Marginal Functional Health Literacy ranges. Student's t-test comparison of mean scores suggested patients with literacy/numeracy deficits

OBJECTIVE: The aim of the study is to ascertain which neurosurgical procedures can be safely and effectively performed in an isolated rural neurosurgical unit, despite the lack of advanced technological infrastructure available in tertiary neurosurgical departments in high-income countries.
METHODS: The authors draw upon their experience of establishing a first-line neurosurgical unit in a remote Pacific archipelago, which was accomplished without significant technological investments. All the patients operated in the neurosurgical unit of the Territorial Hospital of Nouméa, New Caledonia, fr

Digital health is increasingly promoted to address rural health and care workforce shortages. However, its workforce consequences are usually evaluated through implementation or service-delivery outcomes rather than the conditions that enable rural workers to remain, develop, and practice sustainably. We conducted a Joanna Briggs Institute-informed scoping review reported according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews. The protocol was prospectively registered on the Open Science Framework. Five databases were searched for studies …

INTRODUCTION: In decentralized public health systems, rural public health departments have fewer economic resources and higher needs for public health programs and services. This case study describes the evolution of a rural district health department's community and academic partnerships across three phases. Throughout this process, partnerships grounded in community engagement informed the local public health workforce, translated research into practice, and supported the design, implementation, and evaluation of interventions in a rural setting.
METHODS: Our approach is guided by three est

Rural Health Leaders Pipeline programs are intended to increase the number of youth interested in and pursuing health professions in rural communities. This paper presents 2 complementary approaches to Rural Health Leaders Pipeline programs. Two different organizations in Alabama recruit students from 18 specified counties. One organization is a rural, community-based program with college freshmen and upperclassmen from rural communities. Students shadow health professionals for 6 weeks, attend classes, visit medical schools, complete and present health projects, and receive support from onlin…

In utero exposure to income shocks has a lasting effect on child well-being. In an agricultural economy, fluctuations in rainfall directly affect household income. In this paper, we investigate the short- and long-run impact of pre-pregnancy, prenatal, and early-life exposure to fluctuations in rainfall on height for a sample of 2290 children in rural Pakistan. Given the widespread canal irrigation system prevalent in the country, we also investigate how fluctuations in river water flows affect child health. We find that fluctuations in rainfall during the pre-pregnancy period have the most la

Purpose Little is known about the health status of prisoners in low-income countries. In Haiti, prisons typically lack adequate medical care, clean water and food, though some prisoners receive additional food from visitors. The purpose of this paper is to characterize the physical and mental health of Haitian prisoners in three select prisons and examine the effects of having visitors and length of detention on health status. The authors hypothesized that prisoners with more visitors and shorter detention times would have better overall health status. Design/methodology/approach The authors c

Background/Objective: The quality of care afforded to rural, remote, and First Nations Peoples is dependent on access to a health workforce with the capacity to contextualize healthcare and practice to the needs and expectations of these populations. In Australia, the lack of representation of rural health in undergraduate and post graduate health professional education undermines this preparedness and consideration of rural practice uptake and longevity, compounding the inequities confronted by 7 million Australians residing in these locations. Urgent educational reforms are required to addre…

A health survey to describe the morbidity pattern of a rural population in Nigeria was conducted among a 10% random sample of 1091 households in the Kainji Lake area of Nigeria. The diseases most frequently encountered were fevers, gastro-enteritis and chest infections, with prevalence rates of 50.2%, 37.1% and 10.0% respectively. Dental debris, conjunctival pallor, missing teeth and dental calculi were present in 65.8%, 59.1%, 32.2% and 24.1% respectively. The adult males were found to be significantly taller and heavier and with higher blood pressure than the females. Most of the health prob
