Ankylosing Spondylitis (AS) is an autoimmune disease of unknown cause belonging to the group of spondyloarthritides associated with HLA B27. This disease affects the spine and sacroiliac joints, but may also concern peripheral joints and different organs. Symptoms include morning stiffness and dull low back pain, both improving by exercise. AS causes reduction in life expectancy due to subsequent manifestation in different organs, so early diagnosis and treatment are of great importance.

Vertebral osteomyelitis or spondylodiscitis is an uncommon, mainly hematogenous, disease that usually affects adults. The incidence of this condition has steadily risen in recent years because of increases in spine surgery and nosocomial bacteremia, aging of the population, and intravenous drug addiction. Pyogenic infection due to Staphylococcus aureus is the most frequent form of the disease, but tuberculosis and brucellosis are still common causes of spondylitis in Spain. The clinical presentation is nonspecific and the diagnosis is often delayed. Magnetic resonance imaging is the most sensi…

Ankylosing Spondylitis (AS) is one of the rheumatoid diseases, however its clinical features are different. The diagnosis of AS and clinical features are shown, especially the diagnosis is very important. The new modified criteria for diagnosis is detailed here. In addition, the differential diagnosis is exhibited which is significantly useful for the diagnosis of AS. Recently As has been included in the group of seronegative spondylarthropathy (SPA). Now AS is thought to be the central position among SPA which has a lot of similar symptoms. The characteristic symptoms of SPA or AS are spinal,…

Infectious spondylitis usually involves osteomyelitis in two adjacent vertebral bodies and the intervertebral disc (spondylodiscitis). The most common location is the lumbar spine, followed by the thoracic spine. Symptoms are nonspecific, leading to a delay in diagnosis, in many cases, of several weeks. A large number of infectious agents can cause vertebral osteomyelitis, usually reaching the vertebra by hematogenous spread. The most commonly isolated agent is Staphylococcus aureus. Spondylitis remains the most common skeletal manifestation of tuberculosis. As with other forms of osteomyeliti…

Twenty-nine patients with pyogenic vertebral osteomyelitis were reviewed retrospectively after an average follow-up of 3.7 years. We identified 17 patients with predisposing factors (10 diabetes, 4 urinary tract infection, 2 HIV-positive, 1 rheumatoid arthritis). No patient presented with a febrile illness. The lumbar spine was involved in 15 patients. Eighteen patients had neurological impairment at presentation. Eleven patients who were neurologically intact had needle biopsies and the remaining 18 patients who were neurologically compromised had an open decompression. Staphylococcus aureus

The authors report on three cases of spondylitis brucellosa, and their course over a period of more than two years. The clinical and radiological characteristics of spondylitis brucellosa are described, as well as possible ways of distinguishing it diagnostically from tubercular and other non-tubercular types of spondylitis. In the main, confirmation of diagnosis will be provided by immunologic investigations, and these should be routine in any differential diagnostic identification of spondylitis.

A review of twelve cases in which disseminated coccidioidomycosis caused localized infection of the spine showed that eight of the twelve patients were alive and well with no evidence of active infection an average of eleven years after onset (range, two to thirty-five years). One patient who was followed for more than twenty-three years had a slowly developing neurological impairment in the lower extremities as a result of lumbosarcral destruction instability. One patient died early in the course of the disease from fulminating cervical spondylitis and quadriplegia. A second patient had a par…

PURPOSE OF REVIEW: Of the four musculoskeletal domains of psoriatic arthritis (PsA), the axial domain is the least studied. With the advent of targeted therapy that is efficacious in some but not all manifestations of spondyloarthritis (SpA), there is interest in understanding the similarities and differences between axial PsA (axPsA) and ankylosing spondylitis. Moreover, there is also interest in evaluating the axial domain in PsA clinical trials, domain that has traditionally been ignored in such studies. This review aims to summarize the current understanding of the differences between anky…

Erosions of vertebral bodies and intervertebral discs are usually considered as late manifestations of ankylosing spondylitis. Yet they may occur early and indeed constitute the first sign of the disease. This is illustrated by the case reported here, where a 19-year-old woman with an 18-months diagnosis of ankylosing spondylitis was found to have severe destructive lesions of the 6th cervical vertebra without discal lesion nor cervical ankylosis. Surgical biopsy excluded a tumoral or infectious process. Spinal fusion (C4-C7) was performed, and cervical pain was completely relieved with non-st…
