Nocardia scleritis.

PURPOSE: To describe a case of Nocardia scleritis, an unusual ocular infection. METHODS: Case report and review of pertinent literature. RESULTS: An 83-year-old man with leukocytoclastic vasculitis was initially examined for infectious necrotizing scleritis after explantation of an extruded scleral buckle. The patient was successfully treated with sulfonamides. CONCLUSIONS: Nocardia asteroides may cause infectious scleritis in the absence of cataract surgery or trauma. Treatment with sulfonamides can result in a satisfactory outcome.

Toxoplasmic scleritis.

Although toxoplasmosis is the most common infectious cause of posterior intraocular inflammation, it is rarely described in association with scleritis. The authors present five cases of toxoplasmosis with scleritis. Two of the five cases were diagnosed clinically and serologically as having toxoplasmosis. Their retinochoroiditis and scleritis responded well to medical therapy. Retinochroiditis and scleritis that was refractory to treatment developed in the other three patients, two of whom had been receiving immunosuppressive therapy for systemic diseases. Their therapeutic regimens did not in…

Rheumatoid scleritis.

A 59-year-old black female with rheumatoid arthritis developed diffuse anterior scleritis with orbital and adnexal involvement. Computerized axial tomograhy (CAT) demonstrated the location and extent of the lesion. Biopsy of subconjunctival tissue revealed a rheumatoid nodule, characterized by granulomatous inflammation and fibrinoid necrosis. The patient improved rapidly on large doses of systemic corticosteroids. The relationship of scleritis to rheumatoid arthritis is discussed. The histopathologic features of rheumatoid scleritis, its apppearance on the CAT scan, and the management of this…

Scleritis in Iran.

PURPOSE: To present the demographic and clinical presentations of anterior scleritis among Iranian patients. METHODS: This retrospective case-series at a tertiary center in Iran, identified and analyzed anterior scleritis cases admitted from 2008 to 2018. Extracted data included demographics, clinical features, background systemic diseases, utilized therapies, and follow-up data on visual outcomes, ocular complications, and recurrence rate. Patients with incomplete records were excluded from the analysis. RESULTS: Sixty-five patients (83 eyes) with anterior scleritis were included, with a fe…

[Scleritis and episcleritis].

Scleritis and episcleritis are rare ocular inflammatory diseases but deserve to be known by internists because of their frequent association with systemic autoimmune diseases. It is important to distinguish them between because their prognosis, therapeutic management and potential complications are very different. Episcleritis represents a superficial ocular inflammation with usually benign visual prognosis, no complication with local treatment, and is associated with a systemic autoimmune disease in rare cases. In contrast, scleritis is a potentially serious ophthalmological condition that ca…

[Scleritis and episcleritis].

Episcleritis is generally a benign disease with a self-limited course, while scleritis is a severe ocular condition due to a risk of impaired vision in one-fifth of patients and its association with systemic diseases in one third of them. Infectious scleritis, representing 8 % of the etiologies, is mainly of herpetic origin (varicella zoster and herpes simplex viruses). A systemic autoimmune disease is observed in roughly 30 % of scleritis patients: inflammatory rheumatisms (15 %), firstly rheumatoid polyarthritis, systemic vasculitides (8 %), mainly granulomatosis with polyangiitis (Wegener's…

Episcleritis and scleritis.

This article will briefly discuss episcleritis and scleritis as clinical entities. Both are forms of ocular inflammation but have different clinical, therapeutic and prognostic implications. Episcleritis is a self-limited disease, causing mild discomfort and infrequently requiring therapeutic intervention. Scleritis is a painful, inflammatory process, commonly associated with systemic disorders and requires systemic anti-inflammatory therapy. It is important to be able to distinguish between the two clinical conditions, as prompt initiation of therapy for scleritis can be sight saving.

Pseudomonas-induced scleritis mimicking autoimmune necrotizing scleritis. A case report.

To report a case of spontaneous-onset unilateral scleritis and keratitis due to Pseudomonas aeruginosa in a patient with systemic lupus erythematosus who partially responded to antibiotic therapy and achieved complete resolution after IV immunotherapy. A 30-year-old woman with a past medical history of hypothyroidism and systemic lupus erythematosus and on irregular therapy presented with a long-history of thinning of the sclera and cornea refractory to antibiotic therapy despite a positive culture for P. aeruginosa, which eventually resolved with the implementation of immunotherapy. Infectiou…

Pseudomonas aeruginosa scleritis initially presenting as idiopathic diffuse anterior scleritis.

Pseudomonas aeruginosa remains the most common cause of bacterial scleritis. This report illustrates an unusual presentation of P. aeruginosa scleritis, which initially presented as diffuse anterior scleritis with anterior uveitis. The detailed laboratory work-up of the patient was negative, and the initiation of high-dose oral corticosteroid therapy led to further deterioration of clinical condition, with the appearance of a yellowish-white nodule within 3 days. The aspirate from the nodule grew P. aeruginosa, and the scleral inflammation resolved with anti-microbial therapy.

Immunopathology of scleritis.

Conjunctival and scleral biopsies from 25 patients with necrotizing scleritis and 5 patients with recurrent nonnecrotizing scleritis were studied by histopathologic, immunofluorescence, and immunoperoxidase techniques. Vasculitis with fibrinoid necrosis and neutrophil invasion of the vessel wall was present in 75% of the scleral and 52% of the conjunctival specimens. Vascular immunodeposits were found in 93% of the scleral and 79% of the conjunctival tissue tested by immunofluorescence techniques. A dramatic increase in the number of inflammatory cells over normal controls was detected in both…
