Management of chronic upper abdominal pain in cancer: transdiscal blockade of the splanchnic nerves.

BACKGROUND: The use of celiac plexus block to relieve the intractable pain caused by upper abdominal malignancies is well established. However, its effects are inconsistent for many reasons, mainly because of structural anatomic distortion as a consequence for the malignancy. The splanchnic nerve blockade (SNB) seems to be a useful alternative to the celiac plexus block in upper abdominal pain relief. MATERIALS AND METHODS: The pain of 109 patients with unresectable upper abdominal or lower esophageal neoplasms was managed by posterior transdiscal SNBs guided by computed tomography at the Ins

Classification of the abdominal splanchnic nerves--a preliminary attempt to re-evaluate their nomenclatures.

The abdominal splanchnic nerves were observed in 22 cadavers (41 sides). The purpose of the study is to give more accurate definition to each nerve, for there are uncertainties in the nomenclature of these nerves. According to PNA, the abdominal splanchnic nerves are classified into three categories, on the basis of their sizes and their levels, namely Nn. splanchnici major, minor, and imus. Although Mitchell (1935) had already claimed that more attention should be given to the levels of ganglia which gave rise to these nerves, no revision has ever been made. Moreover, there still remains the …

Osteophytes of the spine compressing the sympathetic trunk and splanchnic nerves in the thorax.

The presence of osteophytes compressing the sympathetic structures in the thorax was found in 655 (65.5%) of 1,000 cadavers. In 60.4% of the affected cases, the compression was on the right side, and in 36.9% it was bilateral, although the right side was more severely affected. In 2%, the compression was on the left only. The highest frequency of compression was at the T8-10 level, the right greater splanchnic nerve being the structure most frequently involved. The sympathetic trunk itself (ganglia and cord) was affected only by osteophytes of vertebrae at the lowest thoracic levels; however,

[Vascular relationships of the right great splanchnic nerve in the thorax].

AIM: The surgical assumption of responsibility of the pancreatic pain requires either a truncular coelioscopic or radicular neurectomy of greater splanchnic nerves (gsn). The goal of our work is to describe the way and relations of the right gsn which are variable and rarely described. This constitutes an undeniable peroperational hemorrhagic risk during splanchnicectomy. MATERIAL AND METHODS: After a double side thoracotomy and a bilateral sterno-clavicular desarticulation on 15 adult cadaveric subjects preserved by method of Winckler we removed the sterno-costal drill plate as well as the v…

Anatomical bases of the transhiatus approach to the greater splanchnic nerve.

The level of origin and mode of constitution of the greater splanchnic nerve and its relations in the posterior mediastinum were studied. The aim of this work was to identify the anatomical basis of the transhiatus approach to the right and left greater splanchnic nerves. The azygos venous system was seen to be the main anatomical relation of these nerves. The results of this study should allow the surgeon to perform total bilateral neurotomy.

Anatomical Measurements of the Thoracic Splanchnic Nerve and Clinical Applications.

OBJECTIVE: To describe in detail the characteristics of the travels and relationship of splanchnic nerves, and to provide an anatomical basis for splanchnic nerve block. METHODS: 1) For 20 thoracic splanchnic nerve anatomies, the relationship between the intersection of the greater splanchnic nerve (GSN) and lesser splanchnic nerve (LSN) and the T12 vertebrae was examined. 2) 6 patients with cancer pain in the epigram, unresponsive to conservative treatment, underwent splanchnic nerve radiofrequency ablation. The procedure was performed with computed tomography guidance, and the patients were…

Splanchnic Nerve Block: An Emerging Treatment for Heart Failure.

Heart failure (HF) is a disease syndrome whose management is increasingly challenging given the aging population and efficacious management of acute cardiac events. The current treatment options within our armamentarium incompletely address the unmet needs of HF. Splanchnic nerve block (SNB) is a novel technique that targets the greater splanchnic nerve, a potential therapeutic target in HF. However, the technique confers potential adverse side effects and complications that warrant further investigations. In this review paper, we aim to discuss the inextricable role of splanchnic nerve in HF

The value of angio-CT system on splanchnic nerve neurolysis.

PURPOSE: We aimed to evaluate the effectiveness and safety of splanchnic nerve neurolysis (SNN) with angio-CT, a hybrid system combining computed tomography (CT) with X-ray fluoroscopy. METHODS: Thirty-three SNN procedures with angio-CT performed in 30 patients with severe epigastric cancer pain (11 males and 19 females; median age, 57 years; age range, 19-79 years) between January 2010 and July 2017 were retrospectively evaluated. The primary endpoints were the technical success and adverse event rates. The secondary endpoints included the clinical success rate, defined as a reduction in the

Regulation of the bladder neck closure by lumbar splanchnic nerves at ejaculation in the dog.

To clarify the role of canine thoracolumbar splanchnic nerves for bladder neck closure during ejaculation, these nerves of adult male mongrel dogs were exposed under anesthesia using ketamine hydrochloride and pentobarbital, and electrical stimulation and anatomical dissection studies were performed. Bladder neck closure by the stimulation of each sympathetic nerve was monitored with a 10 Fr silicon catheter equipped with pressure-sensitive rubber balloon placed at the bladder neck. The dissection study revealed that canine thoracolumbar splanchnic nerves consisted of two nerve groups: one bra…

Anatomy of the Sacral Roots and the Pelvic Splanchnic Nerves in Women Using the LANN Technique.

AIMS: To report on our anatomic and electrophysiologic findings about the sacral nerve roots and the pelvic splanchnic nerves during laparoscopic pelvic surgery. METHODS: The pelvic splanchnic nerves and the sacral nerve roots were dissected in 336 consecutive patients undergoing laparoscopy for pelvic pain syndrome or gynecologic diseases. Intraoperative assessment of the functionality of the exposed nerves was performed using the LAparoscopic Neuro-Navigation (LANN) technique. RESULTS: Dissection of the sacral roots and the splanchnic pelvic nerves lateral to the sacral hypogastric fascia …

Participation of the splanchnic nerves in the structure of the cranial mesenteric plexus of the rabbit.

In this study the branch points of the greater splanchnic nerve, lesser splanchnic nerve and lumbar splanchnic nerves from the sympathetic trunk of the rabbit were investigated and their participation in the structure of the cranial mesenteric plexus was estimated.

Responses of T2-T4 spinal neurons to stimulation of the greater splanchnic nerves of the cat.

Effects of electrical stimulation of the greater splanchnic nerves on T2-T4 spinal neurons were determined in 16 cats anesthetized with alpha-chloralose. Of 77 neurons responding to somatic stimuli, 65 (84%) were excited, inhibited, or both excited and inhibited by splanchnic input. Each of the splanchnic responsive cells also was responsive to electrical stimulation of cardiopulmonary sympathetic afferent fibers. All but one neuron with left splanchnic input also received input from the right splanchnic nerve. Short- and long-latency excitatory responses were observed after splanchnic stimula…
