Smooth Muscle Tumors of the Female Genital Tract.

Smooth muscle tumors are the most common mesenchymal tumors of the female genital tract. However, awareness of tumor variants and unconventional growth patterns is critical for appropriate classification and patient management. For example, recognition of fumarate hydratase-deficient leiomyomas allows pathologists to alert providers to the potential for hereditary leiomyomatosis and renal cell carcinoma. Furthermore, myxoid and epithelioid smooth muscle tumors have different thresholds for malignancy than spindled tumors and should be classified by criteria specific to these variants. This art…

Criteria for malignancy in nonvisceral smooth muscle tumors.

Reliable prediction of the behavior of smooth muscle tumors is notoriously problematic at many anatomic locations. Relatively few studies have addressed the minimal histologic criteria for malignancy in smooth muscle tumors of soft tissue and the presently available criteria are not only inconsistently applied but also vary at different anatomic sites. It has been widely believed that the presence of any mitotic activity in a deep-seated smooth muscle tumor should lead to a diagnosis of leiomyosarcoma. Until very recently, the existence of leiomyomas in deep soft tissue has been disputed. Usin…

Rare giant perianal smooth muscle tumor: A case report.

Smooth muscle tumors are benign tumors originating from smooth muscle tissues, mostly occurring in the uterus and digestive tract; however, smooth muscle tumors occurring in the perianal sphincter are very rare. A 48-year-old woman presented with perianal swelling and pain and was misdiagnosed with a perianal abscess after examination at a foreign hospital. Upon further examination, the patient was diagnosed with a perianal smooth muscle tumor, with a size of approximately 6.5 × 5.5 × 4 cm. We succeeded in surgically removing the tumor. The patient was followed up for 2 years after the operati…

Completely infarcted smooth muscle tumor of the placental membrane.

Non-trophoblastic tumor of the placenta is rare, and so is placental smooth muscle tumor. We report leiomyoma of the placental membrane, which was discovered on cesarean section. Histologically, the tumor was a benign leiomyoma with complete necrosis, and this finding was confirmed immunohistochemically. Only six cases of smooth muscle tumors of the placenta have been reported to date. This is the third report of leiomyoma involving the placental membrane.

Transgelin, a Novel Marker of Smooth Muscle Differentiation, Effectively Distinguishes Endometrial Stromal Tumors from Uterine Smooth Muscle Tumors.

Differentiation between endometrial stromal sarcomas (ESSs) and smooth muscle tumors of the uterus can be challenging. Transgelin, a 22 kDa actin-binding protein has recently been shown to be a smooth muscle specific marker. The goal of this study was to determine whether transgelin could accurately distinguish ESSs from smooth muscle tumors. The expression of transgelin, CD10 and smooth muscle actin (SMA) in 13 ESSs (4 low grade, 6 undifferentiated and 3 metastatic), 9 smooth muscle tumors (1 leiomyoma and 8 leiomyosarcomas (LMSs) and 15 soft tissue LMSs was studied. The diagnostic performanc…

Association of Smoothelin, a Cytoskeletal Component of Smooth Muscle Cells, With the Proliferative Features of Uterine Smooth Muscle Tumors.

Smoothelin, a cytoskeletal protein expressed in smooth muscle cells (SMCs), colocalizes with α-smooth muscle actin and is typically restricted to the cytoplasm. Interestingly, in malignant smooth muscle tumors (SMTs), particularly uterine leiomyosarcoma (LMS), aberrant nuclear localization of smoothelin has been observed, though the underlying mechanism remains unclear. To investigate, 99 uterine SMT cases were analyzed, comprising 26 LMS and 73 non-LMS (typical leiomyoma, cellular leiomyoma, mitotically active leiomyoma, and tumors of uncertain malignant potential). Histopathological and immu…

Epstein-Barr virus-associated smooth muscle tumor of the tonsil.

Smooth muscle tumors of the tonsil are rare. Recently, the occurrence of Epstein-Barr virus-associated smooth muscle tumor (EBV-SMT) has been increasingly recognized in immunocompromised patients, mainly post-transplantation and AIDS patients. The clinicopathologic features of EBV-SMT are different from conventional smooth muscle tumors. To the best of our knowledge, EBV-SMT involving the tonsil in an AIDS patient has not been reported. A 27-year-old man presented with a 2.2cm right tonsillar mass six months after AIDS diagnosis. The tumor was composed of a cellular proliferation of oval to sp…

EMMPRIN (CD147) Expression in Smooth Muscle Tumors of the Uterus.

Smooth muscle tumors of the uterus are the most common mesenchymal tumors of the gynecologic tract. The vast majority of these are benign leiomyomas that present no diagnostic difficulty. Because some benign smooth muscle tumors may degenerate and uncommon variants exist, the diagnosis can be challenging in some cases. The goal of this research was to investigate EMMPRIN expression in leiomyomas, leiomyoma variants, and leiomyosarcomas (LMS) to determine whether it has a potential role in differential diagnosis. EMMPRIN expression was investigated with immunohistochemistry in 103 uterine smoot…

Primary intracranial smooth muscle tumors in immunocompromised patients.

Primary intracranial smooth muscle tumors are rare and range from benign to malignant. We report 2 patients with primary intracranial smooth muscle neoplasms in immunocompromised host, 1 patient each with primary intracranial leiomyoma and primary intracranial leiomyosarcoma, both of whom clinically and radiologically mimicked meningioma. Both patients were human immunodeficiency virus (HIV) positive. Hence, smooth muscle neoplasms should be considered in the differential diagnosis of well-circumscribed intracranial lesion, especially in immunocompromised patients.

Uterine smooth muscle tumor of uncertain malignant potential: a case report.

Uterine smooth muscle tumor of uncertain malignant potential (STUMP) is a rare borderline smooth muscle tumor. This paper reports a case of STUMP and discusses its clinical characteristics, diagnostic challenges, and treatment strategies in combination with relevant literature. The patient was a young female who presented with menorrhagia and severe anemia. Pelvic ultrasound revealed a large mass in the uterus, which was suspected to be a uterine leiomyoma. Surgical resection of the mass was subsequently performed, and postoperative pathological examination confirmed the diagnosis of STUMP. Th…

Preauricular Leiomyoma: A Rare Presentation of a Cutaneous Smooth Muscle Tumor.

Cutaneous leiomyoma, also known as leiomyoma cutis, is a rare presentation of an extra-uterine leiomyoma, often found on the extremities, trunk, and genitalia. Solitary lesions rarely occur on the face. This case presents a 63-year-old man with a one-year history of a growing mass in the preauricular region. On examination, a well-defined, painless mass was noted. Given the unusual yet visible location on the face, the decision was made to excise it surgically. After surgical resection, the biopsy specimen stained positively for smooth muscle actin (SMA), confirming a cutaneous leiomyoma. To t

Nasal smooth muscle tumor of uncertain malignant potential: A case report.

Smooth muscle tumors that cannot be histologically classified as leiomyomas or leiomyosarcomas are defined as smooth muscle tumors of uncertain malignant potential. The location of these tumors in the nose is very rare, and the appropriate surgical extent to manage these neoplasms has not been adequately defined. Here we describe the case of a 16-year-old female adolescent who consulted due to a vascular-like tumor in the right nasal cavity who was successfully treated with intranasal surgery. The histological diagnosis was smooth muscle tumor of uncertain malignant potential. Given that these…
