Spondylolysis and spondylolysthesis.

On the basis of our own experience and the most up-to-date reports, we present some of the most controversial features of spondylolysis and spondylolisthesis. In particular, the importance of stress in the etiology of the lysis has been revealed by research carried out on athletes practicing strenuous sports. It has also been shown how the slipping of the olisthesic vertebra does not necessarily cease around the age of 20, but may continue, under certain conditions, even in the young adult. Finally, advice on treatment is given in relation to the various surgical techniques, emphasizing the im…

Spondylolisthesis and spondylolysis.

Spondylolisthesis is a common condition that can be managed both nonsurgically and surgically. More than 80% of children treated nonsurgically have resolution of symptoms. For those patients requiring surgical treatment, fusion in situ may provide adequate treatment for young patients. Patients with neural compression may require decompression to relieve symptoms, and fusion is also usually indicated. High-grade and degenerative spondylolisthesis require care that is unique to those conditions. Spondylolysis is a defect in the pars interarticularis that occurs in approximately 5% of the genera…

Spondylolysis and spondylolisthesis.

Lysis is common and its causes are mainly constitutional. Most of slipping has already occurred when the patient comes for consultation. A further progress is often caused by disc pathology. Neurological deficits are rare. A high-degree olisthesis and L4 location are risk factors for future back pain. Asymptomatic lysis with or without olisthesis should not be treated. A posterolateral fusion in situ without instrumentation gives good results in adolescents and young adults, whereas old patients benefit from instrumentation. Reduction cannot be recommended as a routine method.

Spondylolysis in Athletes.

The high occurrence of spondylolysis in these athletes suggests that certain sports actions predispose athletes to fractures in the neural arches of the lumbar vertebrae.

Comparison of alignment and spondylolysis fracture angle in bilateral and unilateral spondylolysis.

OBJECTIVES: Spondylolysis occurs bilaterally or unilaterally and bilateral spondylolysis increases the risk of developing isthmic spondylolisthesis. The characteristics of the lumbar lordosis angle (LLA), sacral slope angle (SSA), and spondylolysis fracture angle (SFA) in bilateral spondylolysis compared with those in unilateral spondylolysis have not been clarified. The purpose of this study was to compare the LLA, SSA, and SFA of bilateral and unilateral spondylolysis. MATERIALS AND METHODS: Thirty-eight patients with lumbar spondylolysis who visited our clinic for an initial visit and 15 a…

Multi-level spondylolysis.

The incidence of isthmic spondylolysis is approximately 3% to 6% in the general population. Spondylolytic defects involving multiple vertebral levels, on the other hand, are extremely rare. Only a handful of reports have examined the outcomes of surgical treatment of multi-level spondylolysis. Here, we present one case of bilateral pars defects at L3, L4, and L5. The patient, a 46-year-old female, presented with lower back pain radiating into the left lower extremity. Radiographs and CT scans of the lumbar spine revealed bilateral pars defects at L3-L5. The patient underwent lumbar discectomy …

Spondylolysis 2019 update.

PURPOSE OF REVIEW: Provide a comprehensive overview of lumbar spondylolysis, a frequent cause of lower back pain in children and adolescents, from evaluation to management. RECENT FINDINGS: With the surge of structured sports participation in the pediatric population, spondylolysis is a common ailment that afflicts many young athletes due to rigorous competition that taxes the growing spine with repetitive extension and rotation. SUMMARY: Spondylolysis is a fracture through the pars interarticularis. When a child presents with lower back pain, spondylolysis should be at the top of the differ…
