A review of recent studies of undesirable effects of gold salts is presented. Marrow aplasia is potentially the most serious side effect with a poor prognosis. Its prevalence is undetermined but may have been overestimated. Gold nephropathy and thrombocytopenia although more common usually have a good prognosis. Both can be satisfactorily treated without prolonged morbidity or mortality. We report a 44 year old male suffering from rheumatoid arthritis that developed myokymias when treated with gold salts. Myokymia is a clinical phenomenon characterized by involuntary repetitive contractions of muscle fibres, which give an undulating appearance to the overlying skin, last a few seconds, and with a typical electromyographic activity described as myokymic discharges. OBJECTIVE: Evaluate the clinical features of skin reactions to gold salts.
INTRODUCTION: Gold dermatitis is described in the literature as a group of reactions non-specifically associated with these drugs.
PATIENTS AND METHODS: Ten patients (7 males, 3 females, mean age 59.6 years [corrected] with a skin reaction were studied over a 6 year period. All were treated with Allochrysine for rheumatoid arthritis (n = 8), rhizomelic pseudopolyarthritis (n = 1) or arthropathic psoriasis (n = 1).

4 patients with chronic photocontact dermatitis were sensitive to cobalt salts. They presented as cases of contact dermatitis from cement or pig fodder with persistent lesions on exposed areas. Only 2 of them had standard patch test positive reactions to chromate and cobalt, but all showed positive photo-patch tests to cobalt. Photo-oxidation tests proved that cobalt salts are photosensitizing. Exposure to cobalt salts was responsible for the patients' chronic actinic dermatitis.

We describe a new type of nitrenium-based Lewis acids: tetraaryl-1,2,3-triazolium salts. These were fully characterized by NMR and X-ray crystallography. The Gutmann-Beckett acidity numbers were determined to be up to 35.6, which is high compared to those of previously studied nitrenium salts. These salts catalyze the facile hydrosilylation-deoxygenation of ketones, aldehydes, acetals, alcohols, ethers, and silyl ethers under mild conditions in excellent yields. To our knowledge, this represents a first example of triazolium ions used as Lewis acid catalysts.
