[The new oral anticoagulants in surgery].

We can estimated the probability of developing thromboembolic disease and consequently prescribed antithrombotic measures to patients undergoing surgery. The anticoagulants carry the risk of abnormal bleeding, which can sometimes be fatal. Recently, this concept changed with the development of new drugs that retain their antithrombotic activity but decrease their anticoagulant effect; other advantages are: route of administration, predictable bioavailability (generally do not require monitoring), and little interaction with food and other drugs. The most representative are direct factor Xa inh

Piezosurgery in oral and maxillofacial surgery.

This review summarizes current knowledge and experience with piezosurgery, a promising, meticulous and soft tissue-sparing system for bone cutting, based on ultrasonic microvibrations. The main advantages of piezosurgery include soft tissue protection, optimal visibility in the surgical field, decreased blood loss, less vibration and noise, increased comfort for the patient and protection of tooth structure. To date it has been indicationed for use in oral and maxillofacial surgery, otorhinolaryngology, neurosurgery, ophthalmology, traumatology and orthopaedics. The main indications in oral su

Dracunculiasis in oral and maxillofacial surgery.

Dracunculiasis, otherwise known as guinea worm disease (GWD), is caused by infection with the nematode Dracunculus medinensis. This nematode is transmitted to humans exclusively via contaminated drinking water. The transmitting vectors are Cyclops copepods (water fleas), which are tiny free-swimming crustaceans usually found abundantly in freshwater ponds. Humans can acquire GWD by drinking water that contains vectors infected with guinea worm larvae. This disease is prevalent in some of the most deprived areas of the world, and no vaccine or medicine is currently available. International effo

The use of collagen in oral surgery.

The suitability of collagen as a biomaterial for soft tissue temporary cover or implantation is discussed. Current methods used to cover areas denuded of mucosa in the mouth using split skin, mucosa and dermal autografts are examined. The properties of an ideal mucosal graft are stated and the suitability of a collagen heterograft in the form of freeze dried, chemically treated pigskin is questioned. A collagen xenograft has been used as a biological cover for areas of the mouth where soft tissue has been excised, for example, a denture irritation hyperplasia or leukaplakia and when increased

Arteriography in oral and maxillofacial surgery.

A brief review of the methodology of selective external carotid arteriography is presented together with its risks, complications, and contraindications. The indications for this procedure are described with illustrative case material to demonstrate its usefulness in both diagnosis and preoperative evaluation of various types of lesions of the head and neck that may be encountered in the practice of oral and maxillofacial surgery.

Oral surgery at St. Mary's Hospital, Chapelizod. A quarter century experience of day-case oral surgery.

To mark the first quarter century of the existence of the Oral Surgery unit at St. Mary's Hospital, Chapelizod, an audit was undertaken. Twenty-nine thousand, three hundred and seventy-four (29,374) entries in the operations register were analysed, examining annual totals, categories of operation, the changing patterns of anaesthesia, including cancellations on the day, and the evolution of a true day-case service. The results showed a decline in total numbers and in some operation categories. The most striking reductions were in extractions of carious teeth and in preprosthetic surgery. The m

Clinical practice guideline of the spanish society of oral surgery for oral surgery in patients with coagulation disorders.

BACKGROUND: The number of patients treated with coagulation disorders, and more specifically with anticoagulant therapy, has increased worldwide in recent years due to increased life expectancy in developed countries. The protocols for managing this type of patient in oral surgery has varied over recent years, especially after the appearance of new direct-acting oral anticoagulants (DOACs). The assessment of risk of bleeding in this type of patient when undergoing a surgical procedure continues to be a controversial issue for patients, dentists and general practitioners. The objective of this

[Elective surgery and oral carbohydrate loading].

OBJECTIVE: To evaluate the effect of preoperative oral carbohydrate loading on the course of perioperative period. MATERIAL AND METHODS: We examined 93 patients who underwent elective abdominal and retroperitoneal surgery. In the main group (n=47), carbohydrate drink was prescribed prior to surgery (33.5 g of carbohydrates and 4 g of hydrolyzed protein per 100 ml): 400 ml in the evening before surgery and 200 ml 2 hours before surgery. The control group included 46 patients who followed conventional fasting protocol recommended by the ASA (solid food no later than 6 hours before surgery, inta

New oral anticoagulants and oculoplastic surgery.

The use of oral anticoagulants is increasing because of the rising prevalence of atrial fibrillation and other cardiovascular diseases that occur in an aging population. In the last decade, several new oral anticoagulants have emerged, comprising the direct thrombin inhibitor dabigatran and the factor Xa inhibitors apixaban and rivaroxaban. Although these new anticoagulants may not affect clear corneal cataract surgery, their use will impact the management of patients having eyelid, orbital, and nasolacrimal procedures. The objective of this review is to provide a concise and practical approac

Outpatient general anaesthesia for oral surgery.

Outpatient general anaesthesia (GA) is administered for extraction of carious teeth in children and straightforward dento-alveolar surgery such as removal of impacted teeth, cyst enucleation and dental implantology in adults. Although oral surgery is often performed under local anaesthesia, GA is indicated in conditions when local anaesthesia is ineffective and in uncooperative patients because of age, fear or anxiety, mental impairment or physical disability. Outpatient GA for oral surgery yields a high satisfaction score from the patients. The incidence of major morbidity is acceptably low b

[Microwave therapy in outpatient oral surgery].

The biological action of microwave therapy is due to the conversion of high-frequency electromagnetic waves to heat that is produced directly in the tissue at the radiation area. It is a deep heating procedure of limited range with low-level exposure to the subcutaneous fatty tissue; it may be used in subacute and chronic infectious inflammatory conditions, after surgical interventions, in affections of the salivary glands and of the maxillary sinuses, and also in temporo-mandibular joint disorders and facial neuralgias. As a rule, irradiation by microwaves is only a supporting treatment. Its

Jaundice following oral surgery: Gilberts syndrome.

Jaundice developed in two patients following dental operations under general anaesthesia. Gilbert's Syndrome was established as the diagnosis. Caloric deprivation at the time of surgery resulted in the increased plasma concentration of unconjugated bilirubin. The pathogenesis of this not uncommon disorder and the mechanism of fasting hyperbilirubinaemia is discussed.
