Therapeutic progress--review XXIII. Morphine analgesia in terminal care.

Pain as a problem in terminal care is considered in this review with particular emphasis on the use of opiates. The choice of routes of administration is discussed in detail and the role of combination of opiates with anti-emetics and steroids is considered.

Second-trimester termination of pregnancy: nursing care.

Termination of pregnancy because of fetal abnormalities is a physically and emotionally painful event. Prostaglandin E2 (PGE2) intravaginal suppositories are an effective method for inducing labor. Patient care and pain management require both knowledge and sensitivity on the part of the nurse.

Experience with a hospice-care program for the terminally ill.

A hospice-care program offers an opportunity to provide effective palliative care for patients terminally ill with malignant disease and to develop improved methods for coping with the problems of the dying patient. All patients for whom antitumor therapy does not offer a reasonable possibility of cure are eligible for Church Hospital's multidisciplinary program, the focus of which is on both the patient and his family. Acceptance by medical staff, patients and families has been enthusiastic. Both conventional and unconventional methods can be helpful in making terminally ill patients more com

Home care of the frail elderly and the terminally ill.

This article discusses general principles of managing both elderly and terminally ill patients. The decision to care for the chronically sick in their homes is necessarily a joint one, made by the patient and supported by his or her family and physician. The physician must understand the patient's medical problems, and determine that the home environment is conducive to care and that one family member is available as a fulltime caregiver. The physician can call upon other professionals, including physiotherapists, pharmacists, dietitians, social workers, and clergymen, to help him care for the

Terminal ulcers in end-of-life care: a scoping review.

BACKGROUND: In the final stage of life, the skin may fail, leading to the development of terminal ulcers. These ulcers are considered unavoidable, since they occur despite all pressure-relieving interventions being implemented and the best quality of care being provided. However, they are often misdiagnosed as pressure injuries, since healthcare professionals are not always adequately prepared for this topic. Thus, effective palliative care and palliative wound care is not provided and prevalence data is missed, despite it being considered a solid indicator of the quality of nursing care. Sinc

Terminal sedation: an emotional decision in end-of-life care.

A patient with end-stage motor neurone disease was admitted for hospice care with worsening bulbar symptoms. Although he initially walked onto the ward he became very distressed and asked for sedation. After much discussion, this man was deeply sedated, and after some harrowing days, died. Was it right to provide terminal sedation? What should the threshold be for such treatment? How should our personal reservations affect how we approach the distressed patient in an end-of-life situation?

[Terminal care of cancer patients using an implanted venous reservoir].

Eighty-two terminal cancer patients were treated with implanted venous reservoirs using a Groshong catheter. Sixty-seven out of 82 (77%) patients were temporarily discharged or stayed at home on weekends. The mean hospital stay of these implanted patients at death was shorter than that of non-implanted patients (31 +/- 44 vs 51 +/- 46 days, p = 0.1187). Laboratory data of the blood drawn through the port showed no significant difference from the peripheral blood, meaning a potential benefit in reducing the number of punctures. This reservoir also allowed the patients infusion-free time and bat

Which patients with terminal cancer are admitted from home care?

The factors related to admission of patients with terminal cancer who had been referred to a reputable home care service were examined in 415 patients referred in a two-year period and in a prospective study of a randomized one in three sample of the 232 adults still alive one week after referral, who were able to converse and be at home with caring relatives. The reasons given by staff for intermediate admissions were mostly to improve symptom control or provide respite; for final admissions the reasons were symptom control, patients' deteriorated state and relatives needing relief. [Compassionate care for terminally ill term and preterm infants].

Involvement of the family in decisions to withhold or withdraw intensive care and parental involvement in care planning for terminally ill infants does not aggravate or prolong parents' grief responses, their feelings of guilt, or the incidence of pathological grief responses. Effective physical pain and symptom management is critically important. Compassionate care plans, however, need to implement a number of other and equally important components. Parents are not uniform in their perceived needs to make various kinds of contacts with their dying infant. They should be allowed to make their

Impact of Artificial Intelligence on the Care of Terminally Ill Patients.

Introduction: In recent decades, demographic aging has led to an inversion of the population pyramid, with a marked increase in the proportion of older adults. This shift has been accompanied by a higher prevalence of chronic and life-limiting diseases, while there have also been significant technical and scientific advances. However, these developments have not been matched by a proportional expansion of healthcare human resources, including in palliative care (PC). Consequently, healthcare systems face increasing pressure, particularly in the provision of end-of-life care. Artificial intelli

Terminal events in women dying of advanced breast cancer: improving the care for terminal breast cancer patients in Africa.

We studied the terminal events preceding death in all patients dying in hospital over a period of 10 years. Hepatomegaly, massive ascites, cachexia, jaundice, massive pleural effusion and haemoptysis were the most frequent terminal events. The predominance of visceral-related terminal events is unexplained but may be related to site-specific metastasis and premorbid organ pathology.

Loading phenobarbital in paediatric home-based terminal care: A case series.

BACKGROUND: There is a lack of evidence to guide the administration of phenobarbital for managing refractory symptoms in paediatric palliative care patients at home during the terminal phase. CASE PRESENTATION: Three terminal home-based paediatric cases with refractory symptoms that caused significant distress to both the child and family during the final days of life. All three cases had several changes in medications before receiving phenobarbital. CASE MANAGEMENT: The first case had a loading dose of phenobarbital based on an adult guideline; the second case commenced a continuous phenoba

End-of-life care for terminal head and neck cancer patients.

The objective of this study was to describe end-stage disease in patients suffering from advanced head and neck cancer (hnc). It is one of t … in dignity. The short survival time in hospice indicates that incurable patients may be referred to terminal care institutions too late in their dying trajectory. Physicians should be encouraged to talk to their terminally ill patients about the status and incurability of the disease as early as possible to facilitate patient and family decision making on how t …

Effects of Oral Care in Terminal Cancer Patients Shortly Before Death.

OBJECTIVE: We aimed to examine the effects of oral care in patients with terminal cancer approaching death. BACKGROUND: Patients with terminal-stage cancer often experience symptoms like oral dryness and candidiasis. Previous studies showed that without professional dental care, oral hygiene significantly worsens within 3 weeks before death. Therefore, oral management is essential for patients with terminal cancer. However, the specific effects of oral hygiene in these patients remain unclear. MATERIAL AND METHODS: Twenty patients were admitted to two palliative care units at our institution
