Perinatal group B streptococcal infections.

Predisposing factors and clinical presentations associated with early-onset group B streptococcal (GBS) infections in 60 infants were reviewed. Over a three-year period, the incidence of these infections in The Royal Women's Hospital, Melbourne, was 2.0 per 1000 births. The clinical presentation ranged from fulminating sepsis to asymptomatic bacteraemia. In some cases, there was evidence of intrauterine fetal infection, despite intact membranes and a lack of clinical evidence of maternal infection. However, the over-all incidence of fever, before and after delivery, among the mothers of these

Necrotizing group A streptococcal infections associated with streptococcal toxic shock syndrome.

BACKGROUND: Group A streptococci (GAS) cause a variety of life-threatening infectious complications, including necrotizing fasciitis (NF), purpura fulminans (PF), and streptococcal toxic shock syndrome (strepTSS), in which bacteremia is associated with shock and organ failure. METHODS: We reviewed our experience in the management of patients with necrotizing GAS infections from 1991 to 1995. RESULTS: Eight adult patients (6 NF, 2 PF) were identified. Patients presented with fever, leukocytosis, and severe pain, and rapidly developed shock and organ dysfunction. The diagnosis of strepTSS was

[Group B streptococcal infections in adults, excluding genital infections].

Group B streptococcus (GBS) or Streptococcus agalactiae is recognized as a mayor cause of neonatal meningitis, sepsis and infections during pregnancy. However, in recent years there have been several reports concerning GBS infections in non pregnant adult population, specially in immunocompromised hosts and in patients with severe underlying diseases such as diabetes mellitus and cancer. We report a series of 45 cases which occurred in nonpregnant adult population during a period of two years. The average age was 50.8 years and most patients (38/44) had one or more risk factors: diabetes melli

Resurgence of Group A Streptococcal Infections.

Group A Streptococcus (GAS) causes considerable burden on the society with its varied disease manifestations than can range from asymptomatic infection to severe invasive disease which can lead to significant mortality. There was a decline in GAS infections over time with improved living conditions all over the world, however, with the COVID-19 pandemic, there has been a resurgence in the different types of GAS infections. This recent upsurge in the GAS cases worldwide and the increasing rates of antimicrobial resistance has brought the spotlight back on this organism. Many vaccines are under

[Invasive Streptococcal infections - an interdisciplinary challenge].

HISTORY AND CLINICAL FINDINGS: We report on a 34-year-old female patient and a 50-year-old male patient, both of whom were admitted to our emergency department with severe septic conditions. MEDICAL EXAMINATIONS: Both patients were resuscitated and exhibited clinical as well as laboratory evidence of a severe bacterial infection. DIAGNOSIS: Both patients had an invasive infection with Group A Streptococcus. The female patient had a Streptococcal sepsis with severe pneumonia, while the male patient had a Streptococcus-induced necrotizing fasciitis of the upper extremity. THERAPY AND COURSE:

Immunopathogenesis of streptococcal deep tissue infections.

Streptococcus pyogenes is an important human pathogen that can cause a variety of diseases in immunocompetent individuals ranging from uncomplicated superficial infections to severe life-threatening infections including rapidly progressing deep tissue infections, such as necrotizing fasciitis (NF) and severe cellulitis. The pathogenesis of these infections is complex and multifactorial involving numerous virulence factors expressed by the bacteria. Here, we review data from epidemiologic, pathogenomic, and pathogenesis studies that have provided insight into the host-pathogen interactions that

Pathogenesis of group A streptococcal infections.

Group A Streptococcus (GAS; Streptococcus pyogenes) is a human pathogen which causes significant morbidity and mortality globally. GAS typically infects the throat and skin of the host, causing mild infections such as pharyngitis and impetigo, in addition to life threatening conditions including necrotizing fasciitis, streptococcal toxic shock syndrome (STSS), and bacteremia. Repeated infection with GAS may result in the non-suppurative sequelae, acute rheumatic fever, and acute glomerulonephritis. GAS remains sensitive to the antibiotic penicillin which can be administered as a means to treat

Group A streptococcal infections in children.

The group A streptococcus causes the widest range of disease in humans of all bacterial pathogens. Group A streptococcal diseases are more common in children than adults with diseases ranging from pharyngitis and impetigo to invasive infections and the post-streptococcal sequelae--acute rheumatic fever and acute post-streptococcal glomerulonephritis. The global burden of severe group A streptococcal disease is concentrated largely in developing countries and Indigenous populations such as Aboriginal Australians. Control of group A streptococcal disease is poor in these settings and the need fo

Group A beta-hemolytic streptococcal infections.

GABHS is the most common bacterial cause of tonsillopharyngitis, but this organism also produces acute otitis media; pneumonia; skin and soft-tissue infections; cardiovascular, musculoskeletal, and lymphatic infections; bacteremia; and meningitis. Most children and adolescents who develop a sore throat do not have GABHS as the cause; their infection is viral in etiology. Other bacterial pathogens produce sore throat infrequently (e.g., Chlamydia pneumoniae and Mycoplasma pneumoniae), and when they do, other concomitant clinical illness is present. Classic streptococcal tonsillopharyngitis has

Group B streptococcal infections during pregnancy.

Group B streptococcus, acquired intrapartum from the female genital tract, is the leading cause of neonatal sepsis. Intrapartum maternal antibiotic prophylaxis administered to colonized parturients is efficacious in reducing neonatal colonization and morbidity. However, the optimal method of identifying mother-infant pairs at risk remains controversial. Universal antenatal screening has been effective in identifying colonized women for subsequent intrapartum therapy in research protocols. However, the cost-effectiveness and efficacy of this management scheme, in a general population, has not b

Management of staphylococcal and streptococcal infections.

Staphylococcus and streptococcus are bacteria commonly found on the skin and in the mucous membrane. These bacteria are frequently associated with mild cutaneous infections but may invade to cause deeper infections including necrotizing soft-tissue infections, sepsis with or without metastatic lesions, and immunologic or toxin associated sequelae. Early recognition and timely intervention are the hallmarks of appropriate management. Initial empiric therapy should be directed towards a broad spectrum of pathogens that include staphylococcus and streptococcus.

Severe streptococcal infections in historical perspective.

The recent unexplained increase in severe streptococcal diseases in the United States and Great Britain is compared to the 1825-1885 pandemic of fatal scarlet fever. Although scarlet fever may not be representative of all severe streptococcal disease, it was the only one reliably identified in the 19th century. The epidemiology of scarlet fever during the 19th century pandemic suggests the following features of the disease; cocirculation of both virulent and less-virulent streptococcal strains eliciting cross-immunity; circulation of hyperendemic prevalent strains in urban centers of developed
