Early Revision of Suburethral Sling for Persistent Stress Incontinence.

BACKGROUND: Midurethral slings are an effective treatment method for stress urinary incontinence (SUI). Although success rates can be as high as 90%, the remaining patients with disappointing results may require further surgical intervention. In particular, those patients with persistent SUI may need further revision of the existing sling. Literature is limited in the optimal treatment of these patients. CASE SERIES: Five patients who underwent midurethral slings for SUI and had subsequent sling revision were included in this analysis. The technique of revision is outlined in this article. Su

Protective effect of suburethral slings on postoperative cystocele recurrence after reconstructive pelvic operation.

OBJECTIVE: The purpose of this study was to evaluate the independent effect of suburethral sling placement on the risk of cystocele recurrence after pelvic reconstructive operation. STUDY DESIGN: One hundred forty-eight women with cystoceles to or beyond the hymenal ring underwent pelvic reconstructive operation, with or without incontinence procedures, and were evaluated at 12 and 52 weeks after operation with a standardized pelvic examination. Rates of recurrent prolapse, at all sites, were statistically compared between subjects with and without suburethral slings. A multiple regression an

[Correction of stress urinary incontinence in women with a free suburethral sling].

The most serious problem which must be solved in surgical treatment of stress urinary incontinence is providing adequate support for the middle urethra or urinary bladder neck with minimal sling tension. High frequency of positive results of implantation of a free synthetic sling is explained by the fact that a prolen tape does not extend and reliably arrests shift of the vesico-urethral angle in elevation of intraperitoneal pressure unlike other artificial supports (a fascial loop, fixing ligatures, ect.). Thus, the TVT operation is an effective treatment of stress urinary incontinence in wom

Postoperative erosions of the Mersilene suburethral sling mesh for antiincontinence surgery.

The objective of the study was to describe the presentation of vaginal mesh erosions following Mersilene suburethral slings for urinary incontinence. We performed a retrospective review of all Mersilene suburethral slings placed at a tertiary referral center from 1996 to 2007. A total of 772 women underwent placement of a Mersilene suburethral sling. We identified 62 women that underwent surgical revision due to mesh erosion for an overall erosion rate of 8%. The most common presenting symptom was vaginal discharge reported in 37% of women. Other symptoms included vaginal bleeding in 31%, pain

Sling incision with associated vaginal wall interposition for obstructed voiding secondary to suburethral sling procedure.

Obstructed voiding is a well recognized complication following a suburethral sling procedure. Current methods of transvaginal and transabdominal urethrolysis have had variable success rates in relieving the obstruction, and do not restabilize the urethra to prevent potential postoperative stress incontinence. We report on a procedure used in 4 cases which addresses both of these issues. All patients had persistent urinary retention beyond 8 weeks after a suburethral sling procedure. An oupatient procedure consisting of sling incision and associated tissue interposition (vaginal wall in three c

Suburethral sling procedures in the United States: complications, readmission, and reoperation.

INTRODUCTION AND HYPOTHESIS: Hospital readmission is increasingly used as a measure of quality care. Our objective was to evaluate the rate of readmission and associated factors in women undergoing suburethral sling placement in the USA. METHODS: This is a cohort study of suburethral sling procedures performed in the USA from 2012 to 2014 using the American College of Surgeons' National Surgical Quality Improvement Program database. The database was queried for suburethral sling procedures. Records were excluded if concomitant surgery had been performed. Patient and surgical characteristics w

Pain after suburethral sling insertion for urinary stress incontinence.

INTRODUCTION AND HYPOTHESIS: Pain after suburethral sling insertion is an uncommon, but troublesome complication of continence surgery; in some cases it can cause significant distress and disability. It is often not recognised, poorly diagnosed and treated by gynaecologists and urologists unfamiliar with the condition. METHODS: A literature review was performed and the current evidence on pain after suburethral sling insertion reviewed. RESULTS: The tension-free vaginal tape-obturator (TVT-O) procedure is associated with an increased incidence of early groin pain, but this often resolves wit

The urethral motion profile before and after suburethral sling placement.

PURPOSE: We examined the effect of the Monarc suburethral sling on urethral mobility. MATERIALS AND METHODS: We retrospectively studied the records of 54 consecutive women who received a Monarc suburethral sling between July 2005 and November 2008. All patients were examined by volume ultrasound preoperatively and at followup (average 0.7 years). Volume data sets were analyzed using post-processing software. Urethral mobility was described by vectors of movement from rest to a maximum Valsalva maneuver of 6 equidistant points marked evenly along the urethra from bladder neck (point 1) to exte

[Synthetic suburethral sling in the treatment of stress urinary incontinence in women].

UNLABELLED: Since the marketing and success of the TVT technique, other products have been released onto the suburethral sling market. These products can be classified according to four main criteria: TAPE: The material used in every case is polypropylene, in the form of woven monofilaments (TVT, Sparc, Monarc, Uretex Sup), multi-filaments (IVS, Safyre comprising two silicone tension bands), extruded crosslinked polypropylene (Obtape) or silicone-coated polypropylene in contact with the urethra (Uratape). A larger space between the fibres is associated with a lower theoretical risk of infectio

[The voiding after the suburethral sling operation, obstructive or non-obstructive?].

To examine whether or not the suburethral sling operation produces obstruction during voiding, seven females who underwent the sling operation using synthetic material (Vesica Sling Kit) were studied postoperatively urodynamically. Uroflowmetry and residual urine measurement showed no overt voiding difficulties in any cases. However, in one case, a pressure flow study indicated equivocal and the position of the sling material was judged to be too proximal by fluoroscopic monitoring. In all other 6 cases pressure flow was shown to be non-obstructive. Fluoroscopic finding also demonstrated in th

[Delayed necrotizing fasciitis: a complication of stress incontinence surgery by suburethral sling].

We report a 50-year-old woman case with an extensive necrotizing fasciitis (NF). The NF appeared 10 years after a tension free vaginal tape procedure for urinary stress incontinence. Vital prognosis was engaged due to the initial sepsis severity. This kind of complication is rare and could be under estimated. NF usually appear soon after surgery, whatever within the year following implantation. Anyway, NF are always related to a vaginal erosion of the tape.
