Scrub typhus is a widespread and at times serious infection in Asia. If results from central Malaysia can be applied, it appears to be economically important. Diagnosis is often difficult and treatment prone to fail if short courses of antibiotics are used. Prophylaxis is the key area of research with the development of a vaccine being the ultimate goal.

We report a case of a 60-year-old farmer presented with chief complaints of fever, myalgia, and headache of one week duration. Head-to-toe examination revealed an area of central necrosis surrounded by erythema (suggestive of eschar) on nape of the neck. A diagnosis of scrub typhus was made and the patient improved following 7-days course of doxicycline. Eschar is pathognomonic finding for clinical diagnosis of scrub typhus. This is to conclude, head-to-toe examination is indispensable for all the patients presenting with fever, myalgia, and headache.

Scrub typhus is transmitted by trombiculid mites and is endemic to East and Southeast Asia and Northern Australia. The clinical syndrome classically consists of a fever, rash, and eschar, but scrub typhus also commonly presents as an undifferentiated fever that requires laboratory confirmation of the diagnosis, usually by indirect fluorescent antibody (IFA) assay. We discuss the limitations of IFA, debate the value of other methods based on antigen detection and nucleic acid amplification, and outline recommendations for future study.

Two patients with Tsutsugamushi disease (fever) were successfully treated with tetracycline derivatives after typical eschars were found, although one of the patients was initially misdiagnosed as having a drug reaction eruption. Prompt diagnosis and treatment are important because this disease can be associated with considerable morbidity and simple effective treatment is easily available.
