The ultrastructure of the subgingival deposits on the root surfaces of teeth affected by juvenile periodontitis was studied on 12 teeth from nine individuals, 15--30 years of age. The deposits consisted of either microbial masses associated with a pellicle, or of a cuticular material almost free of bacteria. Gram-negative rods and filaments were the predominant microorganisms. "Corncob" configurations consisting of filamentous bacteria surrounded by Gram-positive cocci, and "bristle brush" formations comprising corncobs surrounded by long rods were observed in the superficial layer of the plaq

Clinical lasers are of two types. Soft lasers are essentially an aid to healing, with relatively few rigorous studies available to support their use. Surgical hard lasers, however, can cut both hard and soft tissues, replacing the scalpel and drill in many areas. After initial experiments with the ruby laser, most clinicians have been using argon, carbon dioxide, and now Nd:YAG systems. The first dental laser based on a Nd:YAG engine provides handpieces of similar size to conventional instrumentation, and being fed by a fibre-optic "cable," has the flexibility for intra-oral use that the carbo

Some aspects of pathogenesis and therapeutic approach to the gingival recession are discussed in this short communication with the example of a typical case from Russia, where excessive socket curettage after a tooth extraction resulted in a marked gingival recession. Subgingival plaque and calculus can be secondary to recession. An argument about plaque as a source of microorganisms might be plausible in case of inflammation, although various microorganisms are normal for the oral cavity. From the viewpoint of general pathology, being an atrophic condition, recession can progress due to repea

The Evian Debridement Bur facilitates two fundamental techniques in periodontal surgery--soft-tissue debridement and root planing. The bur debrides granulation and other soft tissues attached to the root or bone and removes hard deposits from root surfaces. It can also be used as an adjunct while performing crown-lengthening procedures and endodontic surgery. The bur is made with blunted flutes and, if used with a relatively light touch, will not cut into the alveolus and will not damage the tooth surface. This article reviews the use and advantages of this bur and shows its effect on the root

The author takes a personal 33-year retrospective view of non-surgical periodontal therapy. The frustrations of the pash have given way to the effective and successful treatments of today. Current concepts of etiology and new non-surgical treatment techniques are reviewed. The outlook for the future is decidedly optimistic.

In scanning electron microscopy (SEM) of the dental root surfaces human cementum resorption by giant multinucleated cells appeared similar in temporary and permanent teeth. Thus the resorbed zones were formed by associated Howship lacunae. On the lacunae walls, the openings of numerous tubules of the cementocytic lacunae were seen. Such aspects were noted during the physiological resorption of temporary teeth. The cementum surface of permanent teeth without pathology, presented frequently such resorption aspects around their apical regions as well as on other regions, the resorbed areas being

Interproximal channeling defects associated with lingual denudation of the root are often encountered in patients with insidiously progressive chronic periodontitis. Several cases of such lesions are described and their potential for repair demonstrated. It is suggested that this type of defect is predictably amenable to successful therapy by exposure and curettage, and that restoration of lost periodontal structures can be anticipated within the interproximal portion of the lesion.

AIM: We recently introduced an experimental surface detection system based on a conventional dental ultrasonic scaler. This device automatically discriminates cementum and dental calculus, which is the prerequisite for complete and thorough calculus removal. In the present study, the detection limits of this device were tested in vitro.
MATERIAL AND METHODS: From 50 extracted teeth, subgingival calculus was gradually removed using a Gracey curette. During this stepwise procedure, detection properties of the surface detection system were continuously monitored and systematically verified until

OBJECTIVE: This case report describes the orthodontic and prosthetic rehabilitation of a patient with resorption of the roots of the maxillary central incisors due to the ectopic maxillary canines.
CLINICAL CONSIDERATIONS: A 16-year-old woman presented with severe resorption of the roots of the maxillary central incisors due to the ectopic maxillary canines. The impacted canines were orthodontically tracted with a lingual arch-supporting temporary central incisors and vertical elastics, and, undesirable root proximity was later corrected by moving the canines distally 1.5 mm apart. Gingival r

This study investigated the ability of a new instrument designed for root debridement, the Periosonic, to remove dentine compared to a hand curette and an ultrasonic scaler. The Periosonic system is a modification of the Micro-Megax endodontic system and uses modified files to remove plaque, calculus and tooth substance. The Periosonic has 2 file types; the Periosonic 1 is a more aggressive file for the removal of gross supra- and subgingival deposits, while the Periosonic 2 is a thinner more flexible file used for the removal of subgingival deposits. 40 human dentine specimens were ground fla

In 16 incisors, one canine, and one premolar with an acute periapical infection which had perforated the cortical bone, an apical curettage was carried out. During the treatment and the following 5 days, the patients were protected with antibiotic therapy. The curettage was performed through a slightly arched incision, convex toward the gingival margin. At the same sitting, a guttapercha root filling was made in the conventional way. Irrespective of operation, which involved a somewhat larger than normal incision, no spread from local to general infection was found in any of the cases. The pat

Invasive cervical resorption (ICR) is a relatively uncommon, insidious, resorptive lesion starting subgingivally at the cervical root surface of a tooth. ICR is of uncertain aetiology, although damage to the cervical periodontal attachment to the tooth appears to be a prerequisite. For the most part the lesion is asymptomatic so early detection can be difficult. Nevertheless, if less than a third of the root of the tooth is affected by an ICR lesion treatment of the resorptive tissue using the chemical escharotic agent trichloracetic acid, with or without surgical access, followed by curettage

The effects of CO2 lasers on human gingiva have been evaluated. Pulsed Nd:YAG lasers have only been available for clinical use in dentistry since 1990. This study evaluated the efficacy of a low-power pulsed laser in removing pocket lining epithelium in humans with moderate periodontitis. 24 specimens of gingival tissue, from 6 patients, were studied microscopically following the application of a pulsed Nd:YAG laser (1064 nm) contact probe with a spotsize of 320 mu. Power settings of 1.25 and 1.75 Watts and a frequency of 20 Hz were used. Treatment time varied from 2 to 3 min. The specimens we
