The development and use of a new scale, the Epworth sleepiness scale (ESS), is described. This is a simple, self-administered questionnaire which is shown to provide a measurement of the subject's general level of daytime sleepiness. One hundred and eighty adults answered the ESS, including 30 normal men and women as controls and 150 patients with a range of sleep disorders. They rated the chances that they would doze off or fall asleep when in eight different situations commonly encountered in daily life. Total ESS scores significantly distinguished normal subjects from patients in various di…

Sleepiness is a physiological function, and can be defined as increased propension to fall asleep. However, excessive sleepiness (ES) or hypersomnia refer to an abnormal increase in the probability to fall asleep, to take involuntary naps, or to have sleep atacks, when sleep is not desired. The main causes of excessive sleepiness is chronic sleep deprivation, sleep apnea syndrome, narcolepsy, movement disorders during sleep, circadian sleep disorders, use of drugs and medications, or idiopathic hypersomnia. Social, familial, work, and cognitive impairment are among the consequences of hypersom…

When it is not due to an extrinsic origin, somnolence may be the main symptom of various diseases. Among these causes of excessive daytime sleepiness, obstructive sleep apnea syndrome is noteworthy for its very important prevalence, estimated at 4% in adult males. Due to repeated upper airway obstructions during sleep, this disease is efficiently treated by continuous positive airway pressure applied through a nasal masks during sleep. Another syndrome, periodic limb movements during sleep may also lead to a sleep fragmentation at the origin of daytime sleepiness. Its treatment is principally …

OBJECTIVES: To report validation data for the Pupillographic Sleepiness Test (PST) in children and adolescents, evaluate its applicability for diagnosing excessive daytime sleepiness and its relationship to sleepiness-associated outcomes.
METHODS: A cross-sectional diagnostic test accuracy study was performed. Patients underwent three PST at 9 a.m. (T1), 11 a.m. (T2) and 1 p.m. (T3) plus the Multiple Sleep Latency Test (MSLT) on a single day. Additionally, two neurocognitive tests were performed and three questionnaires about quality of life, sleep-related self-efficacy and behavioural aspect…

Sleep deprivation may cause accidents, and it has deteriorating effects on health. A measurement of postural steadiness by a portable and af … resulting from watching footage of sleep-deprived people. This finding shows that a posturographic sleepiness tester quantifies physiological sleep deprivation, and is insensitive to the influence of social factors.

STUDY OBJECTIVES: The main objective of the study was to assess the prevalence, the severity, and the daytime course of excessive daytime sleepiness (EDS) in advanced Parkinson's disease (PD) and to explore how people with PD perceive the degree and onset of their sleepiness during objective sleepiness tests. In addition, the occurrence of early-onset rapid eye movement (REM) periods (sleep-onset REM periods [SOREMPs]) in PD was assessed.
METHODS: We analyzed data from 46 people with PD (26 males, mean age 63.5 years, mean UPDRS-III-OFF 34.7). The sleep-wake assessment included Epworth sleepi…

Sleepiness, the biological drive to sleep, is an important construct for the organizational sciences. This physiological phenomenon has received very little attention in the organizational science literature in spite of the fact that it influences a wide variety of workplace behaviors. In this article, we develop a framework through which sleepiness can be fruitfully studied. We describe (a) what sleepiness is and how it can be differentiated conceptually from related concepts such as fatigue, (b) the physiological basis of sleepiness, (c) cognitive and affective mechanisms that transmit the e…

PURPOSE: Excessive daytime sleepiness is the most common complaint reported in sleep clinics. We hypothesised that utilising modern media to deliver an online Epworth Sleepiness Scale, age- and gender-related differences in subjective daytime sleepiness could be assessed.
METHODS: Age, gender and online Epworth Sleepiness Scale (range 0-24 points) of 39,448 subjects were recorded between January 2013 and November 2015.
RESULTS: A significant trend, for males but not females, was found between age and Epworth score (p < 0.001). Average scores were higher for female subjects in their 1st and 2…

Sleepiness is a frequent and underrecognized symptom in neurological disorders, that impacts functional outcomes and quality of life. Multiple and potentially additive factors might contribute to sleepiness in neurological disorders, including sleep quality alterations, circadian rhythm disorders, drugs, and sleep disorders including sleep apnea or central disorders of hypersomnolence. Physician awareness of the possible symptoms of hypersomnolence, and associated causes is of crucial importance to allow proper identification and treatment of underlying causes. This review first provides a bri…

Excessive daytime sleepiness (EDS) is rather common in the elderly, however, it does not mean that EDS is physiological in that population. EDS may be caused by sleep related breathing disorder (SRBD), which is very popular among elderly people. Also EDS of narcolepsy is a life-long symptom. Recently, many reports come out indicating that EDS would be a predictor of future cognitive decline. One report shows that patients with preclinical Alzheimer disease have significantly higher number of naps per week than controls. EDs may be a harbinger of Alzheimer disease.

In summary, the evaluation of the tired patient requires an awareness of the various meanings of tiredness. Furthermore, it is important to differentiate normal sleepiness that is a product of circadian rhythm variation in vigilance from pathologic sleepiness. Sleepiness that results from faulty habits, e.g., altered sleep scheduling, drugs, or sleep restriction, can be readily discerned with the aid of a sleep-wake diary. Because subjective sleepiness is often unappreciated, especially in patients with sleep apnea, methods that rely on self-ratings of the severity of sleepiness, e.g., visual …

Sleepiness while driving is both a very common phenomena regarding 10 to 15% of drivers and one of the main causes of death on highways (one third of fatal accidents). Sleepiness results mainly from behavioural causes: sleep deficit or irregular work schedules. However, many sleep diseases and hypnotics may also induce sleepiness at the wheel. Several objective and subjective measures have been validated to assess sleepiness. Road safety and information to the drivers have now to focus on better informing drivers on the rules of preventing sleepiness at the wheel in order to avoiding hundred o…
