Linear lucency of the skull vault: Fracture or accessory skull suture.

Sutures are a type of fibrous joint that occur only in the skull. However, partial or complete division of bones resulting in anomalous or accessory skull sutures may be misinterpreted as fractures when identified on radiological examination. Thus, it is of paramount clinical and medicolegal importance that these sutures be differentiated from actual fractures which can lead to severe consequences such as false accusation and mis-management of patient. It must be remembered though that the diagnosis in such cases is not straight forward even during postmortem examination which requires careful

Skull Base Meningiomas.

Skull base meningiomas are among the most challenging meningiomas to treat clinically due to their deep location, involvement or encasement of adjacent essential neurovascular structures (such as key arteries, cranial nerves, veins, and venous sinuses), and their often-large size prior to diagnosis. Although multimodal treatment strategies continue to evolve with advances in stereotactic and fractionated radiotherapy, surgical resection remains the mainstay of treatment for these tumors. Resection of these tumors however is challenging from a technical standpoint, and requires expertise in sev

[Basilar skull fractures].

CLINICAL ISSUE: Basilar skull fractures are fractures of the lower part of the skull. They make up about 20% of all skull fractures and are mainly caused by high-velocity blunt trauma and falls from high heights. Depending on their precise location, they can be divided into frontobasal, laterobasal and frontolateral fractures. Possible clinical signs are the presence of cerebrospinal fluid rhinorrhea or otorrhea, periorbital ecchymosis (raccoon eyes), retroauricular ecchymosis (battle sign) and cranial nerve injuries. Furthermore, fractures of the petrous bone can lead to a conductive hearing

Positional Skull Deformities.

BACKGROUND: Especially in the first 6 months of life, skull deformities manifesting as a uni- or bilateral flattening of the occiput often give rise to questions of differential diagnosis and potential treatment. In this review, the authors summarize the current understanding of risk factors for this condition, and the current state of the relevant diagnostic assessment and options for treatment. METHODS: The recommendations given in this selective review of the literature are based on current studies and on existing guidelines on the prevention of sudden infant death, the recommendations of

Skull Base Anatomy.

The anatomy of the skull base is complex with multiple neurovascular structures in a small space. Understanding all of the intricate relationships begins with understanding the anatomy of the sphenoid bone. The cavernous sinus contains the carotid artery and some of its branches; cranial nerves III, IV, VI, and V1; and transmits venous blood from multiple sources. The anterior skull base extends to the frontal sinus and is important to understand for sinus surgery and sinonasal malignancies. The clivus protects the brainstem and posterior cranial fossa. A thorough appreciation of the anatomy o

Everted skull fracture.

OBJECTIVE: Skull bone fractures are common in trauma. They are usually linear undisplaced or depressed; however, a distinct possibility of elevated fracture remains. We describe an entity of everted fracture skull in which the fracture segment is totally everted. The nature of trauma, management, and complications of this unique case are discussed. CASE REPORT: A 21-year-old woman involved in a railway accident presented to us with a primary dressing on her wound. Investigations revealed an everted fracture skull. She underwent surgery with good results. CONCLUSION: We would like to add ever

Skull-base metastases.

Metastasis to the skull-base particularly affects patients with carcinoma of the breast and prostate. Clinically, the key feature is progressive ipsilateral involvement of cranial nerves. Five syndromes have been described according to the metastatic site including the orbital, parasellar, middle-fossa, jugular foramen and occipital condyle syndromes. Magnetic resonance imaging (MRI) is nowadays the most useful examination to establish the diagnosis but plain films, CT scans with bone windows and isotope bone scans remain helpful to demonstrate bone erosion. Normal imaging studies do not exclu

Contrecoup skull fractures.

The authors describe the frequency, pathological features, and significance of contrecoup fractures of the anterior cranial fossae, which occur commonly when falls with occipital or temporal impacts cause fatal head injuries.

[Significance of intraosseous skull cap hematoma for reconstruction of skull injuries].

Diaphanoscopic postmortal examination of blunt impact injuries to the head sometimes revealed non-diaphanous regions deriving from intraossary haematomata. Precise delimitation of these haematomata in the diploe from haematopoietic foci or neoplastic lesions was possible by means of histological examination. The location of the intraossary skull-cap haematomata coincided with corresponding injuries of the scalp as contusions and lacerations; they were often accompanied by fissural bone lesions and linear fractures. As such a diploetic haemorrhage represents a facultative counterpart of scalp c

[Cloverleaf skull and other malformations of the skull. Pathology and clinical aspect].

Malformations of different parts of the cartilage results in achondrogenesis II, thanatophoric dwarfism and in metatropic dwarfism. Whether the base of the skull or the cranial-distal skeleton are affected, both together or separately, depends upon the localization of the defect within the cartilage. The different types of the cloverleaf syndrome are characterised by a defect of vessels and cartilage that affect the chondrocranium and the cranial-distal skeleton in different grades. The mal-development of the vessel-system is particularly marked in the upper back of the calotte.

Penetrating skull stab.

Fatal skull stabs are rare. In the case reported here, a 28-year-old man sustained an isolated penetrating skull injury from a knife and died two days later. The bone shard with the stab puncture, which was neurosurgically removed before death, later allowed the reproducible exact assignment of the murder knife found at the scene to the stab as well as the estimation of the length of the intracranial stab channel. This, together with the findings of the formalin-fixed brain, allowed forensic reconstruction.

Skull Base Tumors.

OBJECTIVE: This article reviews the presenting features, molecular characteristics, diagnosis, and management of selected skull base tumors, including meningiomas, vestibular schwannomas, pituitary neuroendocrine tumors, craniopharyngiomas, chordomas, ecchordosis physaliphora, chondrosarcomas, esthesioneuroblastomas, and paragangliomas. LATEST DEVELOPMENTS: Skull base tumors pose a management challenge given their complex location and, as a result, the tumors and treatment can result in significant morbidity. In most cases, surgery, radiation therapy, or both yield high rates of disease contr

Avian Skull Orthopedics.

Because the avian skull is the reflection of the wide biodiversity of birds, many anatomic, morphologic, and functional variations are encountered. The main objectives of this article are to review the surgical considerations associated with the functional anatomy of the avian jaw apparatus and its variation among species, and to describe the general medical and surgical management of head traumatic and developmental disorders in birds.

[Neonatal skull fracture].

During the latest decades the incidence of birth traumas has decreased significantly. Even so the traumas still contribute to an increased mortality and morbidity. We present a case of spontaneous neonatal skull fracture following a normal vaginal delivery. Abnormal facial structure was seen, and the fracture was identified with an MRI. The fractures healed without neurosurgical intervention. Case reports show that even in uncomplicated vaginal deliveries skull fractures can be seen and should be suspected in children with facial abnormalities.
