Although the commonest manifestation of Salmonella infection is acute gastroenteritis, infection may spread to the blood-stream may and the illness can present with focal lesions in almost any organ with or without septicemia. We describe here a case of Salmonella typhi infection of a sacroiliac joint that was cured with ciprofloxacin therapy for six weeks. The patient was immunologically normal. From November 1988 to June 1989, 142 strains of S. typhi were isolated from patients of typhoid fever in Qingpu County, Shanghai These strains were highly sensitive to ceftazidime, ceftriaxone, ofloxacin and enoxacin, the sensitive rates to chloramphenicol, co-trimoxazole and ampicillin being from 67% to 83%. Of these 142 strains, twenty-two were multiresistant strains, being resistant to chloramphenicol, co-trimoxazole, ampicillin, cefazolin, gentamicin, piperacillin and tetracycline. Bacteriuria due to Salmonella typhi usually occurs following recent typhoid fever or in chronic carrier states. Data from 18 patients with S. typhi bacteriuria, seen during 5 years, were analyzed. Fourteen patients had localized urinary tract infection due to S. typhi. Four others had bacteriuria, probably associated with typhoid fever. The quinolones possess a high degree of in vitro activity against enteric bacteria, including Salmonella. This observation, coupled with the limitations of current antityphoid agents, has resulted in the evaluation of quinolones in the therapy of S. typhi infection, including both enteric fever and chronic intestinal carriage.
