In order to prospectively study the development of strabismus in infancy, the ocular alignment of a large population of normal neonates was studied and follow-up examinations were obtained in a subset of these infants. The characteristic findings of congenital esotropia subsequently developed in three infants who were either orthotropic or exotropic at birth. Pathologic exotropia developed in two infants; both were exotropic at birth, but no more so than most normal neonates. In infants with congenital esotropia or pathologic exotropia, the characteristic deviation appears to develop between 2…

Penalization includes all those methods apart from total occlusion which disturb the central vision of one eye by favouring the other eye. Penalization has a sensory action on functional amblyopia; moreover it also has a motor action, influencing the non-accommodative element of spasm in esotropia. Optical penalization is the most classical method. However, one should also be aware of penalization with partial occlusion or fogging, penalization with mydriatics, and penalization with sectors, which are very often used in association with the classical technique.

The original causes of strabismus and the complications that develop from efforts by a child to adjust to the condition may present a complex diagnostic and therapeutic problem. The complications of suppression, amblyopia, contractures and anomalous visual habits may become more difficult to correct than the cause of the strabismus. The longer the duration of strabismus, the more complex the problem, the more difficult the treatment, and the less the probability of functional cure. Treatment should be started early. Correction may be brought about by the use of glasses, eye patches and orthopt…

Strabismus is common in craniosynostosis, with rates from 39% to 90.9% in Crouzon, Apert, Pfeiffer, and Saethre-Chotzen syndromes. This article reviews the epidemiology of strabismus in these disorders and discusses competing theories of the mechanism, including absent muscles, excyclorotation of muscles, and instability of muscle pulleys. The authors then review options for surgical treatment of the often complex ocular misalignment in these disorders.

Strabismus (misalignment of the eyes; also known as "squint") comprises a common heterogeneous group of disorders characterised by a constant or intermittent ocular deviation often associated with amblyopia (uniocular failure of normal visual development) and reduced or absent binocular vision. The associated poor cosmetic appearance may also interfere with social and psychological development. Extensive twin and family studies suggest a significant genetic component to the aetiology of strabismus. The complexity of the molecular basis of strabismus is now beginning to be elucidated with the i…

The etiology of strabismus has long been observed to have a genetic component. Recent advances in genetic methodology may provide insight into the genetic basis for several types of inherited strabismus, including those associated with genetic multisystem disorders such as Moebius syndrome, Prader-Willi syndrome, craniofacial dysostoses, and mitochondrial myopathies. Inheritance of primary forms of strabismus, such as congenital ocular fibrosis, Brown syndrome and Duane syndrome, has been reported, but less is known of the defective genetic sites. The genetic basis for isolated strabismus that…

Two hundred twenty-six consecutive surgical ptosis cases were carefully evaluated to establish the incidence of strabismus in ptosis. The overall incidence of strabismus in surgical ptosis patients was found to be 36%. Most remarkable were the findings in 113 congenital ptosis patients; 32% of these patients were found to have strabismus. Only 44% of these patients had entities previously described associating strabismus with ptosis. It is postulated that in four of the congenital ptosis patients (3.5%), strabismus developed as a result of their ptosis. In one of these patients, strabismus and…

We have discussed what constitutes a normal visual and ocular system in children and the inherent mechanisms that help each person develop and maintain normal visual function. This ideal status should include good visual acuity in each eye, straight eyes, and the development of fusion. Strabismus represents an obstacle to obtaining these goals and may be caused by poor vision, refractive errors, or anatomic, structural, or innervational abnormalities. Each of these conditions is treatable and correctable when discovered early. With early identification, proper treatment can be instituted and t…

PURPOSE: To present stepped strabismus surgery as a novel technique in cases of small to moderate angle strabismus.
METHODS: Retrospective chart review of cases of stepped strabismus surgery from 2010 untill 2018. In stepped surgery, the first muscle is operated on under rapid induction-recovery IV propofol infusion. Patient is assessed in the OR. If deemed necessary, other muscles are operated on under general anesthesia. No adjustable sutures are used.
RESULTS: The technique was used in 22 cases of superior oblique palsy (SOP) (primary position hypertropia in the range of 12-25 prism diopt…

BACKGROUND: The vertical strabismus manifested by Abraham Lincoln has been noted. This article reviews the historical findings and provides a specific diagnosis.
MATERIALS: Previous reports of symptoms and history relating to Lincoln's left hypertropia were reviewed. A series of photographs were reviewed. Lincoln's own description of his symptoms is provided.
RESULTS: Previous history indicates an intermittent left hypertropia. A family history of vertical strabismus was noted with regard to Mr Lincoln's cousin. There also is a history of trauma to the left frontal area and life-mask evidenc…

A 20-year-old woman with a past history of a sinonasal undifferentiated carcinoma involving the skull base and cavernous sinus, previously treated with surgery and radiation therapy, presented with intermittent horizontal diplopia induced by gaze to the right side. The gaze in the direction of the action of the right VI nerve produced sustained contraction of the right lateral rectus and an exodeviation increasing on left gaze. Ocular neuromyotonia involving the right VI nerve was believed to be secondary to the patient's previous radiation therapy. The discussants review the prior literature

Age related binocular vision disorders (age related strabismus) could include all types of spontaneous strabismus appearing along the lifetime, which are directly connected with the aging process. Neurological strabismus is excluded. Functional and structural changes related to the aging process can induce phoria decompensation, convergence insufficiency decompensation and new onset strabismus as distance esotropia or vertical strabismus induced by the sagging eye syndrome. MRI studies sustain the hypothesis of age related structural changes of extraocular muscles pulleys and their influence o…

PURPOSE OF REVIEW: With both radiologists and ophthalmologists in mind, this article describes the best use of computed tomography and magnetic resonance imaging in the diagnosis of strabismus.
RECENT FINDINGS: Both computed tomography and magnetic resonance imaging are valuable tools in evaluation of strabismus, despite concerns about radiation exposure with computed tomography. Advancements in magnetic resonance imaging are being made and open new techniques that can be useful in the future evaluation of strabismus.
SUMMARY: Computed tomography and magnetic resonance imaging are safe tools…

INTRODUCTION: Ocular myasthenia gravis is a protean disorder and can present with myriad disorders of ocular motility. The diagnosis may not be obvious at presentation and strabismus surgery has occasionally been performed with unexpected outcomes. Strabismus surgery can be performed on patients with ocular myasthenia gravis who have stable ocular deviations, although outcomes may vary.
CASE REPORT: We report two patients who were operated for strabismus and were diagnosed as ocular myasthenia gravis later, when classic signs appeared. The first patient underwent right medial rectus recession…
